Physiological and clinical aspects of apnea diving.

Apnea diving is a fascinating example of applied physiology. The record for apnea diving as an extreme sport is 171 meters, 8:58 minutes. The short time beneath the surface induces profound cardiovascular and respiratory effects. Variations of blood-gas tensions result from the interaction of metabolism and the rapid sequence of compression and decompression. Decompression sickness is possible. Apnea divers can reach depths beyond the theoretic physiologic limit by using the lung-packing maneuver. Apnea divers exhibit a fall in heart rate, which can be trained and is an oxygen-conserving effec

Critical fusion flicker frequency results with 1 or 2 diodes during HBOT session - observational study among hyperbaric attendants.

Hyperbaric oxygen therapy (HBOT) is used as a treatment for acute and chronic diseases. The therapy is conducted in hyperbaric chambers. Hyperbaric medicine centers' use of multiplace chambers assures the patients' safety and comfort due to attendant's presence during each session. Among the patients and in the group of medical attendants the most common side effect of HBOT is middle ear disorder. However, inert gas narcosis occurring during compression to over 2 ATA (10 meters' of sea water equivalent) can be manifested in temporary cognitive function impairment, mostly due to nitrogen narcos

Neurological complications of underwater diving.

The diver's nervous system is extremely sensitive to high ambient pressure, which is the sum of atmospheric and hydrostatic pressure. Neurological complications associated with diving are a difficult diagnostic and therapeutic challenge. They occur in both commercial and recreational diving and are connected with increasing interest in the sport of diving. Hence it is very important to know the possible complications associated with this kind of sport. Complications of the nervous system may result from decompression sickness, pulmonary barotrauma associated with cerebral arterial air embolism

Predicting nitrogen narcosis using the electroencephalogram recorded from experienced divers.

Nitrogen narcosis causes acute cognitive impairment in divers breathing compressed air at depth, increasing injury and fatality risk. Susceptibility varies widely between individuals, making real-time cortical monitoring a potential safety tool. We recorded 32-channel electroencephalography (EEG) from 45 divers (both sexes; aged 20-55 yr) at rest across four randomized crossover hyperbaric studies. We manipulated pressure (101-811 kPa) and breathing gas (air or heliox) to create two classes of recordings: narcotic (high-pressure air) or non-narcotic (low-pressure air; heliox at any pressure);

Dopamine/BDNF loss underscores narcosis cognitive impairment in divers: a proof of concept in a dry condition.

PURPOSE: Divers can experience cognitive impairment due to inert gas narcosis (IGN) at depth. Brain-derived neurotrophic factor (BDNF) rules neuronal connectivity/metabolism to maintain cognitive function and protect tissues against oxidative stress (OxS). Dopamine and glutamate enhance BDNF bioavailability. Thus, we hypothesized that lower circulating BDNF levels (via lessened dopamine and/or glutamate release) underpin IGN in divers, while testing if BDNF loss is associated with increased OxS. METHODS: To mimic IGN, we administered a deep narcosis test via a dry dive test (DDT) at 48 msw in

Mononeuropathy of the medial branch of the deep peroneal nerve in a scuba diver.

Peripheral mononeuropathies occur only rarely in association with decompression illness. The sites previously reported to be affected are areas of potential entrapment in which a peripheral nerve traverses a confined area. In these instances, the pathophysiology has been presumed to be mechanical pressure in an enclosed space by a gas bubble. A rare case is now presented of a peripheral mononeuropathy of the medial branch of the deep peroneal nerve in a scuba diver following surfacing from a 195 foot dive. This case differs from prior reports of mononeuropathy in association with decompression

Hyperbaric nitrogen and pentobarbital on synaptosomal membrane lipids and free fatty acids.

Nitrogen at high pressures and anesthetics increase lipid monolayer surface pressure and in turn modulates monolayer associated lipolytic enzyme activity that could alter membrane lipids. We tested the hypothesis that nitrogen at pressures of 5 and 10 megapascals (MPa) and pentobarbital induce alterations in synaptosomal membrane phospholipid and free fatty acid (FFA). Rat cortical synaptosomes in Krebs-Henseleit buffer were placed in steel chambers and incubated for four hours at 37 degrees C: at 5 or 10 MPa of O2/balance N2; at one 0.1 MPa on room air, and with 10 mg pentobarbital. Probing the limits of human deep diving.

Divers breathing compressed air are restricted to 45 m depth because of the narcotic effects of nitrogen and toxic action of oxygen at increased pressures. Substitution of oxygen-helium for compressed air has permitted divers to reach 600 m. However, at depths greater than 160 m, signs and symptoms of the high pressure nervous syndrome (h.p.n.s.) occur, with tremors, myoclonic jerking, nausea, vomiting, fatigue, somnolence, e.e.g. changes, dyspnoea, and poor sleep with nightmares. It has been the objective of this Laboratory to ameliorate the symptoms of pressure-induced h.p.n.s. by the additi

Diving medicine.

Exposure to the undersea environment has unique effects on normal physiology and can result in unique disorders that require an understanding of the effects of pressure and inert gas supersaturation on organ function and knowledge of the appropriate therapies, which can include recompression in a hyperbaric chamber. The effects of Boyle's law result in changes in volume of gas-containing spaces when exposed to the increased pressure underwater. These effects can cause middle ear and sinus injury and lung barotrauma due to lung overexpansion during ascent from depth. Disorders related to diving

Breathing volumes and gas exchange during simulated rapid free ascent from 100 msw.

The crew of a disabled submarine can be rescued by means of free ascent through the water to the surface. Pulmonary gas exchange was studied during simulated rapid free ascent in subjects standing immersed to the neck in a pressure chamber. The pressure was rapidly increased to 1.1 MPa [100 meters seawater (msw)] followed by decompression at 0.03 MPa/s (3 msw/s). Effective inspired tidal volume, as estimated by an Ar dilution method, fell gradually to zero during decompression from 20 to 0 msw. Directly determined expired tidal volumes were increased up to two to three times at the time of ret

Diving practices in technical divers' community and behaviour towards self-reported unusual symptoms.

INTRODUCTION: The use of gas mixtures containing helium for deep recreational diving is increasingly common, involving complex logistics and decision-making compromises. The characteristics and inherent risks of this practice remain poorly documented. This study aims to provide an epidemiological inventory of practices and diving-related incidents within the technical diving community. METHODS: An international online survey was disseminated on social networks targeting certified trimix divers. It collected demographic data, diving experience, and dive management practices, along with self-re

The physiology and pathophysiology of human breath-hold diving.

This is a brief overview of physiological reactions, limitations, and pathophysiological mechanisms associated with human breath-hold diving. Breath-hold duration and ability to withstand compression at depth are the two main challenges that have been overcome to an amazing degree as evidenced by the current world records in breath-hold duration at 10:12 min and depth of 214 m. The quest for even further performance enhancements continues among competitive breath-hold divers, even if absolute physiological limits are being approached as indicated by findings of pulmonary edema and alveolar hem

Decompression injuries occur on account of the special hyperbaric effects during the emerge phase and require superior therapeutic knowledge. Vitally important is emergency treatment with high concentrated oxygen at an early stage. Sever decompression injuries require oxygenation in a hyperbaric treatment chamber.

Going to Extremes of Lung Physiology-Deep Breath-Hold Diving.

Breath-hold diving involves environmental challenges, such as water immersion, hydrostatic pressure, and asphyxia, that put the respiratory system under stress. While training and inherent individual factors may increase tolerance to these challenges, the limits of human respiratory physiology will be reached quickly during deep breath-hold dives. Nonetheless, world records in deep breath-hold diving of more than 214 m of seawater have considerably exceeded predictions from human physiology. Investigations of elite breath-hold divers and their achievements revised our understanding of possible

[Role of heat-shock proteins of Hsp70 family in changes of narcotic activity of hyperbaric nitrogen during increasing hypoxic stimulus].

The spontaneous motor activity and pose reflexes of male adult rats (Wistar) were observed in the course of high pressure nitrogen compression up to 4,1 MPa. The experiments were carried out under normoxic and hypoxic conditions. Stabile rat motor cortex oxygen tension was recording during the nitrogen compression up to 7,1 MPa under normoxic condition. Sensitivity to nitrogen high pressure to be on the increase under hypoxic conditions. In its turn, resistibility to nitrogen high pressure to be on the decrease under hypoxic conditions (oxygen partial pressure from 0,012 to 0,004 MPa). Quantit

Nitrous oxide consistently attenuates thermogenic and thermoperceptual responses to repetitive cold stress in humans.

Divers are at enhanced risk of hypothermia, due to the independent action of the inspired inert gases on thermoregulation. Thus, narcosis induced by acute (≤2 h) exposure to either hyperbaric nitrogen or normobaric nitrous oxide (N2O) impairs shivering thermogenesis and accelerates body core cooling. Animal-based studies, however, have indicated that repeated and sustained N2O administration may prevent N2O-evoked hypometabolism. We, therefore, examined the effects of prolonged intermittent exposure to 30% N2O on human thermoeffector plasticity in response to moderate cold. Fourteen men partic

Heliox at 4 ATA Reduces Error Rates Compared to Trimix and Air, but It Does Not Affect Short-Term Memory in Hyperbaric Conditions.

Cognitive performance during diving is challenged by hyperbaric conditions, where inert gas narcosis, oxygen partial pressure, and carbon dioxide retention may impair accuracy and reaction time. Breathing gas mixtures such as heliox and trimix are proposed to reduce these effects, but controlled evidence at recreational depths remains limited. Our study investigated short-term effects of hyperbaric exposure at 4 ATA (equivalent to 30 m of seawater) on cognitive functioning and whether the severity of impairment varies across breathing gas mixtures: air, trimix and heliox. Each participant comp
