The clinical and metabolic adaptation to extrauterine life in preterm was comprehensively evaluated in newborns with 28-34 weeks of gestation. A high incidence of aggravating factors that underlie the development of chronic fetal hypoxia has been revealed. The early neonatal events comprised impairment of cerebral flow, jaundice, oedema, respiratory disorders, infectious toxicosis, the manifestation rate being inversely proportional to the gestational age. A significant immunological impairment was noted along with the serum electrolyte imbalance and changes in bilirubin metabolism. Developmen

In the years 1977-8,258 infants weighing less than 1500 g were born at, or transferred to, the Royal Women's Hospital, Melbourne; 177 (68.5%) survived, and 111 of these attended for an ophthalmic examination. Significant ocular lesions were found in 37 (33%): 21 (19%) children had squint, 19 (17%) had a significant refractive error, 11 (10%) had cicatricial retrolental fibroplasia (RLF), and 3 (2.7%) had very poor vision due to optic atrophy associated with cerebral palsy. No children were blind owing to RLF, indicating that the recent increase in survival rate of infants of very low birth wei

Glucose disposal was studied in six very low birth weight infants (birth weight 1093 +/- 67 g (M +/- SEM), gestational age 29.8 +/- 0.6 weeks, postnatal age 5.2 +/- 0.9 days) by constant infusion steady state technique. All babies required artificial ventilation because of respiratory distress syndrome (four) or aspiration pneumonia (two). Nine healthy prematures of comparable birth weight, gestational age, and postnatal age formed the control group. During a constant glucose infusion at a rate of 8 mg/kg/min (7.7 +/- 0.2) mean blood glucose levels of the sick infants were higher than the corr

In very low birth weight infants, the occurrence of bilirubin-related brain damage has been repeatedly observed at low serum bilirubin concentrations in close association with altered pathophysiologic status (hypoxia, acidosis, hypothermia, and so on). This increased susceptibility is accompanied by increased severity and duration of unconjugated hyperbilirubinemia as compared with more mature infants. Clinical manifestations of kernicterus in very low birth weight infants are almost always nonspecific. No single biochemical or physiologic measurement is sufficient to predict the risk for deve

OBJECTIVE: To identify the incidence and characteristics of airway diseases in very low birth weight infants (VLBWIs).
METHODS: A retrospective study of 214 inborn VLBWIs admitted to our NICU between April 2009 and March 2022 (approval no: 2023-0008). Neonatologists ourselves performed bronchoscopy to diagnose airway diseases.
RESULTS: Symptomatic airway diseases were present in 36/214 (16.8%) of VLBWIs. Common airway diseases were tracheobronchomalacia (TBM) and pharyngomalacia. Infants with airway diseases had shorter gestational age, lower birth weight, more boys, and more moderate/severe

OBJECTIVES: High manganese (Mn) levels during fetal growth or prolonged parenteral nutrition (PN) may have adverse effects on neurodevelopment. We aim to report on Mn levels and their short-term impact on clinical course in very low birth weight infants.
METHODS: An observational study including newborns with a gestational age (GA) ≤32 weeks and/or ≤1500 g of birth weight (BW). Newborns received intravenous supplementation of Mn at 1 µg/kg/day (Peditrace ® ) in PN and continued with fortified breast milk. Mothers answered surveys about dietary and other habits and blood levels of Mn in newbor

Despite the fact that feeding a very low birth weight (VLBW) neonate is a fundamental and inevitable part of its management, this is a field which is beset with controversies. Optimal nutrition improves growth and neurological outcomes, and reduces the incidence of sepsis and possibly even retinopathy of prematurity. There is a great deal of heterogeneity of practice among neonatologists and pediatricians regarding feeding VLBW infants. A working group on feeding guidelines for VLBW infants was constituted in McMaster University, Canada. The group listed a number of important questions that ha

BACKGROUND: Renal calcification in preterm infants has been described frequently. The etiologic factors have not yet been fully clarified. The objective of this study was to evaluate the incidence of and risk factors for renal calcification in our population.
METHODS: We retrospectively reviewed the charts of very low birth weight preterm infants during a 1-year period. Renal ultrasound scans were performed at term or before discharge and at a corrected age of 1 year.
RESULTS: Six infants (6%) had renal calcification at term or before discharge compared with 96 who did not. Factors significa

OBJECTIVE: The aim of this study was to assess cerebellar growth of very low birth weight infants from birth to discharge and compare it with term infants.
STUDY DESIGN: Very low birth weight infants were matched by gender, adequacy of weight to gestational age at birth and postmenstrual ages at hospital discharge to term newborns. Exclusion criteria were central nervous system malformation, peri-intraventricular hemorrhage, cerebellar hemorrhage and meningitis. Transverse cerebellar diameter was measured by cranial ultrasound at birth and at hospital discharge in cases, and at birth in match

OBJECTIVES: To measure concentrations of fecal calprotectin (f-calprotectin) in infants with very low birth weight (VLBW; <1500 g) longitudinally and to describe changes in f-calprotectin in infants who develop severe abdominal disease.
PATIENTS AND METHODS: The study included 59 VLBW infants. Seven patients (disease group) developed severe abdominal disease defined as necrotizing enterocolitis (NEC) or a condition leading to laparotomy. The remainder (n = 52) were considered reference infants and had a mean (+/-SD) gestational age of 27.2 +/- 2.6 weeks and a birth weight of 939 +/- 273 g. F-

BACKGROUND: Early growth in very low birth weight (VLBW) infants has been found predictive of their later outcomes. This has led to increased interest in establishing measures to optimise such growth. In facilities without the resources required to undertake long-term audits for all the high risk infants they graduate, these growth parameters may also be used as selection criteria for those meriting such follow up reducing costs.
OBJECTIVES: To describe early growth patterns among a cohort of VLBW infants and determine some of the factors associated with poor growth among them.
DESIGN: Cross

Congenital heart defects in low birth weight infants are typically managed with supportive therapy or palliative surgery, and definitive repair is delayed. However, the morbidity of such an approach has been shown to be high. Recent reports emphasize early surgical repair with good results in low birth weight and premature babies. However, there are no data on the outcomes in infants with very low birth weight (<1,500 g). Since 1990, the authors performed complete repair of congenital heart defects (other than isolated patent ductus arteriosus) in 20 infants weighing 1,500 g or less. Defects i

This study aims to determine the prevalence of and risk factors associated with retinopathy of prematurity (ROP) in very low birth weight (VLBW) infants. All premature VLBW infants, admitted into the neonatal intensive care unit of the University Hospital Kuala Lumpur, were screened from 4 weeks of life. Perinatal and neonatal data were retrieved from the infants' medical notes. Between August 1994 and July 1996, 100 infants had their eyes examined serially. Of the 15 (15%) infants with ROP, all were less than 31 weeks gestation, and only 1 infant had birth weight above 1250 g. Five (5%) infan

A syndrome in very low birth weight premature infants weighing less than 1500 g is evidenced by developmental nutritional rickets and fractures at 75 days of age. In a review conducted over 42 months, 247 very low birth weight cases were identified. Rickets was diagnosed in 96 (39%) infants whose mean age was 50 days and fractures were diagnosed in 26 (10.5%) infants whose mean age was 75 days. These 26 infants experienced 98 fractures: 10 humerus, 13 radius, 8 ulna, 4 metacarpal, 3 clavicle, 54 ribs, 5 femur, and 1 fibula. Risk factors included hepatobiliary disease, total parenteral nutritio

In a cohort study with historical controls of non-asphyxiated very low-birth-weight infants (birth weight < or = 1500 g and gestational age < 33 completed weeks), we evaluated the use of a "minitouch" regime for stabilization after birth and treatment of respiratory distress. This combination of early (prophylactic) treatment with nasal continuous positive airway pressure and minimal handling was introduced as a routine in our Department in 1986. We compared infants born in 1987 and in 1985, when ventilator treatment was used initially in all infants with progressing respiratory distress. The

The timing of the first meconium stool has been considered a marker for proper gastrointestinal functioning in the term infant. There is limited information on the meconium passage patterns of very-low-birth-weight infants of less than 32 weeks' gestation. It is unknown whether feeding practices influence the timing of the first stool in these infants. We retrospectively studied 47 very-low-birth-weight infants with birth weights of 1250 g or less who were previously enrolled in a study of gastrointestinal (GI) priming. Infants whose mothers desired to breast feed (n = 7) were given GI priming

Whereas human milk is the recommended diet for all infants, preterm formulas are indicated for enteral feeding of preterm very low birth weight infants when sufficient maternal breast milk and donor human milk are not available. Feeding with preterm formulas helps to ensure consistent delivery of nutrients. The balance of risks and benefits of feeding preterm formulas versus supplemented maternal and donor breast milk for preterm infants, however, is uncertain. Numerous studies and extensive practice have shown improved growth with preterm formulas, but there is concern for increased risks of

AIM: To determine what percentage of therapeutic interventions for very low birth weight infants undergoing neonatal intensive care is evidence based.
METHODS: The management of 80 very low birth weight infants admitted to our neonatal unit during 1998 was retrospectively reviewed. For each clinical diagnosis e.g. respiratory distress syndrome, patent ductus arteriosus or chronic lung disease all interventions were recorder. Each intervention was then categorised according to the level of supporting evidence. Level I was supported by evidence from randomised controlled trials or meta-analysis
