Gross vulval edema after laparoscopic instillation of 4% icodextrin solution.

Severe vulval edema after laparoscopic ovarian cystectomy with instillation of adhesion-prevention solution (4% icodextrin) is a rare complication. A 17-year-old girl was readmitted to the hospital 2 days after laparoscopic ovarian cystectomy with massive vulval swelling, pain, and anxiety. Gross swelling was potentially caused by accumulation of fluid 4% icodextrin in the labia. The condition resolved spontaneously with application of ice packs and bed rest. Possible mechanisms of this complication are discussed as is the importance of adequate counseling and consent before surgery when adhes

Mesothelial dysplastic changes and lipid peroxidation induced by 7.5% icodextrin.

BACKGROUND: The issue of icodextrin biocompatibility is somehow ambiguous. Whereas some experimental data point at better bicompatibility of icodextrin compared with high glucose concentration fluid, other reports showed substantial cytotoxic effects upon monocytes and cultured mesothelial cells. The present investigation exposes the first attempt to investigate the biocompatibility issue in an in vivo and in situ setup. METHODS: Mice were intraperitoneally injected once a day with the 7.5% icodextrin solution, during 30 consecutive days. Imprints of the mesothelial monolayer covering the ant

Impact of fill volume on ultrafiltration with icodextrin in children on chronic peritoneal dialysis.

BACKGROUND: Icodextrin is a solution of glucose polymers developed to provide sustained ultrafiltration over an extended dwell. Our aim was to determine whether or not fill volume with icodextrin contributes to the ability to achieve ultrafiltration in children. METHODS: The charts of all children on chronic peritoneal dialysis between January 2000 and July 2014 were screened for the use of an icodextrin day dwell. Data were extracted from the electronic chart and the HomeChoice™ Pro card and corrected for body surface area (BSA). RESULTS: Fifty children had an icodextrin day dwell. A linear

Influence of 4% icodextrin solution on peritoneal tissue response and adhesion formation.

BACKGROUND: Postoperative peritoneal adhesion formation following abdominal surgery remains a relevant surgical problem. The application of soluble physico-chemical barriers like 4% icodextrin is one approach to protect the peritoneal surface from getting linked to adhesive scar. The aim of this study was to investigate the influence of 4% icodextrin on peritoneal tissue response both of visceral and parietal peritoneum, adhesion formation and wound healing. METHODS: 40 rats were divided into two groups. After creation of an intraabdominal defect, either 4% icodextrin (Adept®) or sodium chlor

Sodium removal in peritoneal dialysis: the role of icodextrin and peritoneal dialysis modalities.

One of the main goals of dialysis is the control of extracellular volume, because inadequate sodium and fluid removal result in fluid overload and increased mortality. In the present study, we evaluated the roles of continuous ambulatory peritoneal dialysis (CAPD), continuous cycling peritoneal dialysis (CCPD), and the use of icodextrin on sodium removal in 29 patients (n = 18 on CAPD, n = 11 on CCPD). Daily removal of sodium by each modality and dialysis adequacy by Kt/V and creatinine clearance were evaluated. A significantly higher amount of sodium was removed in CAPD patients than in CCPD

Icodextrin peritoneal transport in vitro: effect of sodium deoxycholate, glucose, and methylglyoxal.

The aim of the in vitro studies was to examine the effect of sodium deoxycholate, glucose, and methylglyoxal on icodextrin peritoneal transfer. The rabbit peritoneum in a modified Ussing chamber was an experimental model. Transport and morphometric analyses were performed. In the first of them, the icodextrin (7.5 g/dL) diffusion from the mesothelial to the interstitial side of the membrane, expressed as a diffusive permeability coefficient (P), was evaluated in the control stage, after chemical modification of the membrane using sodium deoxycholate (104 mg/dL), after the addition of glucose (

Predictors of a favourable response to icodextrin in peritoneal dialysis patients with ultrafiltration failure.

BACKGROUND AND AIMS: Icodextrin is a starch-derived glucose polymer that causes sustained ultrafiltration in long dwells in peritoneal dialysis. The aim of this study was to assess factors that were predictive of an increment in ultrafiltration following the introduction of icodextrin in patients with refractory fluid overload. METHODS: Thirty-nine patients (20 male/19 female, mean age 57.7 +/- 2.4 years) on peritoneal dialysis were enrolled in a prospective pretest/post-test, open-label study. All patients had symptomatic fluid overload refractory to fluid restriction (<800 mL/day), frusemid

An exploratory assessment of icodextrin for ultrafiltration failure subtypes in peritoneal dialysis.

Although icodextrin was initially developed for rapid transporters (type 1 UFF), emerging evidence suggests its effects might be independent of peritoneal membrane transport characteristics. This study aimed to evaluate the suitability of 7.5% icodextrin for all PD patients with UFF. We enrolled 19 patients with UFF and 24 non-UFF PD controls. All participants underwent standard, modified, Mini-, and Double-Mini peritoneal equilibrium tests (PET). Key parameters assessed included sodium sieving (∆DNa60, ∆D/PNa60), osmotic conductance to glucose, small-pore ultrafiltration (UFSP), free water tr

Stability of Daptomycin in Dextrose and Icodextrin-Based Peritoneal Dialysis Solutions.

Background: With emerging antibiotic resistance, many patients on peritoneal dialysis require newer antibiotic treatment such as daptomycin. Inadequate clinical information exists across different peritoneal dialysis solutions, including icodextrin, for the stability of intraperitoneal daptomycin. To guide the clinical practice of intraperitoneal daptomycin treatment, we need to establish the stability of daptomycin at dextrose concentration higher than 1.5% and icodextrin, as well as the duration of stability. Methods: We tested the stability of daptomycin in three types of peritoneal dialysi

Icodextrine and insulin resistance in continuous ambulatory peritoneal dialysis patients.

Insulin resistance is commonly observed in uremic patients. Glucose-based peritoneal dialysis solutions have long-term metabolic complications like hyperinsulinemia, hyperlipidemia, and obesity. The purpose of this study was to examine the insulin resistance in patients undergoing continuous ambulatory peritoneal dialysis (CAPD) with standard glucose and icodextrin containing solutions. The entire non diabetic CAPD patients of our center were studied: forty-four patients in all who were on CAPD treatment for 36.2 +/- 23.7 months. Twenty-seven of them (11 male and 16 female) with a mean age of

Peritoneal transport dynamics of glucose and icodextrin: the in vitro comparative studies.

We performed in vitro experiments with the isolated rabbit parietal peritoneum to evaluate the importance of fluid stirring intensification and of chemical modification of mesothelium and interstitium to the peritoneal transport of glucose and icodextrin. We used a mathematical model of mass transport to calculate the diffusive permeability coefficient, P, in centimeters per second. In control conditions (intact tissue; stirring rate: 11 mL/min), the rate of glucose (2.0 g/dL) transfer remained constant, and no differences were observed for transport from the interstitial to the mesothelial (I

Icodextrin lowers serum sodium in dose-dependent fashion: A case report.

Icodextrin (ICO) is widely used in peritoneal dialysis (PD). Hyperosmolar hyponatremia (hypoNa) has been reported as an adverse event of ICO, but it is unknown whether there is a dose effect relationship between the two. We present a case demonstrating this relationship. A 61-year-old woman with end-stage kidney disease (ESKD) and no history of diabetes, began PD with a single daily dwell of 1L of ICO. Her serum Na during the previous 6 months (n = 16) was 138.9 (mmol/L) ± 2.2 (SD). Within days of starting PD, the Na fell to 132 and over the ensuing 9 months (n = 27) it averaged 133.6 ± 2.1. A

Effect of Neutral pH Icodextrin Peritoneal Dialysis Fluid on Mesothelial Cells.

Conventional acidic icodextrin peritoneal dialysate (CI) has low biocompatibility due to its low pH, and a neutral pH icodextrin dialysate (NI) was developed. The influence of NI on the peritoneum has not been clarified. The effects of the two dialysates on cultured rat mesothelial cells were examined. CI, but not NI, increased α-smooth muscle actin, collagen type 1 and 3, and P21 mRNA expressions. CI with neutralized pH did not improve these harmful effects. With NI+ glucose degradation products (GDPs: same concentration as CI), mRNA expressions were comparable to those with NI alone. However

Adhesion-preventing properties of 4% icodextrin and canola oil: a comparative experimental study.

OBJECTIVE: Postsurgical abdominal adhesions are common, serious postoperative complications. The present study compared the usefulness of 4% icodextrin and canola oil in preventing postoperative peritoneal adhesions. METHODS: Twenty-four Wistar albino rats were divided into three groups. Following a laparotomy, a serosal abrasion was made by brushing the cecum, and 3 mL of 0.9% NaCl, 4% icodextrin, or 3 mL of canola oil were intraperitoneally administered for the control, icodextrin, and canola oil groups, respectively. The abdomen was then closed. All of the rats were sacrificed at day 10. M

Icodextrin-induced acute generalized exanthematous pustulosis in a patient with peritoneal dialysis.

Icodextrin has been widely prescribed for peritoneal dialysis (PD) patients with inadequate ultrafiltration, but icodextrin induced acute generalized exanthematous pustulosis (AGEP) has been not well recognized in clinical practice. We described a young-aged female with IgA nephropathy and end stage kidney disease under continuous automated peritoneal dialysis. She developed skin erythema with exfoliation over the groin 7th day after initiation of icodextrin based PD dialysate. Initially, her scaling skin lesion with pinhead-sized pustules affected the bilateral inguinal folds, and then it ext

The role of icodextrin in peritoneal dialysis: protocol for a systematic review and meta-analysis.

BACKGROUND: Previous meta-analyses have found several advantages of icodextrin compared with glucose in the application of peritoneal dialysis (PD), such as an improvement of peritoneal ultrafiltration during the long dwell and a reduction in episodes of uncontrolled fluid overload. However, the effect of icodextrin on patient-relevant outcomes remains unclear. This review aims to evaluate the benefits and harms of icodextrin in comparison with conventional glucose PD solution in patients with end-stage kidney disease receiving PD. METHODS: Randomized controlled trials of icodextrin comparing

Icodextrin reduces the risk of congestive heart failure in peritoneal dialysis patients.

PURPOSE: Icodextrin can enhance ultrafiltration and consequently improve fluid balance and can control blood pressure and reduce left ventricular mass for peritoneal dialysis (PD) patients. This study investigated whether icodextrin use could reduce the risk of congestive heart failure (CHF) for PD patients. METHODS: From the Taiwan National Health Insurance database, we identified 5462 newly diagnosed end-stage renal disease patients undergoing PD from 2005 to 2010. Incidence rates and hazard ratio of CHF were estimated for patients with and without icodextrin treatment by the end of 2011.

Effect of icodextrin on heart rate variability in diabetic patients on peritoneal dialysis.

INTRODUCTION: Spectral analysis of heart rate variability is a noninvasive method for evaluating autonomic cardiovascular dysfunction under various clinical conditions, such as in dialysis patients, in whom an imbalance between the sympathetic and parasympathetic nervous system appears to be an important risk factor for sudden cardiovascular death and arrhythmia. ♢ OBJECTIVE: We compared the effect of icodextrin-based dialysis solution, an option that allows for better metabolic and fluid overload control, with that of glucose-based dialysis fluid on sympathetic and parasympathetic activity i
