Sensitivity to hymenoptera in adult males.

The authors studied the sera of 86 selected adult male office workers for IgE antibodies to yellow jacket and honey bee venoms and to environmental inhalants and foods. Structured histories provide information about the experiences of these men with the Hymenoptera and other immunological information. Three men (3%) gave a history of a local or general reaction to the Hymenoptera but no antibodies were detected. Ten men (12%) had IgE antibodies to yellow jacket or honey bee venoms but none gave a history of a reaction. Nine of these men were atopic. The authors speculate that IgE antibodies to

Accelerated Hymenoptera testing in pediatric patients.

BACKGROUND: Skin testing is an important component of evaluating Hymenoptera sensitization in patients presenting with a suspected Hymenoptera systemic reaction. Accelerated skin testing protocols have been reported in adults, but they have not been evaluated in children. OBJECTIVE: Our aim was to assess the safety of an accelerated Hymenoptera testing protocol in the evaluation of Hymenoptera venom hypersensitivity systemic reactions in pediatric patients. METHODS: A retrospective chart review of pediatric patients evaluated for Hymenoptera venom allergy at an academic allergy clinic from J

[Toxic reaction induced by Hymenoptera stings].

Clinical symptoms of toxic reactions occurred in two patients following multiple stinging by bees and wasps respectively. The first cause is that of 76-year old woman attacked by a swarm of bees (about 200 stings), the other case presents a 69-year old man who was stung by several dozen of wasps. In the first case the toxic reaction was manifested by shock, acute renal failure, tissue damage of the skin, muscles and liver and haemolysis which resulted in the patients death. These symptoms occur as a results of the cytotoxic effects of bee venom components such as melittin, phospholipase and ki

Urine protein excretion after Hymenoptera sting.

The urinary excretion of albumin was measured on 7 consecutive days after a bee or wasp sting in 20 healthy persons, using a semiquantitative immunochemical chromatographic procedure (Micral stick). The mean albumin excretion was 11.1 +/- 15.0 (SD) mg/l. This was not significantly different from the findings in a control group of 17 volunteers with no history of bee or wasp sting (mean 8.7 +/- 17.4 mg/l). Pathologic albumin excretion (50-100 mg/l) was found in three of the sting group, with mean 37.2 +/- 23.7 mg/l. After 2 more months the urinary albumin excretion had normalized in two of the

Hyper-accelerated immunotherapy with hymenoptera venoms. Immunotherapy with hymenoptera venoms can be done in 3.5 to 6 hours, in a hospital environment with the precaution of having made a threshold of cutaneous sensitivity. The 3.5 h technique should only be used for subjects who are weakly sensitive to wasp venom. The 6 h technique should be used for: Subjects with weak sensitisation to been and hornet venom. Those who are strongly sensitive to wasp venom. The 3 day technique should be used for those who are strongly sensitive to been and hornet venom. This division of the indications is ess

[Allergy to Hymenoptera venom (author's transl)].

The venom injected with the sting of female Hymenoptera may be very dangerous to man. It contains toxic compounds similar to those of snake venom but these only affect men in case of numerous stings at the same time. In contrast, about 25% of the population may become sensitized to venom proteins (enzymes), the least common, but most serious, allergic reactions being anaphylactic shock, rarely fatal. Diagnosis rests upon questioning, skin tests with Hymenoptera venom and immunological investigations for specific IgE and IgG, the latter being considered as protective antibodies. First-aid treat

Hymenoptera Venom Immunotherapy Meets Factitious Disorder.

Factitious disorder on self is a psychiatric disorder in which individuals fabricate or induce signs or symptoms of a disease. Factitious anaphylaxis, with symptoms suggestive of a life-threatening allergic reaction, is extremely rare. Several cases of factitious disorder reactions during allergen immunotherapy for airborne allergens have been reported. We report the case of a young female patient who presented factitious anaphylaxis during venom immunotherapy to vespid venom extract. Symptoms of stridor, dyspnea, coughing and loss of consciousness were observed during the built-up phase of ve

Risk factors in Hymenoptera venom allergy.

. Risk factors should be part of the decision, of which patient should be offered venom immunotherapy (VIT) and how VIT should be performed. Risk factors for a severe systemic anaphylactic reaction (SAR) after a Hymenoptera field sting include a preceding less severe sting reaction, a wasp sting, an increased baseline serum tryptase concentration (BSTC), mastocytosis, older age, ACE inhibitor medication, and male gender. During VIT, treatment with honey bee venom is the most important risk factor for a SAR. Further risk factors include a high BSTC (for vespid VIT only), presence of venom speci

Basophil-activation tests in Hymenoptera allergy.

PURPOSE OF REVIEW: Despite recent advances in our understanding of basophil biology and discovery of new markers for basophil activation, tests measuring basophil activation are not widely utilized in Hymenoptera allergy. Studies of the basophil-activation test in Hymenoptera allergy were examined and the clinical utility of this test was assessed. RECENT FINDINGS: It has been demonstrated that the results of basophil-activation tests correlate quite well with those of serum IgE testing or skin-prick tests. Many studies compare test outcomes with history in patients and nonallergic controls,

Ventricular fibrillation after a hymenoptera sting.

A 54-year-old male, who had presented 2 ventricular fibrillation (VF) episodes with post-anoxic coma and later a return to consciousness and to a sinusal rhythm after defibrillation, have been placed a ventricular intracavitary stimulator (AICD). Since all the tests including the coronarygraphy have been found normal, we have paid attention to the anamnesis revealing an Apis mellifera sting before both VF episodes, previously considered irrelevant for the absence of relevant local symptoms. Intracutaneous tests were negative to Vespula sp. and positive to A. mellifera extract at a concentratio

Hymenoptera venom immunotherapy and field stings.

BACKGROUND: Anaphylactic sting reactions in patients with Hymenoptera venom allergy are prevented by venom immunotherapy (VIT) in most patients. OBJECTIVE: The aim of this study was to investigate re-sting reactions in the field during VIT or during an observation period of up to 13 years after cessation of treatment. Furthermore we sought to identify patients at higher risk of developing a systemic allergic reaction (SAR) and to assess possible correlations with basal serum tryptase concentration. METHODS: The clinical data of 192 patients with a recorded field sting during VIT were evaluat

Clinical consequences of toxic envenomations by Hymenoptera.

Many familiar Hymenoptera are brightly colored and can sting painfully-thus, their threat and clinical importance may be exaggerated. Most stinging insects only sting to defend themselves or their colonies from predators. The clinical nature of Hymenoptera envenomations contrasts that of other venomous animals, including other arthropods, primarily because allergic reaction, not direct intoxication, is the usual main concern. This review focuses mainly on the clinical features of direct toxicity to Hymenoptera envenomations, which can induce a high incidence of acute renal failure, liver failu

Diagnosis and treatment of allergy to hymenoptera venoms.

PURPOSE OF REVIEW: This review overviews select advances reported in the literature from mid-2008 to the present in the diagnosis and management of individuals with immediate-type hypersensitivity to hymenoptera venom(s). RECENT FINDINGS: Skin testing remains the principal confirmatory test for sensitization to hymenoptera venoms. Use of dialyzed extracts in North America could enhance the sensitivity of intradermal skin testing. Hymenoptera venom components are overviewed with focus on their use in IgE antibody microarray assays to distinguish true dual sensitization to yellow jacket venom (

Hymenoptera anaphylaxis and mast cell activation disorders.

PURPOSE OF REVIEW: Among patients with hymenoptera venom allergy (HVA), a high proportion have an underlying systemic mastocytosis. It is essential to identify them for an adequate management. RECENT FINDINGS: The clinical presentation of anaphylaxis after stinging -cardiovascular symptoms and absence of cutaneous- may point to a clonal mast cell disease (MCD). In addition, these patients usually have low mast cell burden, so highly sensitive techniques are needed to detect clonality. SUMMARY: It is important to recognize patients with HVA associated with a clonal MCD since the handling of i

Basophil activation test in Hymenoptera venom allergy.

Before starting venom-specific immunotherapy (VIT), systemic sting reactions to Hymenoptera venoms require allergological workup in order to prove an IgE-mediated reaction and to identify the culprit insect venom. In addition to skin tests and the determination of specific IgE antibodies, the basophil activation test (BAT) using flow cytometry has emerged as a powerful tool and sensitive marker for this purpose in recent years. BAT seems to have a better informative value in terms of clinical relevance compared to the other tests. In Hymenoptera venom allergies, BAT is particularly useful for

Multiscale head anatomy of Megaphragma (Hymenoptera: Trichogrammatidae).

Methods of three-dimensional electron microscopy have been actively developed recently and open up great opportunities for morphological work. This approach is especially useful for studying microinsects, since it is possible to obtain complete series of high-resolution sections of a whole insect. Studies on the genus Megaphragma are especially important, since the unique phenomenon of lysis of most of the neuron nuclei was discovered in species of this genus. In this study we reveal the anatomical structure of the head of Megaphragma viggianii at all levels from organs to subcellular structur

[Hymenoptera venom allergy: what's new in 2023].

Hymenoptera venom allergy is a central thematic in allergology. The recent limitation in the obtention of certain venom products has forced Swiss centers to adapt their diagnostic and therapeutical approaches. In this review, we will discuss diagnostics tools using recombinants serologies, recent recommendations for the screening of indolent systemic mastocytosis and the different immunotherapy protocols available for venom desensitization using aqueous and aluminum hydroxide-adsorbed purified venoms.

Hymenoptera Allergy and Mast Cell Activation Syndromes.

Mast cell activation syndrome (MCAS) can be diagnosed in patients with recurrent, severe symptoms from mast cell (MC)-derived mediators, which are transiently increased in serum and are attenuated by mediator-targeting drugs. When KIT-mutated, clonal MC are detected in these patients, a diagnosis of primary MCAS can be made. Severe systemic reactions to hymenoptera venom (HV) represent the most common form of anaphylaxis in patients with mastocytosis. Patients with primary MCAS and HV anaphylaxis are predominantly males and do not have skin lesions in the majority of cases, and anaphylaxis is
