Insensible water loss in extremely preterm infants cared for in incubators with 70% and 90% relative humidity.

OBJECTIVE: To determine the insensible water losses (IWL) in preterm infants cared for in incubators with 70% or 90% relative humidity (RH). STUDY DESIGN: This was a single center retrospective study of extremely preterm infants between 22- and 28-weeks gestational ages nursed in 70% and 90% RH. IWL was calculated using a formula that utilized infants' fluid intake, urine output and body weight changes. IWL between the two groups were compared using independent t-test. Multivariate linear regression was used to assess the effects of humidity on IWL after adjusting for confounding variables.

Effects of an early transfer from incubator to a warming crib in very low birthweight preterm infants.

PURPOSE: Very low birth weight infants are cared for postnatally in the incubator because of adverse consequences of hypothermia. Data on the optimal weight of transfer to a warming crib are rare. The aim of this study was to determine the course of temperature and body weight during a standardized transfer to a warming crib at a set weight. METHODS: Prospective intervention study in very low birthweight infants who were transferred from the incubator to a warming crib at a current weight between 1500 g and 1650 g. RESULTS: No infant had to be transferred back to an incubator. Length of hosp

A clinical comparison of radiant warmer and incubator care for preterm infants from birth to 1800 grams.

OBJECTIVE: The objective of this study was to compare radiant warmer and incubator care for preterm infants from birth with respect to temperature control and weight gain. METHODS: Sixty preterm infants <33 weeks' gestation were randomized at birth to radiant warmer or incubator care. The initial goal was to maintain abdominal temperature at 36.8 degrees C in both groups and axillary temperature at 36.8 to 37.3 degrees C; air servocontrol was used for incubator infants. Infants in both groups received added humidity for 5 days if their weight was <1000 g and for 3 days if they weighed between

Early skin-to-skin contact or incubator for very preterm infants: study protocol for a randomized controlled trial.

BACKGROUND: Skin-to-skin care immediately following delivery is a common practice for term infants and has been shown to improve cardiorespiratory stability, facilitate early bonding, and promote breastfeeding. Since 2007, the use of skin-to-skin care has been practiced for preterm infants from 32 weeks of gestation in the delivery room at St. Olav's University Hospital. In the present study we aim to investigate whether skin-to-skin care following delivery is safe, and how it affects early and late outcomes compared to standard care for very preterm infants. METHODS/DESIGN: A randomized cont

Disposable low-cost cardboard incubator for thermoregulation of stable preterm infant - a randomized controlled non-inferiority trial.

BACKGROUND: Incubators and radiant warmers are essential equipment in neonatal care, but the typical 1,500 to 35,000 USD cost per device makes it unaffordable for many units in low and middle-income countries. We aimed to determine whether stable preterm infants could maintain thermoregulation for 48 h in a low-cost incubator (LCI). METHODS: The LCI was constructed using a servo-heater costing 200 USD and cardboard infant-chamber. We conducted this open-labeled non-inferiority randomized controlled trial in a tertiary level teaching hospital in India from May 2017 to March 2018. Preterm infan

Body Size, Cerebral Blood Flow, Ambient Temperature, and Relative Brain Temperatures in Newborn Infants under Incubator Care.

Subtle changes in body temperature affect the outcomes of ill newborns. However, the temperature profile of neonatal brains remains largely unknown. In open-cot care, increased cerebral perfusion is correlated with higher superficial brain temperatures. This study investigated the dependence of brain temperature (relative to rectal temperature) on ambient temperature, body size, cerebral perfusion, and metabolism in infants receiving incubator care. Rectal, scalp, and brain temperatures, superior vena cava flow, and brain oxygenation were assessed using echocardiography, thermo-compensatory te

Laser-Based Length-Measuring Board for the Measurement of Infant Body Length from Outside an Incubator: Proposal and Assessment of a Model.

INTRODUCTION: Opening the incubator side wall to insert a non-sterile length-measuring device carries the risk of microbial contamination and thermal instability for preterm infants. To reduce this inconvenience, a laser-based length-measuring board is proposed to measure body length from outside the incubator. METHODS: This device has two laser-line-shaped cursors which can be pointed to opposite ends of a segment to be measured. It is attached to the outer side of one of the incubator's side walls in such a manner as to ensure that its axis is parallel to the longitudinal axis of the segmen

Thermoregulation, incubator humidity, and skincare practices in appropriate for gestational age ultra-low birth weight infants: need for more evidence.

BACKGROUND: Although not universal, active care is being offered to infants weighing < 500 g at birth, referred to as ultra-low birth weight (ULBW) infants appropriate for gestational age. These infants have the greatest risk of dying or developing major morbidities. ULBW infants face challenges related to fluid and heat loss as well as skin injury in the initial days of life from extreme anatomical and physiological immaturity of the skin. Although there is an emerging literature on the outcomes of ULBW infants, there is a paucity of evidence to inform practice guidelines for delivering optim

Does maternal intrapartum antibiotic treatment prolong the incubation time required for blood cultures to become positive for infants with early-onset sepsis?

BACKGROUND: We hypothesized that maternal intrapartum antibiotic treatment delays the growth of the organism in the blood culture obtained during the work-up for infants with suspected early-onset sepsis (EOS). METHODS: Single center, retrospective review of infants with blood culture-proven EOS over 13.5 years period. EOS was defined by isolation of a pathogen from blood culture obtained within 72 hours of birth and antibiotic treatment for  ≥ 5 days. RESULTS: Among 81 infants with positive blood cultures, 38 were deemed to have EOS and 43 were deemed contaminants. The organisms grown were

Analysis Model for Infant Incubator Adverse Events Using Retrieval-Augmented Generation Combined With Dual-Adapter Fine-Tuning: Development and Evaluation Study.

BACKGROUND: Infant incubator adverse events refer to various harmful incidents that occur during the normal use of marketed infant incubators and result in, or may result in, bodily harm. In recent years, however, the number of reported adverse events has continued to rise. This trend has made the monitoring of infant incubator adverse events time-consuming and labor-intensive when relying solely on manual processing by medical device adverse-event monitoring personnel. Meanwhile, general-purpose large language models (LLMs) still face domain knowledge gaps and hallucination issues in speciali

A randomized controlled trial to evaluate the efficacy and safety of a double-walled incubator compared to a radiant warmer in the care of extremely low-birth-weight infants.

This study compared the safety and efficacy of double-walled incubators versus radiant warmers in extremely low-birth weight (ELBW: birth weight < 1000 g) preterm infants, hypothesizing that incubator care would reduce the time to regain birth weight compared with radiant warmer care. In this single-center, nonblinded, randomized controlled trial, ELBW infants with gestational age ≥25 to ≤32 weeks were randomized shortly after birth to stay in either an incubator or a radiant warmer for the first week of life, followed by radiant warmer care for all infants. The primary outcome was time to reg

Exploring physiological stability of infants in Kangaroo Mother Care position versus placed in transport incubator during neonatal ground ambulance transport in Sweden.

BACKGROUND: The positive effects of Kangaroo mother care in NICU's are well documented but, to a lesser extent, explored during inter-hospital neonatal transport. Inter-hospital transport, with the infant placed in a transport incubator, increases the risk of separation while infants in Kangaroo mother care position implies that the parents accompany the transport. There exists limited knowledge if physiological stability differs when transported in Kangaroo mother care position compared to transport in a transport incubator. AIMS: The aim of this study was to compare physiological stability

Infants born very preterm react to variations of the acoustic environment in their incubator from a minimum signal-to-noise ratio threshold of 5 to 10 dBA.

INTRODUCTION: Very early preterm infants (VPIs) are exposed to unpredictable noise in neonatal intensive care units. Their ability to perceive moderate acoustic environmental changes has not been fully investigated. RESULTS: Physiological values of the 598 isolated sound peaks (SPs) that were 5-10 and 10-15 dB slow-response A (dBA) above background noise levels and that occurred during infants' sleep varied significantly, indicating that VPIs detect them. Exposure to 10-15 dBA SPs during active sleep significantly increased mean heart rate and decreased mean respiratory rate and mean systemic

Thermal stability during early skin-to-skin vs incubator care in infants born at 22-25 weeks' gestation: a non-inferiority prospective cohort study.

OBJECTIVE: Skin-to-skin care (SSC) of extremely preterm infants are often withheld early in life due to the risk of hypothermia. We aimed to evaluate if SSC is non-inferior to incubator care with regard to maintaining normothermia. STUDY DESIGN: A non-inferiority study was conducted in infants with a gestational age of 22-25 weeks, during October 2023-January 2026. Axillary temperatures were obtained at three time points; in the incubator, during SSC, and immediately after transfer back to the incubator. RESULTS: Body temperature data were obtained from SSC sessions (n = 125) in the first 2

Study protocol of "A preterm infant life-saving package including a simple and affordable incubator and a portable flow generator mask-CPAP in resource-limited settings" SAVE preterm trial, saving preterm infants by adopting vital equipment.

INTRODUCTION: Despite a significant global decline in the mortality rate for children under the age of 5, the neonatal mortality rate (NMR) has not significantly improved over the last thirty years. Globally, 250 million newborn babies die each year, and 98% of the neonatal deaths occur in middle- and low-income countries. The leading cause of death is preterm birth; and complications due to the prematurity of the infant include respiratory failure, hypothermia, infections, and dehydration. Treatment for these complications may require respiratory support devices, temperature management using

Ending Neonatal Deaths From Hypothermia in Sub-Saharan Africa: Call for Essential Technologies Tailored to the Context.

Neonatal death represents a major burden in Sub-Saharan Africa (SSA), where the main conditions triggering mortality, such as prematurity, labor complications, infections, and respiratory distress syndrome, are frequently worsened by hypothermia, which dramatically scales up the risk of death. In SSA, the lack of awareness on the procedures to prevent hypothermia and the shortage of essential infant devices to treat it are hampering the reduction of neonatal deaths associated to hypothermia. Here, we offer a snapshot on the current available medical solutions to prevent and treat hypothermia i

Improving newborn health in countries exposed to political violence: An assessment of the availability, accessibility, and distribution of neonatal health services at Palestinian hospitals.

INTRODUCTION: Geopolitical segregation of Palestine has left a fragile healthcare system with an unequal distribution of services. Data from the Gaza Strip reflect an increase in infant mortality that coincided with a significant increase in neonatal mortality (12.0 to 20.3 per 1,000 live births). OBJECTIVE: A baseline study was carried out to evaluate available resources in neonatal units throughout Palestine. STUDY DESIGN: A cross-sectional, hospital-based study was conducted in 2017 using the World Health Organization's "Hospital care for mothers and newborn babies: quality assessment and
