Parental Mental Health Conditions and Infant Health Outcomes Among Military Families.

INTRODUCTION: Little is known about the effects of parental mental health burdens during pregnancy on infant health among military families, who are subject to various stressors unique to military life. The present study leveraged infant data from the DoD Birth and Infant Health Research (BIHR) program and self-reported parental survey data from the Millennium Cohort Study (MCS) to examine associations of parental mental health conditions with adverse infant health outcomes. MATERIALS AND METHODS: Subjects included singleton infants captured in BIHR program data, born between July 2001 and De

The microbiome, guard or threat to infant health.

Despite improvements in survival for very low birthweight (VLBW) premature infants, there continues to be significant morbidity for these infants at remarkable cost to the healthcare system. Concurrent development of the preterm infant intestine alongside the gut microbiome in the clinical setting rather than in the protected in utero environment where it would usually occur creates significant vulnerabilities for the infant's immature intestine and immune system, resulting in devastating illness and neurological injury. However, the microbiome also has the capacity to promote healthy developm

Maternal influences on infant gut microbiome and health.

The establishment of the infant gut microbiome is critical for later health1,2, yet how it is shaped by maternal and early-life factors remains unclear. Here we metagenomically sequenced 4,526 longitudinal faecal samples from 714 mother-infant pairs in the Dutch birth cohort Lifelines NEXT, spanning 12 weeks of pregnancy to 1 year postpartum. We integrated these data with 474 clinical and exposure variables, and with ultra-deep sequencing of breast milk and vaginal microbiomes. We observe that the maternal gut microbiome undergoes only subtle changes during pregnancy and postpartum, influenced

State-Level Abortion Policies and Infant Health Outcomes.

IMPORTANCE: Previous research has found associations between abortion policies and infant health. Differing methods complicate understanding which abortion policies matter for infant health. OBJECTIVE: To examine associations between multiple state-level abortion policies and infant health. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study obtained birth certificate data and cohort-linked birth-infant death files and used logistic regression, adjusting for individual and state controls, state-year combination fixed effects, state-specific time trends, and standard errors clu

The Michigan infant mental health home visiting model.

Selma Fraiberg's pioneering work with infants, toddlers, and families over 40 years ago led to the development of a field in which professionals from multiple disciplines learned to work with or on behalf of infants, very young children, their parents, and the relationships that bind them together. The intent was to promote social and emotional health through enhancing the security of early developing parent-child relationships in the first years of life (Fraiberg, 2018). Called infant mental health (IMH), practitioners from fields of health, education, social work, psychology, human developme

Assessing and responding to infant mental health needs.

Early recognition of mental health problems in maltreated infants, coupled with tailored support to aid recovery, has the potential to offer significant benefits to a population whose needs have perhaps remained hidden in the past. All forms of maltreatment may adversely affect infant mental health, but perhaps none more so than emotional neglect. Universal contact with families with young children makes health visitors ideally placed to provide early support to this vulnerable group, but such practice requires considerable knowledge and understanding. This paper examines how emotional neglect

Possible impact of phthalates on infant reproductive health.

Phthalates adversely affect the male reproductive system in animals, inducing hypospadias, cryptorchidism, reduced testosterone production and decreased sperm counts. Phthalate effects are much more severe after in utero than adult exposure. Little is known about human health effects. This study discusses two recent studies on perinatal phthalate exposure, which indicated that human testicular development might be susceptible to phthalates. One study analysed phthalate monoesters in breast milk and reproductive hormone levels in infants. Five of six phthalates [monoethyl-(MEP), monobutyl- (MBP

The infant walker. A previously unrecognized health hazard.

The use of infant walkers within a large prepaid pediatric group practice was investigated. Of 195 patients (age range, 5 to 15 months) surveyed during a three-months) surveyed during a three-month period, 150 patients (77%) used infant walkers. Of those, 47 patients (31%) had suffered mishaps. These included closed head trauma, fractures, lacerations, tooth evulsion, and soft palate perforation. There is no proved benefit associated with the use of such walkers. The risk of injury associated with the use of infant walkers should be of concern for those physicians who care for preambulatory ch

The infant food industry and international child health.

Declining breast-feeding, with accompanying increased marasmus and diarrhea, has occurred in developing countries because of many factors, including inappropriate health services, new urban life styles, and so forth. The infant food industry must bear a considerable burden of blame as a result of "unethical" advertising. Responses have most recently included various journalistic and legal actions. There is a need for revised roles for the infant food industry, and for mechanisms to monitor intrinsically harmful practices.

Calcium and bone health in infants.

Osteopenia, rickets, and fractures from nutrient deficiencies can occur during infancy, particularly in preterm infants. Bone mass accretion during the first year of life is equal to or greater than that achieved at any other stage of life, including adolescence. Optimizing calcium and bone status during infancy can have immediate benefits in maintaining calcium homeostasis and preventing disturbances in bone mineralization and can provide long-term benefits by helping infants to later reach their maximum genetic potential for peak bone mass, a prerequisite for the prevention of osteoporosis a

Discovery of Novel Probiotic Species to Improve Infant Health.

UNLABELLED: Selecting an appropriate infant formula can be challenging for parents given the wide range of available options, which may lead to suboptimal choices or frequent switching. Our recent studies in mice demonstrated that the maternal microbiota-particularly Limosilactobacillus reuteri present in breast milk-plays a critical role in shaping neonatal immunity by promoting the production of immunoglobulin A (IgA), a key antibody for defense against enteric pathogens. To extend these findings to humans, we analyzed fecal samples from healthy breastfed infants using shotgun metagenomic se

Maternal microbiota and gestational diabetes: impact on infant health.

Gestational diabetes mellitus (GDM) is a common complication of pregnancy that has been associated with an increased risk of obesity and diabetes in the offspring. Pregnancy is accompanied by tightly regulated changes in the endocrine, metabolic, immune, and microbial systems, and deviations from these changes can alter the mother's metabolism resulting in adverse pregnancy outcomes and a negative impact on the health of her infant. Maternal microbiomes are significant drivers of mother and child health outcomes, and many microbial metabolites are likely to influence the host health. This revi

Tummy Time and Infant Health Outcomes: A Systematic Review.

CONTEXT: The World Health Organization recommends tummy time for infants because of the benefits of improved motor development and reduced likelihood of plagiocephaly. Because of poor uptake of these recommendations, the association of tummy time with other health outcomes requires further investigation. OBJECTIVE: To review existing evidence regarding the association of tummy time with a broad and specific range of infant health outcomes. DATA SOURCES: Electronic databases were searched between June 2018 and April 2019. STUDY SELECTION: Peer-reviewed English-language articles were included

Immunosuppression in pregnancy: choices for infant and maternal health.

Successful pregnancy outcomes are possible after all types of solid organ transplantation and thousands of successful pregnancies in such women have been reported. As immunosuppressive medications are required to maintain adequate graft and maternal survival, major concerns are the effect of these agents on the fetus and the effect of pregnancy on the well being of mother and graft, against a background of continuing advances and modifications in immunosuppressive therapy. Women should avoid unnecessary medications during pregnancy but clinicians worry most about teratogens; agents (environmen

Maternal-Foetal/Infant Interactions-Gut Microbiota and Immune Health.

In recent years, the number of scientific publications on the role of intestinal microbiota in shaping human health, as well as the occurrence of intestinal dysbiosis in various disease entities, has increased dynamically. However, there is a gap in comprehensively understanding the factors influencing a child's gut microbiota. This review discusses the establishment of gut microbiota and the immunological mechanisms regulating children's microbiota, emphasising the importance of prioritising the development of appropriate gut microbiota in a child from the planning stages of pregnancy. Biological Relevance of Goat Milk Oligosaccharides to Infant Health.

Milk is often regarded as the gold standard for the nourishment of all mammalian offspring. The World Health Organization (WHO) recommends exclusive breastfeeding for the first 6 months of the life of the infant, followed by a slow introduction of complementary foods to the breastfeeding routine for a period of approximately 2 years, whenever this is possible ( Global Strategy for Infant and Young Child Feeding; WHO, 2003). One of the most abundant components in all mammals' milk, which is associated with important health benefits, is the oligosaccharides. The milk oligosaccharides (MOS) of hu
