Isotonic saline solution as maintenance intravenous fluid therapy to prevent acquired hyponatremia in hospitalized children.

OBJECTIVES: This review aims to evaluate if the use of hypotonic saline solutions as maintenance intravenous fluid therapy in hospitalized children increases the risk of hyponatremia, if the administration of isotonic fluids is able to protect against acquired hyponatremia and if the isotonic solutions increase the risks of deleterious effects such as hypernatremia or fluid overload. SOURCES: We researched the relevant literature on the PubMed (Jan 01 1969 to Jul 13 2011), EMBASE (1989 to 2011) and Cochrane Library (1989 to 2011) databases. Furthermore, references of selected studies were inc

Topical therapy for dry eyes.

'Artificial tear' solutions, perhaps more accurately termed 'ocular wetting' solutions, have been the mainstay of topical therapy for keratoconjunctivitis sicca (KCS). These solutions have been designed with water-soluble polymers in an attempt to increase their ocular retention time or ability to wet the ocular surface. Treatment with ocular wetting solutions has been non-specific in that there is no evidence that dry spot formation is essential to the induction of surface disease in KCS patients, nor is there evidence that water-soluble polymers are beneficial to epithelium. In KCS, tear fil

Reduction in water permeability of the rabbit conjunctival epithelium by hypotonicity.

The effects of unilateral exposure to hypotonic media on the diffusional water permeability of the isolated rabbit conjunctiva were determined. For these experiments, a segment of the bulbar-palpebral conjunctiva was mounted between Ussing-type hemichambers under short-circuited conditions. Unidirectional diffusional water fluxes (Jdw) were measured in either direction by adding 3H2O to one hemichamber and sampling from the other. Electrical parameters were measured simultaneously. Jdw were determined in control isosmotic conditions and after dilution of one of the bathing solutions from 290 t

Introducing TOPMAST, the first double-blind randomized clinical trial specifically dedicated to perioperative maintenance fluid therapy in adults.

BACKGROUND: Although prescribed to every patient undergoing surgery, maintenance fluid therapy is a poorly researched part of perioperative fluid therapy. The tonicity of the chosen solutions, could be an important cause of morbidity, with hyponatremia being a potential side effect of hypotonic solutions, where isotonic solution could lead to fluid overload. METHODS: The TOPMAST-trial is an ongoing prospective single-center double-blind randomized trial comparing an isotonic and a hypotonic maintenance fluid strategy during and after surgery in patients undergoing different types of major tho

Hypotonic resistance of boar spermatozoa: sperm subpopulations and relationship with epididymal maturation and fertility.

Hypotonic resistance of boar spermatozoa was investigated by measuring the ratio of live/dead spermatozoa (SYBR-14/propidium iodide) by flow cytometry after hypotonic stress. The survival rate of ejaculated spermatozoa incubated in hypotonic solutions ranging from 3 to 330 mmol/kg followed a sigmoid curve that fitted a simple logistic model. The critical osmolality value (Osm(crit)) at which 50% of spermatozoa died was determined with this model. Hypotonic resistance of spermatozoa increased with temperature between 15 and 39 degrees C and decreased after hydrogen superoxide treatment, but was

TRPV4 channels mediate exacerbated response to mechanical cues in spontaneously hypertensive rats.

Upon hypoosmotic stimulation, cardiomyocytes undergo a transient positive inotropic effect (Pie) associated with an increase in the amplitude of intracellular Ca2+ transients. However, the underlying mechanisms remain elusive. The Transient Receptor Vanilloid 4 channel (TRPV4) promotes Ca2+ entry and, thus, could contribute to hypotonic swelling-induced Pie. TRPV4 have not been studied in spontaneously hypertensive rats (SHRs). We aimed to determine if TRPV4 contributes to swelling-induced Pie in Wistar rats and if this response is altered in SHR. Cardiomyocytes were isolated from 8 to 12-mont

Chloride channels regulate HIT cell volume but cannot fully account for swelling-induced insulin secretion.

Insulin-secreting pancreatic islet beta-cells possess anion-permeable Cl- channels (I(Cl,islet)) that are swelling-activated, but the role of these channels in the cells is unclear. The Cl- channel blockers 4,4'-diisothiocyanostilbene-2,2'-disulfonic acid (DIDS) and niflumic acid were evaluated for their ability to inhibit I(Cl,islet) in clonal beta-cells (HIT cells). Both drugs blocked the channel, but the blockade due to niflumic acid was less voltage-dependent than the blockade due to DIDS. HIT cell volume initially increased in hypotonic solution and was followed by a regulatory volume dec

[Severe hyponatremic encephalopathy after pediatric surgery: report of seven cases and recommendations for management and prevention].

BACKGROUND: The infusion of hypotonic solutions in the perioperative period can lead to the development of hyponatraemic encephalopathy which is a severe, life threatening but preventable complication. CLINICAL PRESENTATION: Seven children aged 3-6 yr, ASA 1 or 2, operated on for a scheduled surgical procedure, presented at the 11th postoperative hour with seizures or status epilepticus, associated with vomiting (5/7), and a constant loss of consciousness (median Glasgow Coma Scale at 7), while one child presented with a respiratory arrest. At arrival in ICU, serum sodium was 120 mmol.L-1. Al

Treatment of gastrointestinal viruses.

The most common enteric viruses responsible for diarrhoea are rotavirus, enteric adenoviruses, caliciviruses including the Norwalk agent and astrovirus. These infections are usually mild to moderate in severity, self-limiting and of short duration and thus, specific antiviral therapy is not recommended. The standard management of these infections is restoration of fluid and electrolyte balance and then maintenance of hydration until the infection resolves. WHO oral rehydration therapy (ORT) was introduced about 30 years ago and has saved the lives of many infants and young children. During the

A decrease in renal medullary tonicity stimulates anion transport in Henle's loop of rat kidneys.

To investigate the effect of reduction in renal medulla osmolality on loop of Henle (LOH) net bicarbonate reabsorption, clearance and microperfusion experiments were performed on Sprague-Dawley rats. The decrease of renal medulla osmolality was induced by intravenous infusion of either a large dose of mannitol (mannitol protocol) or a hypotonic solution (hypotonic protocol) delivered at a rate to match the sodium and bicarbonate load of the control period. During the mannitol protocol, clearance data demonstrated a rise in glomerular filtration rate (GFR), renal plasma flow, urine pH, and frac

Effects of osmotic stress on rabbit corneal endothelium.

The effects of osmotic stress on corneal endothelium were investigated by exposing rabbit corneas to anisosmotic conditions, and then perfusing the corneas with isosmotic glutathione bicarbonate Ringer solution for 4 hr at 35 degrees C. During the perfusion, endothelial function was assessed by measuring corneal thickness with a specular microscope. After perfusion, the corneas were prepared for scanning and transmission electron microscopy. Endothelial ultrastructure and function were well maintained after exposure to a wide range of osmolality (0.12-2.7 osmol/kg), but this tolerance of osmot

Cardiovascular effects of hypertonic sodium chloride solutions when injected into the liquor space of anaesthetized cats.

1 In cats anaesthetized with chloralose, hyper- and hypotonic solutions were injected into the cisterna magna (in 0.5 ml) or into a lateral cerebral ventricle (in 0.2 to 0.3 ml), with aqueduct cannulated to prevent the injected solution from entering the subarachnoid space, and the effects on blood pressure and heart rate were examined. 2 Cisternal injections of hyper- and hypotonic solutions of NaCl (0.51 M and 0.05 M), glucose (1.03 M and 0.10 M), or sucrose (1.02 M and 0.10 M), as well as distilled water produced a rise in arterial blood pressure with tachycardias. Treatment of severe hyponatremia with continuous renal replacement therapy: A case and review of corrective strategies.

Treatment of severely hyponatremic patients with continuous renal replacement therapy (CRRT) presents a unique challenge given the lack of commercial options for hypotonic replacement solutions or dialysate. We report the case of a 55-year-old male who presented with profound, symptomatic hyponatremia in the setting of acute kidney injury (AKI). The patient was found to have a serum sodium concentration of 97 mEq/L because of free water retention that occurred during severe AKI from viral gastroenteritis and rhabdomyolysis. Continuous veno-venous hemofiltration (CVVH) was required for AKI comp

Possible role of basolateral cell membrane in proximal renal tubule osmoregulation.

The cellular response to hypotonic stimulation was studied with videometric methods in 266 proximal renal tubules dissected from Carassius auratus (goldfish). Ringer Lactate Versus Hybrid Isotonic/Hypotonic Saline in Delayed Graft Function in Deceased-donor Kidney Transplantation: A Real-world Protocol Change Study.

BACKGROUND: Delayed graft function (DGF) remains a major complication after deceased-donor kidney transplantation. Balanced crystalloid solutions have been associated with lower DGF incidence compared with 0.9% saline, largely based on strategies using Plasma-lyte. However, the role of Ringer lactate compared with other low-chloride solutions, such as hypotonic saline, remains unclear. We therefore evaluated whether a protocol-driven change in perioperative crystalloid strategy was associated with DGF and early posttransplant kidney function. METHODS: We conducted a retrospective, single-cent

Dynamic changes in neuronal volume resulting from osmotic and sodium transport manipulations.

Electrophysiologic parameters such as input resistance and response to microperfusion of neurotransmitters vary under circumstances where neurons from isolated ganglia of Aplysia californica are subjected to either long-term (several hours) blockade of active sodium transport or to hypo- or hyperosmotic solutions. Since one of multiple possible events under these circumstances is neuronal volume changes, we have developed a system using cultured Aplysia neurons and confocal scanning laser microscopy to directly monitor cell volume when the osmolarity of the perfusing solution is altered and wh
