Dual radioisotopic study: a technique for the evaluation of vasculogenic impotence.

Arterial and venous systems are the main points for the evaluation of vasculogenic impotence. To evaluate both of these systems in the same study we propose a dual radioisotopic study in which 99mtechnetium (99mTc) and 133xenon (133Xe) were used. The changes in 99mTc and 133Xe radioactivities administered intravenously and intracavernously, respectively, were monitored before and after intracavernous papaverine injection. These changes were determined as time activity curves, which were generated from the region of interest over the penis. A 99mTc penogram index derived from the 99mTc time act

Internal iliac artery revascularization in the treatment of vasculogenic impotence.

Five impotent men underwent internal iliac artery revascularization in conjunction with end-to-side aortobifemoral bypass after preoperative testing suggested a vasculogenic cause for impotence. All patients had abnormal preoperative penile/brachial arterial pressure indices (mean, 0.42 +/- 0.12). Following operation, all patients regained erectile capability and had normal postoperative penile/brachial indices (mean, 0.80 +/- 0.06). One patient developed retrograde ejaculation, emphasizing the need for meticulous nerve-sparing dissection with this operation. Internal iliac artery revasculariz

[Investigation and surgical treatment of vasculogenic impotency (author's transl)].

Vasculogenic impotency does exist. Series of 45 operated cases with at least 6 month follow up give the main points of diagnosis and surgical new methods. Organic impotency is assessed by permanent leakage of morning erection, and monitoring of nocturnal penile tumescence (P.E.N.). Arterial insufficiency is present when penile pressure index (I.P.P.) is below 0,95 and Doppler penile notation up to 5 (N.D.P.). Selective hypogastric arteriography shows the site and importance of lesions. Venous incompetence is explored by cavernosography and passive erection output studies. 45 patients experienc

Male sexual impotence in diabetes mellitus: vasculogenic versus neurogenic factors.

In an attempt to understand better the role of vascular and neurogenic alterations in the pathophysiology of diabetic impotence, 29 impotent male patients with diabetes mellitus (DM) were investigated by means of doppler ultrasound examination (DUE) of penile arteries, sacral evoked responses (SERs), peripheral nerve conduction studies, and, in most cases, nocturnal penile tumescence monitoring (NPTM). Fourteen patients showed isolated alterations of penile vascular supply, four isolated alterations of penile segmental innervation, and seven combined vasculogenic and neurogenic abnormalities;

Cigarette smoking and other vascular risk factors in vasculogenic impotence.

A total of 132 consecutive patients with erectile impotence underwent extensive evaluation, including vascular evaluation with intracavernous injection of papaverine and penile duplex ultrasonography, to determine the etiology of impotence. Three vascular risk factors, smoking, diabetes mellitus and hypertension, were investigated for their impact on vasculogenic impotence. The patients were divided into four groups: one with no risk factors, one with one vascular risk factor, one with two vascular risk factors, and one with all three risk factors. The results of penile vascular evaluation in

Angiodynography (color-coded duplex sonography) in the evaluation of vasculogenic impotence.

The penile arteries of 18 men with erectile dysfunction were examined by angiodynography (color-coded duplex sonography). Blood flow velocity was measured before and after intracavernous injection of papaverine/phentolamine. The angiodynographic findings were compared to arteriography. Normal values of peak flow velocity (after injection) were obtained from 6 men with normal arteriographic findings (deep artery greater than 25 cm/s, superficial artery greater than 30 cm/s). Angiodynography enables good imaging of the four penile arteries superior to duplex sonography. A strong correlation with

Vasculogenic impotence evaluated by high-resolution ultrasonography and pulsed Doppler spectrum analysis.

Vasculogenic impotence was evaluated by high-resolution sonography and quantitative Doppler spectrum analysis in 21 patients and two normal volunteers. Erection was induced by intracorporeal injection of papaverine, and B-scan imaging and Doppler analysis were performed with the penis flaccid and erect. The corpora cavernosa and its deep arteries, median septum, and corpus spongiosum were clearly displayed in every subject, with the dorsal vein and dorsal artery seen ventral to the corpora cavernosa. In the flaccid state, in all subjects, Doppler analysis demonstrated flow in the dorsal arteri

Surgical correction of vasculogenic impotence in a patient after bilateral renal transplantation.

This article documents a vascular cause of erectile dysfunction in a patient with ligation of both internal iliac arteries after bilateral renal transplantation. The diagnosis was made by the noninvasive measurement of the penile arterial systolic pressure and confirmed by pelvic arteriography. Correction of the patient's impotence was accomplished by restoration of penile blood flow by a saphenous vein bypass graft between the external iliac and internal iliac arteries.

Auto-injection of the corpus cavernosum with a vasoactive drug combination for vasculogenic impotence.

Intracavernous injection of papaverine hydrochloride with phentolamine mesylate rapidly produces transitory penile tumescence, which can be followed by erection and coitus provided there is sexual stimulation. Coital penetration was possible in 59 of 62 patients with impotence of divers etiologies (vascular, diabetic, iatrogenic and Peyronie's disease) who underwent injection and were sent home to attempt coitus. One patient had a prolonged erection that was treated successfully with aspiration of a corpus. When coitus was successful the patient was offered training in self-injection. With sel

Early experience with balloon angioplasty of internal iliac arteries for vasculogenic impotence.

Five patients with vasculogenic impotence underwent balloon angioplasty of internal iliac and/or internal pudendal artery obstructions in an attempt to increase the blood pressure in the dorsalis penis artery and thus improve erectile function. Four patients had bilateral obstructions, and one patient had unilateral iliopudendal arterial disease. Balloon angioplasty was used to dilate bilateral lesions in three patients and unilateral lesions in two patients. Four of the five patients had improvement in erectile function for 2 to 18 months (mean, 12 months) after angioplasty. We conclude that

Vasculogenic impotence: diagnosis and objective evaluation using quantitative segmental pulse volume recorder.

Pulse volume recordings were obtained in 60 normal subjects and 74 patients with a history of impotence using a technique that is simple, reliable, inexpensive, reproducible and capable of standardisation. It measures the contributions of all vessels in the penis. The components of the electronic package are described. The pulse volume amplitudes, pulse contours and other measurements in normal subjects are defined. The results give a reliable index of penile blood flow. Pulse volume recordings were also found to correlate with the severity of pelvic angiographic patterns of occlusive disease.

Diagnosis of vasculogenic impotence: combination of penile xenon-133 washout and papaverine tests.

The present study evaluates both penile xenon-133 washout (XWT) and papaverine tests (PT) in the diagnosis of vasculogenic impotence. XWT was accomplished by subcutaneous injection of xenon-133 (1-2 mCi in 0.1 mL saline solution) into the dorsal coronal prepuce. Abnormal XWT was suggested in patients whose clearance time (T1/2) was longer than 7.5 minutes and whose penile blood flow rate (Q) was less than 6 mL/100 g tissue/min. PT was done by intracavernous injection of papaverine (60 mg in 20 mL normal saline). Abnormal PT was indicated in patients whose onset of full erection was more than t

High-flow priapism and glans hypervascularization following deep dorsal vein arterialization for vasculogenic impotence.

Microsurgical penile revascularization is becoming an increasingly applied technique in patients with arteriogenic or mixed arteriogenic and venogenic impotence. Deep dorsal vein arterialization has been used successfully in selected patients. Aside from failure of the procedure and the occasional problems associated with vascular surgery, priapism and glans hypervascularization are specific complications of deep dorsal vein arterialization. Priapism in these cases is 'high-flow'; the functional arterial-cavernous fistula can overcome the maintenance of the flaccid state and cause persistent e

[Evaluation of vasculogenic erectile impotence using DICC (dynamic infusion cavernosometry & cevernosography) with intracavernous injection of papaverine].

Dynamic infusion cavernosometry and cavernosography (DICC) with intracevernous injection of papaverine was performed in 28 men. The change of the corporal body pressure after injection of papaverine and the intracavernous pressure decrease within the first 30 seconds after discontinuation of saline infusion were observed. Four different patterns of cavernometric findings can be identified on the basis of the intracavernous pressure behavior in response to injection of papaverine, infusion of saline and discontinuation of infusion: pattern A represents no vasculogenic insufficiency, pattern B r

The assessment of pelvic circulation after internal iliac arterial reconstruction: a retrospective study of the treatment for vasculogenic impotence and hip claudication.

The standard surgical treatment of vasculogenic impotence or hip claudication involves repairing vascular lesions, especially in the internal iliac arteries. It is difficult, however, to make a definite diagnosis or an accurate judgement of the therapeutic effects of this treatment due to the trouble in ruling out any other disorders. During the last five years, 19 patients with impotence and associated apparent internal iliac artery stenosis or occlusion and 2 patients with hip claudication, underwent internal iliac arterial reconstruction. The patients' ages ranged from 37 to 70 with a mean

Comparison of selective internal iliac pharmaco-angiography, penile brachial index and duplex sonography with pulsed Doppler analysis for the evaluation of vasculogenic (arteriogenic) impotence.

Between July 1987 and February 1988 selective internal iliac angiography was performed before and after intracavernous injection of papaverine plus phentolamine in 43 patients with erectile dysfunction. In 63% of the patients stenosis or occlusion of the pudendal artery was found. The penile brachial index was calculated and duplex sonography with pulsed Doppler analysis was performed in 23 patients. Angiography and penile brachial index correlated in only 39% of the patients, whereas selective internal iliac angiography and duplex sonography correlated in 91% (21 of 23). In 2 patients duplex

[Measurement of penile brachial index (PBI) in patients with impotence].

PBI was measured in 555 patients chosen at random from those who visited our impotence clinic between October 1984 and September 1986. Of 555 patients with impotence 29 (5.2%) showed a low PBI level of less than 0.6. On the basis of carefully taken history, risk factors causing the disturbance of penile blood flow were found in 13 (44.8%) of these 29 patients. They included diabetes mellitus, hypertension, surgery of pelvic organ, vertebral injury, vascular disorder of brain and gangliosympathectomy. Although penile angiography is necessary to make the definite diagnosis of vasculogenic impote
