National perspectives of barriers by insurance and pharmacy benefit managers in pediatric inflammatory bowel disease.

OBJECTIVES: Early biologic initiation, dose optimization, and therapy modification based on disease phenotype are key to improving outcomes in pediatric inflammatory bowel disease (IBD). Enacting optimized therapy is often impeded by the lack of United States Food and Drug Administration (FDA) approval for pediatric use of newer advanced therapies or intensified dosing regimens. These barriers often result in initial payor denial of coverage and added prior authorization burden on physicians, leading to patient delays in medication initiation and therapy optimization, and development of diseas

Benefits of and strategies to update premium rates in the US National Flood Insurance Program under climate change.

The United States' National Flood Insurance Program (NFIP) has accumulated over $20 billion in debt to the US Treasury since 2005, partly due to discounted premiums on homes in flood-prone areas. To address this issue, FEMA introduced Risk Rating 2.0 in October 2021, which is able to assess and charge more accurate and equitable rates to homeowners. However, rates must be continually updated to account for increasing flood damage caused by sea level rise and more intense hurricanes due to climate change. This study proposes a strategy to adopt updated premium rates that account for climate cha

The effect of health insurance coverage on medical care utilization and health outcomes: Evidence from Medicaid adult vision benefits.

Increasing the proportion of adults that have regular, comprehensive eye exams and reducing visual impairment due to uncorrected refractive error and other common eye health problems are federal health objectives. We examine the effect of vision insurance on eye care utilization and vision health outcomes by taking advantage of quasi-experimental variation in Medicaid coverage of adult vision care. Using a difference-in-difference-in-difference approach, we find that Medicaid beneficiaries with vision coverage are 4.4 percentage points (p<0.01) more likely to have seen an eye doctor in the pas

The trade-off between coverage and benefit: spatiotemporal variation of China's health insurance and its influencing factors.

BACKGROUND: Achieving universal health coverage has become a global priority, yet internal regional inequalities remain a major challenge within health systems worldwide. China has achieved universal health insurance coverage; however, substantial inequalities persist across provinces, shaped by decentralized welfare governance and uneven socioeconomic development. Existing research has primarily examined urban–rural and population-based inequities, but comprehensive national-level evidence on regional disparities remains limited, particularly since UHC was achieved. This study investigates th

The Impact of Unemployment Insurance Benefit Cuts on Mental Health: Evidence From Early Pandemic Program Expirations.

This study is the first to estimate the effects of unemployment insurance (UI) benefit contractions on mental health. We leverage a unique quasi-natural experiment: early termination of the Federal Pandemic Unemployment Compensation (FPUC) and Pandemic Unemployment Assistance (PUA) programs across more than 20 states during the summer of 2021. Using high-frequency data from the Census Bureau's Household Pulse Survey (HPS), our difference-in-differences estimates suggest that early withdrawal from FPUC and PUA increases the probability of reporting anxiety symptoms among working-age adults with

Utilization of silver diamine fluoride by dentists in Canada: a review of the Non-Insured Health Benefits Dental Claims database.

INTRODUCTION: In August 2020, Indigenous Services Canada's Non-Insured Health Benefits (NIHB) program approved reimbursement for silver diamine fluoride (SDF), a dental caries-arresting agent, for NIHB-eligible clients of all ages. We investigated the utilization of SDF for NIHB-eligible children and youth and determined trends and regional differences. METHODS: The NIHB program provided data on SDF claims for children and youth (< 17 years) from 1 August 2020 to 31 July 2022. We derived descriptive statistics and calculated rates of SDF application by dividing the number of children and yout

An actuarial analysis shows that offering lung cancer screening as an insurance benefit would save lives at relatively low cost.

Lung cancer screening is not established as a public health practice, yet the results of a recent large randomized controlled trial showed that screening with low-dose spiral computed tomography reduces lung cancer mortality. Using actuarial models, this study estimated the costs and benefits of annual lung cancer screening offered as a commercial insurance benefit in the high-risk US population ages 50-64. Assuming current commercial reimbursement rates for treatment, we found that screening would cost about $1 per insured member per month in 2012 dollars. The cost per life-year saved would b
