Rural Medicine and the Farmers' "Right to Health" in 1980s South Korea: The Rural Health Insurance and the Health Insurance Reform Movement.

The history of Korean health security is usually told as a compressed achievement, from workplace insurance in 1977 to universal coverage in 1989 and unified National Health Insurance in 2000. Departing from that framework, this article places the Rural Health Insurance of 1988, which extended coverage to farming and fishing villages, within the longer history of rural medical marginalization since liberation, in which the equitable distribution of medical care remained an unmet task across regimes. It asks how farmers' health briefly emerged as a social category and was then reduced to a disp

Surfactant lung delivery with LISA and InSurE in adult rabbits with respiratory distress.

BACKGROUND: In preterm infants, InSurE (Intubation-Surfactant-Extubation) and LISA (less invasive surfactant administration) techniques allow for exogenous surfactant administration while reducing lung injury associated with mechanical ventilation. We compared the acute pulmonary response and lung deposition of surfactant by LISA and InSurE in surfactant-depleted adult rabbits. METHODS: Twenty-six spontaneously breathing surfactant-depleted adult rabbits (6-7 weeks old) with moderate RDS and managed with nasal continuous positive airway pressure were randomized to 3 groups: (1) 200 mg/kg of s

Long-term opioid therapy for chronic non-cancer pain in Germany.

BACKGROUND: No data are available on the prevalence and predictors of (high-dose) long-term opioid therapy (LTOT) and on abuse/addiction of prescribed opioids by patients with chronic non-cancer pain (CNCP) outside North America and Scandinavia. METHODS: We analysed randomly selected claims records of 870,000 persons (10% of insureds) in a large German medical health insurance organization during the fiscal year 2012. RESULTS: The prevalence of LTOT prescriptions (defined by at least one opioid prescription per quarter for at least three consecutive quarters) for CNCP was 1.3% of all insured

The prevalence and geographic distribution of Crohn's disease and ulcerative colitis in the United States.

BACKGROUND & AIMS: Previous US studies of inflammatory bowel disease (IBD) prevalence have sampled small, geographically restricted populations and may not be generalizable to the entire nation. This study sought to determine the prevalence of Crohn's disease (CD) and ulcerative colitis (UC) in a large national sample and to compare the prevalence across geographic regions and other sociodemographic characteristics. METHODS: We analyzed the health insurance claims for 9 million Americans, pooled from 87 health plans in 33 states, and identified cases of CD and UC using diagnosis codes. Preval

Challenges toward achieving universal health coverage in Ghana, Kenya, Nigeria, and Tanzania.

Although many sub-Saharan African countries have made efforts to provide universal health coverage (UHC) for their citizens, several of these initiatives have achieved little success. This study aims to review the challenges facing UHC in Ghana, Kenya, Nigeria, and Tanzania, and to suggest program or policy changes that might bolster UHC. Routine data reported by the World Bank and World Health Organization, as well as annual reports of the national health insurance schemes of Ghana, Kenya, Nigeria, and Tanzania, were analyzed. The data were supplemented by a review of published and gray liter

Possible objectives and resulting entitlements of essential health care packages.

The notion of a defined 'core package of essential health care services' has appeared in many different health reform proposals in the 1990s. This paper attempts to explore the possible objectives of the 'core package' component of health care reform. Two board applications are apparent: the use of essential packages to ration scarce public funds and the incorporation of a minimum benefit package into 'managed competition' type reforms, where they constitute a mandated minimum level of private insurance cover. Eight possible objectives for an essential benefit package are described: To protect

Multiplicative Impact of Adverse Social Determinants of Health on Outcomes in Lupus Nephritis: A Meta-analysis and Systematic Review.

OBJECTIVE: Social determinants of health (SDoH) likely contribute to outcome disparities in lupus nephritis (LN). Understanding the overall burden and contribution of each domain could guide future health equity-focused interventions to improve outcomes and reduce disparities in LN. Objectives of this meta-analysis were to 1) determine the association of overall SDoH and specific SDoH domains on LN outcomes and 2) develop a framework for the multidimensional impact of SDoH on LN outcomes. METHODS: We performed a comprehensive search of studies measuring associations between SDoH and LN outcom

Implications of Kenya's Enhanced Public Officers Medical Scheme on Equity in Strategic Purchasing and Progress Towards Universal Health Coverage.

BACKGROUND: Kenya transitioned its health financing from the National Health Insurance Fund (NHIF) to the Social Health Authority (SHA) in 2024. This model was designed to advance efficiency and equity, ultimately leading to universal health coverage (UHC). Shortly thereafter, the government introduced a parallel Enhanced Public Officers Medical Scheme Fund (POMSF), tailored to privilege public officers through dedicated funding and benefits. This article explores the implications of this targeted scheme on equity in strategic purchasing and the journey towards UHC. METHODOLOGY: The article i

Prevalence and multilevel factors associated with clinical breast examination uptake among women in sub-Saharan Africa: a systematic review and meta-analysis.

BACKGROUND: Clinical breast examination (CBE) is a feasible early-detection method in low-resource settings, but critical gaps remain regarding its uptake in sub-Saharan Africa. We estimated the prevalence of CBE uptake and synthesized multilevel factors associated with uptake, organized within a socio-ecological framework. METHODS: We conducted a PRISMA-guided systematic review and meta-analysis of cross-sectional studies assessing self-reported CBE uptake. PubMed, Embase, Web of Science, CINAHL, Africa-Wide Information, Google Scholar, and gray literature were searched. Random-effects meta-

Increases in benzodiazepine prescribing for postpartum anxiety during COVID-19.

PURPOSE: Postpartum mood disorders affect many women following childbirth. Prescribing medication for depression and anxiety is one strategy for the effective treatment of postpartum mood disorders. Left untreated, mothers experiencing these disorders and their infants face increased risks of adverse health outcomes. Little is known about how diagnosis and treatment of postpartum mood disorders changed during COVID-19. METHODS: We used a retrospective pooled cross-sectional design in a sample of privately-insured postpartum women in U.S. claims data from January 1, 2016 to December 31, 2020.

Does more generous health insurance reimbursement lead to moral hazard? Evidence from large-scale administrative data in China.

This paper exploits the discrete increase in reimbursement rates at the statutory retirement age as a quasi-natural experiment to examine ex post moral hazard in inpatient care in China. Using nationally representative administrative data covering more than 450,000 hospitalization records across 29 provinces, we implement a cohort difference-in-differences design to identify the causal effect of insurance generosity on healthcare utilization. We find that a 5-percentage-point increase in reimbursement rates increases length of stay by 1.73 days and total inpatient expenditures by RMB 709.98. T

Does Volunteering Give an Extra Health Boost? Volunteering and High Blood Pressure Among Insured and Uninsured U.S. Adults in Late Midlife.

INTRODUCTION: High blood pressure (HBP) is a major predictor of cardiovascular disease, a leading cause of death in the U.S. Growing evidence suggests that formal volunteering in later life is associated with a lower prevalence of HBP; however, it remains unclear whether these benefits extend to adults in late midlife and whether they differ by health insurance status. METHODS: This prospective cohort study analyzed data from the 2012-2018 Health and Retirement Study. The analytic sample included 4,169 respondents between the ages of 50 and 64. Because blood pressure was measured in half the

The association between health insurance status and utilization of health services in rural Northern Ghana: evidence from the introduction of the National Health Insurance Scheme.

BACKGROUND: Many households in low- and middle-income countries face financial hardships due to payments for health care, while others are pushed into poverty. Risk pooling and prepayment mechanisms help to lessen the impact of the costs of care as well as assisting to achieve universal health coverage (UHC). Ghana implemented the National Health Insurance Scheme (NHIS) for the promotion of access to health services for all Ghanaians. In this paper, we examined the association between health insurance status and utilization of outpatient and inpatient health services in rural poor communities.
