Further investigations into the potentiation of infection by intra-articular injection of polysulfated glycosaminoglycan and the effect of filtration and intra-articular injection of amikacin.

Polysulfated glycosaminoglycan (PSGAG) recently have been reported to potentiate the infectivity of Staphylococcus aureus in horses with experimentally induced septic arthritis. Four groups of 8 horses each had 1 midcarpal joint injected with approximately 33 viable colony-forming units (CFU) of S aureus plus either 1 ml of saline solution (group 1), 250 mg of PSGAG (group 2), 250 mg of PSGAG passed through a 0.6-microns filter (group 3), or 250 mg of PSGAG plus 125 mg of amikacin (group 4). Horses that developed clinical signs consistent with sepsis were euthanatized, and samples were collect

Intra-articular Injectates: What to Use and Why.

Intra-articular injections are a nonsurgical treatment modality that can be used to manage osteoarthritis, naturally occurring or surgically induced acute synovitis, and intra-articular ligamentous or tendon injury. This option may be assistive for patients in which other conservative modalities are ineffective, or in conjunction with other forms of treatment. It may also be used as the primary treatment. Injectates labeled for use in companion animal joints include corticosteroids and viscosupplements. Additional injectates, that are not specifically approved for use in companion animals are

Safety and efficacy of intra-articular injection of the hip.

The aim of the present study was twofold: a) to establish the therapeutic efficacy of the hip injection with marcaine and steroid; (b) to establish the safety of the procedure with respect to possible bacterial contamination at the time of passage of needle into the hip joint. Thirty-five patients with osteoarthritis of the hip were injected. All patients were admitted as day cases and the procedure was performed in a laminar flow theatre under full aseptic conditions. The hip was aspirated before injection. Aspirate and injecting needles were sent for microbiological examination. Both aerobic

Drug-loaded nanoparticles for intra-articular injection.

Drug loaded nanoparticles (NPs) were developed as a model intra-articular injection (IAI) formulation to mitigate early stage osteoarthritis (OA). Different types of celecoxib-loaded nanoparticles were prepared by a hybrid method that combines homogenization and solvent evaporation. The hydrodynamic diameter of the nanoparticles prepared were approximately 200 nm (PLLA: 238 ± 19 nm; PCL: 249 ± 28 nm; PLA: 252 ± 18 nm; PMMA: 234 ± 21), and zeta potential were about -40 mV (PLLA: -45.3 ± 2.3 mV; PCL: -38.0 ± 0.9 mV; PLA: -44.4 ± 3.2 mV; PMMA: -45.5 ± 2.7 mV). Gene Delivery to Joints by Intra-Articular Injection.

Most forms of arthritis are incurable, difficult to treat, and a major cause of disability in Western countries. Better local treatment of arthritis is impaired by the pharmacokinetics of the joint that make it very difficult to deliver drugs to joints at sustained, therapeutic concentrations. This is especially true of biologic drugs, such as proteins and RNA, many of which show great promise in preclinical studies. Gene transfer provides a strategy for overcoming this limitation. The basic concept is to deliver cDNAs encoding therapeutic products by direct intra-articular injection, leading

Intra-Articular Steroid Hip Injections Association with Fracture: A Case Series.

INTRODUCTION: Osteoarthritis (OA) is one of the most common orthopedic conditions in the world. Corticosteroid injections (CSIs) are a common treatment option for these patients, with many different options available to patients. The effects vary based on the individual, with some patients experiencing no relief. CSIs work through a variety of immunosuppressive effects, yet the effects on bone health are still debated. Some studies have shown detrimental effects on bone, ranging from bone degeneration to causing fracture. This case series presents a potential link to femoral neck fracture (FNF

[Arthrocentesis of the temporomandibular joint and intra-articular injections : An update].

INTRODUCTION: Arthocentesis of the temporomandibular joint combined with intra-articular washout and, more recently, intra-articular injection of pharmacological agents has been developed from the 1990s and is nowadays extensively in use for the treatment of temporomandibular dysfunctions (TMDs). The goal of our work was to answer 3 questions: 1. Is intra-articular washout effective for the treatment of TMDs ? 2. What kind of pharmacological agents may nowadays be injected in addition to washout and are these injections useful ? 3. What is the place of these treatments in the treatment strateg

Clinical evaluation of intra-articular injections for lumbar facet joint pain.

Lumbar facet joint (LFJ) pain is a common cause of low back pain. Thirty-four consecutive patients with a clinical diagnosis of LFJ pain received injections of a local anaesthetic agent and a long-acting corticosteroid agent under radiological control. Nineteen patients experienced immediate lessening of their pain after the injection of local anaesthetic agent, their response supporting the clinical diagnosis. Twelve of these 19 patients experienced clinically significant relief of pain for more than six months. LFJ injection offers improved diagnosis and treatment of LFJ pain.

Intra-articular corticosteroid injections increase the risk of requiring knee arthroplasty.

AIMS: Recent studies have suggested that corticosteroid injections into the knee may harm the joint resulting in cartilage loss and possibly accelerating the progression of osteoarthritis (OA). The aim of this study was to assess whether patients with, or at risk of developing, symptomatic osteoarthritis of the knee who receive intra-articular corticosteroid injections have an increased risk of requiring arthroplasty. METHODS: We used data from the Osteoarthritis Initiative (OAI), a multicentre observational cohort study that followed 4,796 patients with, or at risk of developing, osteoarthri

Osteomyelitis of the Knee Following Intra-Articular Injections: A Case Series.

Septic arthritis of the knee is rare in adults. This leads to difficulty in making early diagnosis that invariably leads to delayed treatment with consequent destruction of the joint. The delay in diagnosis is largely attributed to absence of clinical signs of flagrant infection. Reported are three adult patients who presented with painful swollen knee and inability to walk few weeks after intra-articular injection for osteoarthritis. This paper discusses the cases in which the difficulties in the early diagnosis of septic arthritis of knee in adults led to the development of osteomyelitis.

Intra-articular injections of hyaluronic acid (viscosupplementation) in the haemophilic knee.

Intra-articular injections (IAIs) of hyaluronic acid, also called viscosupplementation, can be used for the treatment of radiological haemophilic arthropathy of the knee, that is when mild-to-moderate degenerative changes can be visualized on plain radiographs. This article aims to define the efficacy of IAIs of hyaluronic acid in the treatment of radiological haemophilic arthropathy of the knee. A review of recent literature on the topic has been performed. The literature seems to support the use of hyaluronic acid in the treatment of knee osteoarthritis, because it diminishes pain and improv

Multiple Hip Intra-Articular Steroid Injections Increase Risk of Periprosthetic Joint Infection Compared With Single Injections.

BACKGROUND: Patients with hip osteoarthritis often temporize their symptoms with multiple intra-articular steroid hip injections (IASHIs) before undergoing total hip arthroplasty (THA). Although there is recent evidence to suggest that IASHI can lead to an increased risk of future periprosthetic joint infection (PJI), the potential increase in risk of PJI after multiple IASHIs compared with single IASHI remains largely unknown. The aim of the study was to evaluate whether multiple IASHIs are associated with increased risk of PJI compared with single IASHI in THA patients. METHODS: We evaluate
