Small Intestine-Targeted Long-Acting Oral Insulin Formulation Based on Engineered Milk Protein Nanoparticles.

Insulin therapies remain essential for glycemic control in diabetes mellitus, yet conventional subcutaneous injection is associated with poor patient compliance, risk of hypoglycemia, and other adverse effects. Oral insulin formulations offer a promising alternative by improving patient adherence and mimicking the endogenous insulin pathway. However, their clinical trials, mostly in enteric capsules and tablets, have yielded limited efficacy due to low insulin bioavailability. In this study, we prepared an oral insulin formulation by coencapsulating insulin and the permeation enhancer sodium c

A FAERS database study on visual impairment adverse events associated with three long-acting insulin analogues.

PURPOSE: To utilize the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) to systematically evaluate and compare the risk signals, clinical characteristics, and potential influencing factors of visual impairment adverse events (AEs) associated with three major long-acting insulin analogues (insulin degludec, insulin detemir, and insulin glargine). METHODS: This retrospective pharmacovigilance study analyzed FAERS data from Q1 2004 to Q2 2025. Reports explicitly listing any of the three target insulins as the "primary suspect" were extracted. Data extraction and deduplic

Glycogenic hepatopathy following attempted suicide by long-acting insulin overdose in patient with type 1 diabetes.

Patients with poorly controlled insulin-dependent type 1 or type 2 diabetes rarely present with glycogenic hepatopathy, which is characterized by hepatomegaly and liver enzyme abnormalities. Glycogenic hepatopathy occurs as a consequence of excessive accumulation of glycogen in hepatocytes caused by insulin. We report a young male patient with type 1 diabetes mellitus who developed glycogenic hepatopathy following a suicide attempt by insulin overdose via subcutaneous injection. The patient's medication/nutrition compliance and adherence to insulin were poorly controlled due to comorbid schizo

Population Pharmacokinetic and Pharmacodynamic Modeling to Support Molar Dose Ratio Determination of Long-Acting Insulin Analogs.

BACKGROUND AND OBJECTIVE: INS068 is a new soluble, long-acting insulin analog intended to cover basal insulin requirements in patients with type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM). The purpose of this study was to determine the molar dose ratio of INS068 to insulin degludec (IDeg) by comparing the clinical pharmacokinetic (PK) and pharmacodynamic (PD) profiles of INS068 and IDeg. The PD endpoints include glucose infusion rate (GIR) and glycated hemoglobin (HbA1c). METHOD: Population pharmacokinetic and pharmacodynamic (PopPK/PD) analysis was performed to characteri

[Effects of long-acting insulin degludec on type 2 diabetic hemodialysis patients with poor glycemic control].

BACKGROUND: The ratio of elderly complicated diabetes mellitus (DM) to all hemodialysis (HD) patients has increased. The number of elderly DM patients with poor glycemic control has also increased due to complications, such as dementia. By means of appropriate glycemic control, the risk of cardiovascular disease might decrease among diabetic HD patients, and improvement of their prognosis could be expected. When glycemic control is poor when treating diabetic HD patients with oral hypoglycemic agents, insulin treatment could be indicated. METHODS: Using continuous glucose monitor (CGM), we ev

An update on the treatment of type 1 and type 2 diabetes mellitus: focus on insulin detemir, a long-acting human insulin analog.

Basal insulin analogs are used to minimize unpredictable processes of NPH insulin. Modification of the human insulin molecule results in a slower distribution to peripheral target tissues, a longer duration of action with stable concentrations and thus a lower rate of hypoglycemia. Insulin detemir is a basal insulin analog that provides effective therapeutic options for patients with type 1 and type 2 diabetes. For glycemic control, no significant differences were found in HbA1c levels compared with NPH and insulin glargine. It is comparable with insulin glargine in significantly reducing rate

Ultra-long-acting insulin degludec has a flat and stable glucose-lowering effect in type 2 diabetes.

AIMS: Insulin degludec (IDeg) is a new-generation, ultra-long-acting basal insulin that forms soluble multihexamers upon subcutaneous injection, resulting in a depot from which IDeg is absorbed slowly and continuously into circulation. This double-blind, two-period, incomplete block cross-over trial investigated the pharmacodynamic and pharmacokinetic properties of IDeg at steady state (SS) in people with type 2 diabetes. METHODS: Forty-nine subjects treated with insulin without concomitant oral anti-diabetic drugs were given IDeg (0.4, 0.6 and/or 0.8 U/kg) once daily for two 6-day periods, s

Control of blood glucose levels in the streptozotocin diabetic rat using a long-acting heat-treated insulin.

Diurnal plasma glucose levels have been studied two hourly in streptozotocin diabetic rats during insulin treatment. Protamine Zinc Insulin induced a very steep fall in plasma glucose level from 359 +/- 100 (SD) mg/100 ml to 91 +/- 49 mg/100 ml within two hours. Plasma glucose was then low (13-60 mg/100 ml) until after 18 hours when an equally steep rise in glucose concentration ocurred. Six other insulin preparations and several insulin treatment regimens were tested with the aim of normalising the 24 hour plasma glucose profile of streptozotocin diabetic rats. One preparation, a non-commerci

Closed-loop subcutaneous insulin infusion algorithm with a short-acting insulin analog for long-term clinical application of a wearable artificial endocrine pancreas.

Considering the management and safety of the insulin delivery route when a wearable artificial endocrine pancreas is applied to ambulatory diabetic patients on a long-term basis, we developed a s.c. insulin infusion algorithm by analyzing the dynamics of a s.c. injected short-acting insulin analog (Insulin Lispro) by a three-compartment model. Principally the insulin infusion algorithm was developed as a transfer function with the first-order delay in both proportional and derivative actions to blood glucose concentrations. The parameters for this algorithm were calculated to simulate a physio

Early vs. late initiation of long-acting basal insulin during IV insulin in pediatric diabetic ketoacidosis: a GRADE-assessed systematic review and meta-analysis.

UNLABELLED: The effectiveness of starting long-acting basal insulin during the intravenous (IV) phase of pediatric diabetic ketoacidosis (DKA) is still a topic of debate. We question whether it can improve outcomes without causing additional harm. We performed a systematic review following PRISMA guidelines and a random-effects meta-analysis (PROSPERO: CRD420251155626) of studies involving patients under 18 years with DKA. We defined "early basal" as starting subcutaneous glargine or detemir while IV insulin was still being administered, with a planned overlap of four hours or more. Comparator

Biocompatible Nanogels Synthesized via RAFT Polymerization for Long-Acting Onset-Controllable Glucose-Responsive Insulin Delivery in Diabetic Mice.

Glucose-responsive insulin delivery (GRID) nanocarriers that can respond to glucose fluctuations rapidly and deliver insulin autonomously for long-acting normoglycemic control with low risk of hypoglycemia are highly desirable but still unattained for diabetic care. Herein, we report a class of biocompatible nanogels with controllable onset of glucose response for self-regulated insulin delivery. The narrowly distributed nanogels are prepared via controlled polymerization using the poly[oligo(ethylene glycol) methyl ether methacrylate (Mn = 500)] (pMEO9MA) as macro-chain transfer agents copoly

Long-acting Insulin in Diabetes Therapy: In Silico Clinical Trials with the UVA/Padova Type 1 Diabetes Simulator.

The University of Virginia /Padova Type 1 Diabetes (TID) simulator has been widely used for testing artificial pancreas controllers, and, recently, novel insulin formulations and glucose sensors. However, a module describing the pharmacokinetics of the new long-acting insulin analogues is not available. The aim of this contribution is to reproduce multiple daily insulin injection (MDI) therapy, with insulin glargine 100 U/mL (Gla-100) as basal insulin, using the TID simulator. This was achieved by developing a model of Gla-100 and by incorporating it into the simulator. The methodology describ

[Lipoatrophy after use of long acting insulin glargine analogue in a 32-year-old patient with type 1 diabetes].

A case of a 32-year-old woman with type 1 diabetes diagnosed 1.5 year before presention. The patient was referred to the diabetology department due to decompensation of diabetes and excessive hypodermic atrophy of both thighs. Early symptoms of lipoatrophy appeared after 1.5-2 months of insulin glargine use, in spite of frequently changed hypodermic needles and injection sites on both thighs. Simultaneously, deterioration of diabetes compensation was observed (hyperglycaemia between meals and in the morning), which was corrected by the patient with extra injections of a short acting analogue.
