Severely injured geriatric patients return to independent living: a study of factors influencing function and independence.

Our previous work demonstrated that geriatric trauma patients (age greater than 65 years) consume disproportionate amounts of health care resources. In the past we hypothesized that late mortality is high, long-term outcome is poor, and return to independence is low in a severely injured geriatric population. Of 6,480 trauma admissions over 5 years, geriatric patients (n = 495) with blunt trauma injury (n = 421) and an ISS greater than 16 (n = 105) who survived until discharge (n = 61) underwent long-term follow-up (mean = 2.82 years). We surveyed 20 measures of functional ability; 10 measures

Validation of rule-based inference of selected independent activities of daily living.

This paper explores the validity of a rule-based inference method of selected independent activities of daily living (ADLs). An inexpensive ADL monitoring system was installed in the community for 37 days to monitor a middle-aged, healthy individual living alone. The subject was given a personal digital assistant (PDA), running custom activity diary software, and asked to record activities in real-time. Rule-based activity inference algorithms were refined on data from 17 days, and data from the remaining 20 days were used for validation. The chisquare statistic was computed for 2 x 2 continge

"Halfway Independent": Experiences of formerly homeless adults living in permanent supportive housing.

Permanent supportive housing (PSH), which combines affordable public housing with social services, has become the dominant model in the United States for providing housing to formerly homeless people. PSH has been effective in reducing re-entry to homelessness, yet has shown limited evidence of improving formerly homeless individuals' mental health and quality of life. This study aimed to understand the lived experiences of formerly homeless adults' adjustment to tenancy in PSH, with a focus on how living in PSH has affected their meaningful activity and social engagement. Based on a phenomeno

High Prevalence of Falls Independent of Age in Adults Living With Chronic Pain.

OBJECTIVES: High risks of falls have been reported in older adults with chronic pain but chronic pain similarly affects adults of all ages. This cross-sectional study aimed to determine the prevalence of falls and associated risk factors in adults of all ages living with chronic pain. MATERIALS AND METHODS: Patient-reported data were analyzed from 591 adults with chronic pain enrolled in a local pain clinic between November 2017 and April 2019. Sociodemographic, history of falls, and biopsychosocial measures of pain were examined to identify and describe adults with chronic pain who fell in t

Number of people in the United States experiencing ambulatory and independent living difficulties.

Estimating the characteristics of the "disabled" population is necessary for some governments and of interest to health researchers concerned with producing disability prevalence rates. Because generating easy-to-understand estimates of disability in the population is important, this article provides U.S. population estimates for two disability-related measures by using the 2009 to 2011 American Community Survey Public Use Microdata Sample file. The number of people who have "independent living" and "ambulatory" difficulties is calculated from a sample of 9,204,437 (representing >309 million p

Transition strategies for youths with sensory impairments: Educational, vocational and independent living considerations.

Students with sensory impairments comprise a heterogeneous group that is often underserved by rehabilitation and other social services. This underserved status has been attributed to poor interagency collaboration and eligibility issues, the low incidence nature of these disabilities, and the shortage of trained professionals to address the specialized needs of these individuals. This article examines important educational and vocational characteristics of this population to assist rehabilitation professionals in supporting more effective transitions into employment and other adult roles. Spec

Stage-independent biomarkers for Alzheimer's disease from the living retina: an animal study.

The early diagnosis of neurodegenerative disorders is still an open issue despite the many efforts to address this problem. In particular, Alzheimer's disease (AD) remains undiagnosed for over a decade before the first symptoms. Optical coherence tomography (OCT) is now common and widely available and has been used to image the retina of AD patients and healthy controls to search for biomarkers of neurodegeneration. However, early diagnosis tools would need to rely on images of patients in early AD stages, which are not available due to late diagnosis. To shed light on how to overcome this obs

"Living Independently Means Everything to Me": The Voice of Australian Autistic Adults.

BACKGROUND: Despite the desire of many autistic adults to live independently, recent studies suggest that only about one in six autistic adults do. This includes living alone, with a partner, or with friends. This is a rate that is lower than adults with other disabilities. Existing studies from other countries have explored the potential barriers to independent living, with very few exploring the actual experiences of autistic adults. To the best of our knowledge, there have been no published studies exploring the firsthand moving out experiences of autistic adults globally and none that have

Acting Independently While Living Alone: The Strategies and Struggles of Cancer Patients.

Cancer patients who live alone place specific importance on acting independently during treatment. We want to describe what it means to act independently and which strategies patients use to continue to act independently. We used a qualitative design, based on grounded theory. We interviewed 32 patients, 17 of them a second time. Patients who live alone defined acting independently in two different ways: It meant not only doing things alone but also using the help of others in a controlled way. These two meanings lead to two types of strategies. As treatment evolves, patients needed to change

The Women Independently Living Alone with a Medical Alert Device (WILMA) trial.

Women are more likely to live alone compared with men, and therefore have more difficulty activating emergency medical systems for stroke. The goal of this study was to examine the benefit of wearing medical alert devices to activate emergency medical systems for elderly women living alone. This was a randomized, controlled pilot trial. Women over 60 with at least 1 stroke risk factor were recruited from Southeast Michigan. Subjects received either a medical alert device or control. The primary outcome was change in health-related quality of life (HRQOL) from baseline to 90 days of wearing the

Periodontal attachment loss in independently living older adults in Ontario, Canada.

This paper describes the periodontal disease experience of a community-dwelling population aged 50 years and older, living in four communities in Ontario, Canada. The periodontal status of this population was assessed using attachment loss and the extent and severity index. Attachment loss was measured at two sites on each tooth using a pressure-sensitive periodontal probe. Complete periodontal data were obtained on 624 subjects. The mean number of sites per subject was 37.9. In line with recent US studies, the diagnostic threshold for a diseased or previously diseased site was set at 2 mm of

Problems in relation to independent living: a retrospective study of physically disabled school-leavers.

Problems in relation to independent living were studied in a cohort of 80 physically disabled young people on average five years after leaving school. 38 per cent were living, or planned to live, independently. This group was more likely to be of average intellectual ability and to have a physical impairment other than cerebral palsy. Of the 35 per cent who felt that their physical disabilities were such that they would be unable to manage without care from parents in the family home, at least 80 per cent needed daily help with personal hygiene, dressing and travel. 90 per cent of the parents

Challenges in Independent Living for Middle-Aged Autistic Adults in China: A Qualitative Study.

As the first cohort of formally diagnosed autistic individuals in China enters later life, the disruption of premature old-age care among middle-aged autistic adults-while they strive for independent living in old age-has emerged as an urgent public health issue. This study employed an exploratory qualitative design. Data collection methods included 34 semi-structured interviews and participant observation. Analysis revealed that premature care disruption unfolds through three sequential stages. First, while families still provided care, three resource deficits undermined independent aging for
