Persistent IFN-I signaling associated with the HIV-1 reservoir fuels immune exhaustion and reveals therapeutic targets.

The viral reservoir established after HIV-1 infection remains the primary barrier to curing HIV. Recent studies suggest that proviral HIV-1 DNA, low-level viral transcripts, and other reservoir-derived viral products may contribute to persistent innate immune activation and type I interferon (IFN-I)-related inflammatory programs. This sustained activation drives a state of chronic immune activation, which induces and maintains exhaustion in immune cells such as CD4+ T cells. Exhaustion is characterized by loss of effector function and upregulation of inhibitory receptors (immune checkpoints),

Unveiling the nexus between direct-acting antivirals in hepatitis C virus elimination and immune response.

The introduction of direct-acting antiviral (DAA) therapy has been a game-changer in the elimination of hepatitis C virus infection. DAAs treatment achieved higher rates of sustained virological response among HCV-infected individuals across different virus genotypes. DAAs directly target HCV viral several proteins in the HCV lifecycle resulting in controlling the infection. So far, the immune system also plays a crucial role in effective viral eradication. Prolonged antigen exposure, coupled with high viral loads, are key factors that drive immune system failure and the development of chronic

Re-Emergence of Dendritic Cell Vaccines for Cancer Treatment.

Dendritic cells (DCs) are essential in immunity owing to their role in activating T cells, thereby promoting antitumor responses. Tumor cells, however, hijack the immune system, causing T cell exhaustion and DC dysfunction. Tumor-induced T cell exhaustion may be reversed through immune checkpoint blockade (ICB); however, this treatment fails to show clinical benefit in many patients. While ICB serves to reverse T cell exhaustion, DCs are still necessary to prime, activate, and direct the T cells to target tumor cells. In this review we provide a brief overview of DC function, describe mechanis

Endometriosis immune microenvironment timing shifts: from immune escape to immune exhaustion.

Endometriosis(EMS) symptoms, progression, and onset are all linked to the patient's immune system dysfunction. Research on immune cells and their molecular mechanisms has long been a prime focus of EMS studies. Nonetheless, the immune environments of EMS patients and cells are constantly changing. How to effectively study and treat EMS depends on our ability to comprehend the temporal changes in immunity in EMS patients. Consequently, the immunological time sequence of endometriosis is methodically discussed and summarized in this article as four steps: 1) immune escape, in which Ectopic Endom

Single-cell landscape of primary central nervous system diffuse large B-cell lymphoma.

Understanding tumor heterogeneity and immune infiltrates within the tumor-immune microenvironment (TIME) is essential for the innovation of immunotherapies. Here, combining single-cell transcriptomics and chromatin accessibility sequencing, we profile the intratumor heterogeneity of malignant cells and immune properties of the TIME in primary central nervous system diffuse large B-cell lymphoma (PCNS DLBCL) patients. We demonstrate diverse malignant programs related to tumor-promoting pathways, cell cycle and B-cell immune response. By integrating data from independent systemic DLBCL and folli

Mitochondrial dysfunction in sepsis-induced immunoparalysis: from immune-cell metabolic reprogramming to clinical biomarkers.

Sepsis-induced immunoparalysis is a dynamic state of acquired immune dysfunction characterized by impaired antigen presentation, lymphocyte exhaustion, defective innate immune responses, and increased susceptibility to secondary infection. Increasing evidence suggests that mitochondrial dysfunction is a key metabolic mechanism underlying this immune failure. Rather than acting as a uniform injury signal, mitochondrial abnormalities affect immune-cell subsets in distinct ways: monocytes and macrophages lose antigen-presenting capacity, neutrophils develop impaired migration and antimicrobial ac

Manipulating Offense and Defense Signaling to Fight Cold Tumors with Carrier-Free Nanoassembly of Fluorinated Prodrug and siRNA.

Chemoimmunotherapy has shown great potential to activate an immune response, but the immunosuppressive microenvironment associated with T cell exhaustion remains a challenge in cancer therapy. The proper immune-modulatory strategy to provoke a robust immune response is to simultaneously regulate T-cell exhaustion and infiltration. Here, a new kind of carrier-free nanoparticle is developed to simultaneously deliver chemotherapeutic drug (doxorubicin, DOX), cytolytic peptide (melittin, MPI), and anti-TOX small interfering RNA (thymocyte selection-associated high mobility group box protein, TOX)

NK cell exhaustion in Wilson's disease revealed by single-cell RNA sequencing predicts the prognosis of cholecystitis.

Metabolic abnormalities associated with liver disease have a significant impact on the risk and prognosis of cholecystitis. However, the underlying mechanism remains to be elucidated. Here, we investigated this issue using Wilson's disease (WD) as a model, which is a genetic disorder characterized by impaired mitochondrial function and copper metabolism. Our retrospective clinical study found that WD patients have a significantly higher incidence of cholecystitis and a poorer prognosis. The hepatic immune cell landscape using single-cell RNA sequencing showed that the tissue immune microenviro

Harnessing Immune Rejuvenation: Advances in Overcoming T Cell Senescence and Exhaustion in Cancer Immunotherapy.

Immunotherapy has transformed the landscape of cancer treatment, with T cell-based strategies at the forefront of this revolution. However, the durability of these responses is frequently undermined by two intertwined phenomena: T cell exhaustion and senescence. While exhaustion is driven by chronic antigen exposure in the immunosuppressive tumor microenvironment, leading to a reversible state of diminished functionality, senescence reflects a more permanent, age- or stress-induced arrest in cellular proliferation and effector capacity. Together, these processes represent formidable barriers t

Role of the Complement System in the Modulation of T-Cell Responses in Chronic Chagas Disease.

Chagas disease, caused by the intracellular pathogen Trypanosoma cruzi, is the parasitic disease with the greatest impact in Latin America and the most common cause of infectious myocarditis in the world. The immune system plays a central role in the control of T. cruzi infection but at the same time needs to be controlled to prevent the development of pathology in the host. It has been shown that persistent infection with T. cruzi induces exhaustion of parasite-specific T cell responses in subjects with chronic Chagas disease. The continuous inflammatory reaction due to parasite persistence i

The R1441C-Lrrk2 mutation induces myeloid immune cell exhaustion in an age- and sex-dependent manner in mice.

Age is the greatest risk factor for many neurodegenerative diseases, yet immune system aging, a contributor to neurodegeneration, is understudied. Genetic variation in the LRRK2 gene affects risk for both familial and sporadic Parkinson's disease (PD). The leucine-rich repeat kinase 2 (LRRK2) protein is implicated in peripheral immune cell signaling, but the effects of an aging immune system on LRRK2 function remain unclear. We analyzed peritoneal macrophages from R1441C-Lrrk2 knock-in mice and observed a biphasic, age-dependent effect of the R1441C-Lrrk2 mutation on peritoneal macrophage func

Cluster of differentiation 36-engineered photosynthetic cyanobacteria enable immunometabolic reprogramming in triple-negative breast cancer.

Triple-negative breast cancer (TNBC) exhibits profound resistance to immunotherapy, largely due to a tumor microenvironment constrained by dysregulated lipid metabolism and persistent hypoxia. Excessive fatty acid uptake mediated by the scavenger receptor CD36 (Cluster of differentiation 36) imposes metabolic stress on immune cells while simultaneously fueling tumor growth, whereas hypoxia independently enforces immune exhaustion and macrophage immunosuppression. Despite extensive efforts, targeting either pathway alone has yielded limited therapeutic benefit, underscoring the need for coordin

High Levels of CD244 Rather Than CD160 Associate With CD8+ T-Cell Aging.

Aging leads to functional dysregulation of the immune system, especially T cell defects. Previous studies have shown that the accumulation of co-inhibitory molecules plays an essential role in both T cell exhaustion and aging. In the present study, we showed that CD244 and CD160 were both up-regulated on CD8+ T cells of elderly individuals. CD244+CD160- CD8+ T cells displayed the increased activity of β-GAL, higher production of cytokines, and severe metabolic disorders, which were characteristics of immune aging. Notably, the functional dysregulation associated with aging was reversed by bloc

Synergetic regulation of cancer cells and exhausted T cells to fight cold tumors with a fluorinated EGCG-based nanocomplex.

Immune therapy that targets PD-L1 (programmed cell death-ligand 1) is attractive to augment immune response by breaking the programmed cell death-1 (PD-1)/PD-L1 axis. However, T cell exhaustion associated with insufficient T cells infiltration may diminish the efficacy of cancer therapy. Here, we report a novel delivery system of FEGCG/FPEI@siTOX composed of fluorinated EGCG (FEGCG) and fluorinated polyethyleneimine (FPEI) for delivery of small interfering RNA anti-TOX (thymus high mobility group box protein, TOX) to treat tumor and metastasis. In this way, the reduction in PD-L1 expression by

Pan-cancer analysis reveals HDAC1 as a key regulator of immune infiltration and T cell exhaustion.

Histone deacetylases (HDACs) are critical epigenetic regulators that play pivotal roles in tumorigenesis and cancer progression. Class I HDACs have garnered significant attention due to their essential functions in cell proliferation and differentiation; however, their baseline expression patterns within the immune system and specific mechanisms of action in the tumor microenvironment (TME) remain incompletely understood. This study systematically analyzed the expression profiles of Class I HDAC family members in immune tissues and comprehensively investigated their expression levels, prognost

T cell exhaustion: Focus on molecular pathways.

T cell exhaustion developed as a safeguard against excessive immune damage, but it can impair the immune system's ability to combat infections and cancer. Scientists are focused on developing strategies to revitalize exhausted T cells and enhance immune responses. Research indicates that the state of T cells within tumors is closely linked to the outcomes of immune checkpoint blockade therapy. This manuscript reviews the complete molecular pathways concerning exhaustion triggered in cells due to transcription factor metabolic dysfunction, microRNA regulation, and the cytokine environment. Lymphocyte Suppression and Exhaustion, Conventional, and Accelerated.

The essential mammalian immune system lymphocytes include T cells, B cells, and natural killer cells. Any impairments of their functionalities can have severe consequences, since these lymphocytes each contribute as an interactive team in responding to pathogen infections or cancers. Such impairments include lymphocyte exhaustion, such as T cell, B cell, or natural killer cell exhaustion, or lymphocyte suppression impairing one or more of these cells. Lymphocyte exhaustion can have any intensity from mild to severe, having a severity scale worsened by various exposures and time periods of cons

The expression of CTLA-4 in hepatic alveolar echinococcosis patients and blocking CTLA-4 to reverse T cell exhaustion in Echinococcus multilocularis-infected mice.

Alveolar echinococcosis (AE) is a zoonotic parasitic disease caused by the infection of Echinococcus multilocularis (E. multilocularis) larvae. Cytotoxic T-lymphocyte antigen 4 (CTLA-4) produces inhibitory signals and induces T cell exhaustion, thereby inhibiting the parasiticidal efficacy of the liver immune system. Therefore, the purpose of this study is to explore how T-cell exhaustion contributes to AE and whether blocking CTLA-4 could reverse T cell exhaustion. Here we discovered that the expression of CTLA-4 was increased in the infiltrating margin around the lesion of the liver from AE

New treatments to reach functional cure: Rationale and challenges for emerging immune-based therapies.

The landscape for chronic HBV therapy is rapidly evolving. The latest generation of antiviral drugs provide robust virus suppression with a high barrier to resistance that facilitates long-term treatment. However, low rates of HBsAg loss demonstrate that additional strategies are needed to consistency achieve a functional cure. The immune system can clear HBV and establish long-term control over the virus. Sufficiently boosting HBV immunity in chronic patients has been very difficult due to immune exhaustion, immune dysregulation, and inhibitory pathways suppressing the immune response. Therap
