The superior hypogastric plexus (SHP) is a complex nervous collection located at the lumbosacral region below the level of the aortic bifurcation. As a part of the autonomic nervous system, it is an extension of the preaortic plexuses and continues bilaterally as the hypogastric nerves that ultimately contribute to the inferior hypogastric plexus. Although commonly described as a plexiform structure, several morphologic variations exist. BACKGROUND: Superior hypogastric plexus (SHP) neurolysis is a well-established intervention for pelvic cancer pain; however, lumbosacral transitional vertebrae (LSTV) can present significant technical challenges during fluoroscopically guided procedures.
CASE REPORT: A 70-year-old patient with refractory pelvic pain secondary to advanced cervical cancer and LSTV underwent successful neurolysis of the SHP and ganglion impar. A modified transdiscal approach was performed under fluoroscopic guidance, following a detailed preprocedural imaging analysis to account for the anatomical variation.

METHODS: A patient with severe chronic nonmalignant penile pain after transurethral resection of the prostate underwent a single superior hypogastric plexus block with local anesthetic and steroid. The superior hypogastric plexus (SHP) is the part of the autonomic nervous system, which is responsible for the sympathetic innervation of pelvic organs and extrapelvic genitals in humans of both sexes. The SHP also functions as the anatomic pathway for the major part of visceral sensitive fibers originating from pelvic viscera. In this study, the morphology of the SHP was analyzed through anatomical dissections performed both in human adult and fetal cadavers. A computerized morphometrical investigation of the SHP was also performed and the resulting quantitative data statistically assessed.
