Hemiplegia caused by inadvertent intra-carotid infusion of total parenteral nutrition.

A 24 year-old woman developed acute hemiplegia and a seizure following accidental catheterization of the right common carotid artery and total parenteral nutrition infusion. Magnetic resonance imaging of the brain showed lesions in the frontal lobe and putamen consistent with an ischemic stroke. Angiography through the central venous catheter confirmed its intra-arterial location. The patient's weakness improved after hyperbaric oxygen treatment. We concluded that stroke or seizures during total parenteral nutrition administration through a central venous catheter should alert one to the possi

Total parenteral nutrition by peripheral venous infusion in patients with oesophageal carcinoma.

The safety and efficacy of pre-operative intravenous feeding using a peripheral venous infusion technique was evaluated in 15 black patients with oesophageal carcinoma. Energy requirements were based on individual energy expenditure at rest calculated from oxygen consumption and the respiratory quotient. Patients received 7 600 non-protein kJ and 9,4 g nitrogen daily for 14 days. Although no measurable improvements in nutritional status were noted after intravenous feeding, peripheral venous alimentation using the dual energy system proved an effective method of preventing progressive weight l

Evidence supporting early nutritional support with parenteral amino acid infusion.

Postnatal growth of extremely low birth weight (ELBW) infants remains poor and does not come close to approximating rates of in utero growth. There is good evidence that early deficiencies in protein may be an important contributor to the poor growth outcomes observed in this population. Protein losses are inversely related to gestational age, and ELBW infants lose 1% to 2% of their total endogenous body protein stores each day that they receive glucose alone. It is now abundantly clear from a variety of studies that providing intravenous amino acids to sick premature infants in early postnata

Development and validation of an intelligent weight verification management system for neonatal ready-to-use parenteral nutrition infusions.

OBJECTIVE: To develop an intelligent weight verification management system for the quality of ready-to-use neonatal parenteral nutrition (PN) infusions and validate its reliability and application efficacy, aiming to enhance the quality and safety of these infusions. METHODS: An intelligent weight verification approach was established through drug density determination, maintenance of information parameters, automatic calculation of theoretical weight by the hospital information system (HIS), and actual measurement using a high-precision electronic scale. A total of 350 bags of neonatal PN or

Cost savings of outpatient parenteral antimicrobial therapy using a digital infusion pump.

BACKGROUND AND OBJECTIVES: Outpatient parenteral antimicrobial therapy (OPAT) enables IV antibiotic administration outside hospital settings and has been found to reduce costs and improve patient satisfaction. In Norway, OPAT has been less established due to the preference for narrow-spectrum antibiotics often requiring multiple daily doses. We undertook a cost evaluation of an OPAT programme using a continuous ambulatory delivery device (CADD), and comparison with costs of inpatient treatment for equivalent IV antibiotic courses. MATERIALS AND METHODS: A retrospective cost analysis was condu

Induration at Injection or Infusion Site May Reduce Bioavailability of Parenteral Phenobarbital Administration.

BACKGROUND: Phenobarbital is well tolerated and effective for controlling agitation or preventing convulsion at the end of life. No information is available concerning parenteral bioavailability of phenobarbital when induration develops at the injection or infusion site. We investigated whether induration at injection or infusion site is related to phenobarbital bioavailability via parenteral routes of continuous subcutaneous infusion and intermittent subcutaneous or intramuscular injection. METHODS: A retrospective analysis was conducted on the medical data obtained from 18 patients who rece

Insulin infusion with parenteral nutrition in extremely low birth weight infants with hyperglycemia.

From Nov. 7, 1983, to Nov. 6, 1986, all infants with birth weight less than or equal to 1000 gm admitted to Oregon Health Sciences University who had persistent hyperglycemia and glycosuria were treated with graded insulin infusion while energy intake was increased to at least 100 kcal/kg/day (419 kilojoules/kg/day). The records of these infants were reviewed to define the clinical characteristics of infants likely to develop hyperglycemia and to see whether insulin administration would allow goals for energy intake to be met. There were 76 surviving infants; 34 received insulin and 42 did not

[Influence of total parenteral nutrition in immature rats: effect of glutamine infusion on bowel integrity].

Total parenteral nutrition (TPN) is accepted important therapeutic adjunct in spite of many complications for management of pediatric patients who aren't allowed to eat. Recently, bacterial translocation was added to the complications of TPN. The purpose of this study was to examine the influence of TPN on the gut in immature rats. 65g rats were randomized to one of four groups: Group C (control) received food and water ad libitum. Group P (TPN) received standard TPN solution. Group G (TPN+GLT) received glutamine (7g/400ml of TPN). Group E (TPN with enhanced protein) received TPN solution with

Emergency therapy of asthma: comparison of the acute effects of parenteral and inhaled sympathomimetics and infused aminophylline.

Forty-eight patients who presented with acute episodes of asthma were randomized to treatment with subcutaneously administered epinephrine, inhaled isoproterenol, or intravenously administered aminophylline. The patients' couses were followed clinically and with spirometry. Although there were no significant differences between the groups before treatment for any measured variable, at the end of 1 hour, the mean improvement inforced expiratory volume in one second (FEV1) was significantly greater for patients treated with epinephrine (0.76 L) or nebulized isoproterenol (0.79 L) than for those

Successful parenteral nutrition weaning using a chyme re-infusion device: Case reports.

BACKGROUND: Chyme reinfusion involves returning chyme (partially digested food) collected in an ostomy appliance from the proximal limb of a fistula or stoma back into the distal limb. With development of a chyme reinfusion device the process can now be undertaken without manual manipulation. CASE REPORT: Two case reports are presented showing how successful chyme re-infusion can eliminate the need for parenteral nutrition, a cornerstone in the treatment of intestinal failure. The practical steps taken in the weaning process are described. CONCLUSION: Chyme reinfusion can be a realistic alte

Variations in adverse events associated with different infusion modalities of parenteral nutrition: A pharmacovigilance study.

BACKGROUND: Parenteral nutrition can be administered through various infusion modalities. However, limited research has examined the differences in adverse reactions associated with these modalities. METHODS: Data from the Food and Drug Administration adverse event reporting system database were analyzed using disproportionality analysis to identify and compare adverse event signals across different infusion modalities. RESULTS: A total of 1642 adverse event reports related to parenteral nutrition were included: 356 in the all-in-one group, 85 in the two-in-one group, 1086 in the fat emulsio

Risk factors for nephrotoxicity associated with continuous vancomycin infusion in outpatient parenteral antibiotic therapy.

OBJECTIVES: Continuous vancomycin infusion is increasingly used for outpatient management of infections, but the relationship between vancomycin and nephrotoxicity is controversial. We investigated the risk factors associated with nephrotoxicity in this setting. METHODS: A retrospective cohort study of patients receiving continuous vancomycin infusion as outpatient parenteral antibiotic therapy (OPAT) was performed. The likelihood of developing nephrotoxicity (> or =50% increase in serum creatinine from baseline) was evaluated in relation to demographic variables, underlying co-morbidities, i

Wernicke encephalopathy and beriberi during total parenteral nutrition attributable to multivitamin infusion shortage.

OBJECTIVE: Wernicke encephalopathy (WE) is an acute neurologic disorder characterized by a triad of ophthalmoplegia, ataxia, and mental confusion. WE is attributable to thiamine (vitamin B1) deficiency. Beriberi is the systemic counterpart of thiamine deficiency and often manifests in cardiovascular collapse. WE is usually associated with alcoholism and malnutrition. It has also been seen in people with gastrointestinal diseases with malabsorption. Patients who have received total parenteral nutrition (TPN) without proper replacement of thiamine have also developed WE. Since November 1996, the

Tissue storage of vitamins A and E in rats drinking or infused with total parenteral nutrition solutions.

The total parenteral nutrition (TPN) rat and its sham-operated control were used as a model to compare the metabolism and storage of vitamins A and E when they are administered intravenously or orally. Male Fisher rats were depleted of both vitamins for several months with a diet free of vitamins A or E, but containing retinoic acid for growth. TPN solutions containing aqueous dispersions of retinol, retinyl palmitate and dl-alpha-tocopheryl acetate were infused at 2.3 ml/h into the jugular veins of 10 TPN rats. Eight sham-operated control rats drank similar volumes from food cups. TPN rats re
