Care of the infant with sclerema neonatorum.

High-risk infants are particularly prone to sepsis which can be accompanied by sclerema neonatorum. This condition is manifested by skin hardening. Causation of hardening of a sick infant's skin must be identified for appropriate treatment to be implemented. Nursing care of the infant with sclerema is very complex. Throughout the course of the disease, the infant frequently requires antimicrobial therapy, ventilatory support, exchange transfusions, precise intake and output measurements, and temperature control. Psychosocial support for the family and infant is also an important part of the nu

Infant/toddler oral health care: guidelines for the primary care physician.

Since primary care physicians treat infants and toddlers on a regular basis, they oversee the growth and development of children and are usually the first health care providers to evaluate their oral health status. When examining an infant or a toddler, certain oral health conditions should be considered, such as tooth eruption, fluoridation, preventive oral hygiene, diet, and orofacial development. This article provides pertinent information in these areas that can serve as a guide for primary care physicians in their routine care of the infant/toddler.

Parent-Training with Kangaroo Care Impacts Infant Neurophysiological Development & Mother-Infant Neuroendocrine Activity.

A randomized control trial was conducted to investigate the effects of skin-to-skin, chest-to-chest contact (kangaroo care, KC) in mother-infant dyads on patterns of infant brain activity and associated mother-infant neurohormone releases. 33 mother-infant dyads participated during pregnancy (29-38 weeks gestation), at neonatal and 3-month periods. Overall, analyses indicated that: 1) infants in the KC group showed left frontal brain activation patterns (asymmetry and coherence) associated with KC training; 2) KC produced moderate to large increases in oxytocin levels; and 3) KC yielded modera

[New recommendations for maternal-infant care in HIV].

In 2023, BREACH («Belgium Research on AIDS and HIV Consortium») presented new Belgian consensus guidelines for perinatal management of HIV-positive mothers. This article aims to highlight them. We review maternal management, discussing the importance of antiretroviral agents during pregnancy, the recommended follow-up and the preferred mode of delivery. The specificities for the infant will be addressed and include management at birth, post-exposure prophylaxis, new developments regarding breast-feeding and necessary follow-up.

Peripartum depression and infant care, sleep and growth.

Peripartum depression is a common, serious complication in mothers. To assess the influence of infant care, sleep and growth on the risk of peripartum depression, 1,271 mothers of healthy one-month-old infants completed comprehensive questionnaires including the Edinburgh Postnatal Depression Scale. Relationships between high depression scores and variables related to infants' care, sleep and growth were assessed adjusting for other variables. High depression scores were found in 233 mothers, which were associated with variables related to infants' care (poor satisfaction with infant care supp

Care of the infant of the diabetic mother.

Gestational diabetes mellitus (GDM) from all causes of diabetes is the most common medical complication of pregnancy and is increasing in incidence, particularly as type 2 diabetes continues to increase worldwide. Despite advances in perinatal care, infants of diabetic mothers (IDMs) remain at risk for a multitude of physiologic, metabolic, and congenital complications such as preterm birth, macrosomia, asphyxia, respiratory distress, hypoglycemia, hypocalcemia, hyperbilirubinemia, polycythemia and hyperviscosity, hypertrophic cardiomyopathy, and congenital anomalies, particularly of the centr

[Home care for the high-risk newborn infant].

With increased survival of extremely low birth weigh (ELBW) and very ill infants, a lot of them are discharged with unresolved medical issues that complicate their subsequent care. Infants born preterm with low birth weight who require neonatal intensive care experience a much higher rate of hospital readmission and death during the first year after birth compared with healthy term infants. Despite initial hospital care which is one of the most expensive of all kind of hospitalization, home care services are sometimes still sparse though the high risk of this group for failure to thrive, respi

The medically complex premature infant in primary care.

The survival rate of the smallest and youngest of premature infants has continued to improve as medical technology has progressed. The current edge of viability is represented by infants born at 23 to 25 weeks' gestation. Neonatal survival of infants at 23 weeks' gestation ranges from 11% to 30%. Survival to hospital discharge for infants ranging from 23 to 26 weeks' gestation is about 70%; 30% to 50% of these infants have moderate to severe disability. Nurse practitioners and physicians will be meeting these young infants in primary care offices after they have been discharged from the neonat

Skin Care for the Extremely Low-Birthweight Infant.

Extremely low-birthweight (ELBW) infants are at increased risk for infection because the innate immune function of their skin is underdeveloped as they lack a competent epidermal barrier. Thus, neonatal clinicians need to pay careful attention to skin care practices, particularly for periviable infants. In this review, we describe the challenges of skin care in ELBW infants and summarize strategies to prevent skin injury, minimize damage when it occurs, and enhance cutaneous innate immunity.

Infant Skin Care Products: What Are the Issues?

BACKGROUND: Infant skin is susceptible to dryness and irritation from external factors, including topical skin care products not formulated for the infant's skin. This may increase the risk of contact dermatitis. Parents frequently express concern regarding potential harm from ingredients in skin care products and seek information. This is complicated by several skin care myths. PURPOSE: The purpose of this literature review was to provide evidence-based information to educate parents on the use of products for preterm and term infants. SEARCH STRATEGY: Multiple searches using PubMed were co

Craniosynostosis: caring for infants and their families.

Craniosynostosis is a developmental anomaly with premature closure of the cranial sutures causing an abnormally shaped skull in an infant. Recommended surgical treatment involves cranial vault reconstruction to open the closed suture, increase intracranial volume, and allow the brain to grow normally. Parents work with a multidisciplinary team during the evaluation process and face various preoperative and postoperative stressors. Critical care nurses can improve the care of the infants and their families by being knowledgeable about the anatomy, assessment, and surgical and nursing management

Caring for the infant of a diabetic mother.

In the United States, approximately 100,000 infants are born to diabetic mothers each year. If diabetes in pregnancy is uncontrolled, the diversity of resulting health problems can have a profound effect on the embryo, the fetus, and the neonate. These infants are at risk for a multitude of physiologic, metabolic, and congenital complications such as macrosomia, asphyxia, respiratory distress, hypoglycemia, hypocalcemia, hyperbilirubinemia, polycythemia and hyperviscosity, cardiomegaly, and central nervous system disruption. Preconception control of glucose metabolism throughout the trajectory

[Neonatal care of premature infants].

Perinatal intensive centers deal with preterm infants of the region. Therefore, the rate of preterm deliveries at the Department of Obstetrics and Gynecology, Medical University of Pécs was high in 1997, about the double of the national rate (18.7%). Among prematures the rate of very low birth weight (VLBW) infants (less than 1500 g) was about 40%, moreover, the rate of extremely low birth weight infants (less than 1000 g) was 39% within the latter group. Related to the new regulation concerning preterm delivery the number of immature infants (weighing less than 750 g) with poor outcome increa

Assessing characteristics of mother-infant dyadic synchrony among vulnerable mothers using the Infant CARE-Index.

OBJECTIVE: The postnatal period crucially influences the mother-infant relationship and subsequently the infant's cognitive, physical, and psychosocial development. Poor maternal mental health may impact early interactions, potentially causing the infant's developmental needs to go neglected. METHODS: In this cross-sectional study, we used the Infant CARE-Index to examine mother-infant interaction patterns among mothers with a history of depression and/or anxiety. Ten mother-infant dyads recruited from a Danish obstetric department participated in video recordings of mother-infant interaction
