BACKGROUND AND PURPOSE: Knowledge of the extent and distribution of infarcts of the posterior cerebral artery (PCA) may give insight into the limits of the arterial territory and infarct mechanism. We describe the creation of a digital atlas of PCA infarcts associated with PCA branch and trunk occlusion by magnetic resonance imaging techniques.
METHODS: Infarcts were manually segmented on T(2)-weighted magnetic resonance images obtained >24 hours after stroke onset. The images were linearly registered into a common stereotaxic coordinate space. The segmented images were averaged to yield the

Infarction of the hippocampus and the right ventroposterolateral thalamus was observed. Angiography revealed a posterior cerebral artery occluded near its origin, immediately upstream from the posterior communicating artery. The infarction was limited to the areas irrigated by the branches originating in the proximal part of the artery. More distal branches for the calcarine scissure and the temporooccipital gyruses were not involved.

Visual disturbances such as homonymous hemianopia and visual hallucinations often suggest central nervous system pathology, particularly occipital lobe infarction. However, ocular conditions may present similarly and lead to misdiagnosis. We report the case of a 73-year-old man presenting with right-sided visual field deficits and visual hallucinations, initially suggestive of posterior cerebral artery stroke. Neuroimaging and electroencephalograms were unremarkable. Further ophthalmologic evaluation revealed advanced normal-tension glaucoma (NTG) with bilateral field constriction and signific

Moyamoya disease is a rare pathological disorder characterized by progressive intracranial artery stenosis and collateral vessel formation. Posterior cerebral artery involvement is rare with a predilection towards infarction. Herein we present a case of a young female with moyamoya disease treated with bilateral encephalomyosynangiosis which subsequently progressed to posterior cerebral artery involvement, requiring encephalomyosynangiosis to prevent further infarction.

The clinical features and imaging findings of posterior cerebral artery infarction (PCAI) in 18 cases verified by CT scan and/or MRI, DSA, SPECT were reported, and their interrelation were analyzed. It was found that the main clinical feature of PCAI was visual defect with amnesia syndrome, the lesion was predominant in the right hemisphere. In examining the structure and function of brain and vessels, each of CT, MRI, ECT and DSA had its own advantage and might complement the others, CT and MRI were better in displaying the structure of cerebral parenchyma than ECT and DSA, whereas, MRI was s

Primary intracranial ependymal cysts are congenital, benign, ependyma-lined cysts rarely seen in the pediatric age group. The authors report such a case of intracranial fronto-parietal ependymal cyst in a 3 y-old girl. Computed tomography showed a large cystic lesion which was not in communication with the ventricular system and was associated with ipsilateral posterior cerebral artery infarct and raised intracranial pressure causing midline shift. A differential diagnosis of the commoner arachnoid, hydatid or porencephalic cyst was considered. Surgical removal to decompress the brain was done

Posterior cerebral artery (PCA) territory infarction is not uncommon. Published series were concentrated either on isolated deep PCA territory infarcts or on incomplete calcarine artery territory infarcts. Although, correlations between clinical symptoms, causes of stroke and outcome at 6-months in patients with superficial PCA territory stroke are less well known. We sought prospectively stroke causes, infarct topography, and clinical findings of 137 patients with superficial PCA territory infarcts with or without mesencephalic/thalamic involvement, representing 11% of patients with posterior

BACKGROUND: Magnetic resonance (MR) imaging of posterior cerebral artery infarction with hemiparesis (PCAH) is limited to case reports with T2-weighted lesions involving the cerebral peduncle.
OBJECTIVE: We sought to extend the MR imaging correlate of PCAH.
METHODS: The case histories and MR/MR angiography findings of 4 patients with PCAH were examined.
RESULTS: Distinct from previously described MR imaging findings with PCAH, diffusion-weighted imaging and/or FLAIR changes were noted in the thalamogeniculate artery (TGA) territory in one patient; in the distribution of the TGA and lateral

Numerous reports have described a variety of clinical syndromes resulting from posterior cerebral artery (PCA) infarction, whereas only a few pathoanatomical and retrospective clinical studies have investigated the underlying mechanisms. Therefore we attempted to determine the causes of infarction in the superficial posterior cerebral artery (PCA) territory by means of a more comprehensive, modern vascular and cardiac study. During a 4-year period 74 consecutive patients (49 men, 25 women) with acute PCA infarction documented on CT (n = 74) and MRI (n = 41) were included in the study. Patients

Unilateral asterixis developed in a 56 year old man, 5 months after an infarct in the posterior cerebral artery territory, involving the posterolateral nuclear complex of the thalamus, documented by CT-scan and MRI. Unilateral asterixis in rarely reported in association with thalamic lesions and usually develops as an immediate postictal phenomenon. We discuss possible physiopathological mechanisms explaining the occurrence and the delayed onset of these involuntary movements.

OBJECTIVE: To identify the possible anatomic sites and risk factors for the development of confusion or delirium in patients with posterior cerebral arterial (PCA) infarction.
MATERIALS AND METHODS: Twenty-nine patients aged 34-86 years with PCA infarction were divided into two groups: one with and the other without perturbed mentation. The clinical and laboratory data, including neuroimages, were retrospectively reviewed. Student-t, chi-square and Fisher's exact tests were performed for data analysis.
RESULTS: Confusion or delirium tended to develop in the left (10/13) or bilateral (5/5) PC

In order to clarify the etiology of brain infarction in the posterior cerebral artery (PCA) territory, we investigated 85 patients (29 women and 56 men, mean age: 63.6 years old) with PCA territory infarction confirmed by computed tomography or magnetic resonance imaging. Patients with thalamic infarction alone were excluded from the present study. Cerebral angiography was performed in 72 subjects. The diagnosis of the mechanism of brain infarction was made on the basis of cerebral angiographic and echocardiographic (presence of cardiac disease as a potential embolic source) findings. Embolism

The thalamus is predominantly supplied by multiple small vessels originating from the posterior communicating artery and the P1 and P2 segments of the posterior cerebral artery (PCA). The artery of Percheron (AOP) is a rare anatomical variant of arterial supply to the thalamus. This single thalamic perforating branch supplies the bilateral thalamus so that occlusion results in a characteristic cerebral infarction. Herein, we report a case of posterior cerebral artery occlusion that developed into an AOP infarction. A 74-year-old man, who had undergone coronary artery bypass grafting 5 days pre

A spontaneous dissection of the carotid artery is a well-known cause of cerebral infarction, mostly due to an embolus from the affected vessel segment. For haemodynamic and anatomical reasons the territories of the middle and anterior cerebral arteries are usually involved. We report two cases of carotid artery dissections resulting in infarctions exclusively in the territory of the posterior cerebral artery. The underlying anatomical conditions were a fetal origin of the posterior cerebral artery and an embolic supra-ophthalmic occlusion of the internal carotid artery. To our knowledge simila

OBJECTIVE: Heading disorientation is a type of pure topographical disorientation. Reported cases have been very few and its underlying mechanism remains unclear. We report an unusual presentation of a 60-year-old man with recurrent transient heading disorientation heralding an acute posterior cerebral artery infarction.
DESIGN: Case report.
CONCLUSION: Acquired injury to the right retro-splenial region can result in a specific variant of topographical disorientation known as heading disorientation that may present as an atypical transient ischaemic attack-like symptom heralding acute cerebra

BACKGROUND: Fluid-attenuated inversion recovery hyperintense vessels (FHVs) are known to reflect stagnant or slow blood flow within the cerebral artery. FHVs are frequently observed in patients with acute cerebral infarction accompanied by arterial occlusion or significant stenosis of the anterior cerebral circulation. However, FHVs have not been studied in the context of posterior cerebral circulation. Thus, we investigated the prevalence of FHVs and its clinical significance in patients with acute posterior cerebral artery (PCA) territory infarction.
METHODS: In this retrospective study, co

BACKGROUND AND PURPOSE: Lateral thalamic infarction (LTI) is usually caused by small vessel disease (SVD), i.e., occlusion of the deep perforator. However, focal atherosclerotic posterior cerebral artery disease (PCAD) may produce LTI via thrombotic occlusion of the perforator. We aimed to investigate the prevalence of PCAD in LTI and differences in clinical and imaging findings between LTIs associated with PCAD and SVD.
METHODS: We retrospectively evaluated 58 consecutive patients with isolated LTI who underwent diffusion-weighted imaging (DWI) and MR angiography (MRA) within 7 days after st

BACKGROUND AND PURPOSE: Although cardiac embolism has been shown to be the leading stroke mechanism of posterior cerebral artery (PCA) territory infarction, intrinsic PCA atherosclerotic disease may play a more important role in regions where intracranial diseases are prevalent. We aimed to assess the etiologies and stroke pattern of PCA territory infarction in a Korean population.
METHOD: We reviewed consecutive patients with acute PCA territory infarction who underwent diffusion-weighted MRI (DWI) and magnetic resonance angiography (MRA) within 7 days after onset.
