Intermittent Interatrial Block in a Patient With Recurrent Transient Ischemic Attacks.

Interatrial block (IAB) is a conduction disorder linked to atrial fibrillation (AF) and ischemic stroke. Intermittent IAB, often triggered by premature atrial complexes (PACs), may precede AF and increase thromboembolic risk. We present a case of embolic stroke of undetermined source (ESUS) with intermittent partial IAB detected on ECG. Serial ECGs and Holter monitoring were analyzed. Intermittent partial IAB with dynamic P-wave changes was observed. No AF or alternative embolic source was identified. Intermittent IAB may indicate atrial vulnerability in ESUS. Recognizing subtle ECG changes co

Right bundle branch block as a marker for interatrial septal abnormalities.

BACKGROUND: Interatrial septal anomalies, which include atrial septal defect, patent foramen ovale, and atrial septal aneurysm, are common disorders among adult patients. Early detection of interatrial septal anomalies is important in order to prevent haemodynamic consequences and/or thromboembolic events. Electrocardiogram offers some clues that should serve as hints for detection of interatrial abnormalities. The aim of our study was to analyse the interatrial septum by transoesophageal echocardiography in patients with electrocardiogram signs of right bundle branch block and in those withou

Value of Interatrial Block for the Prediction of Silent Ischemic Brain Lesions.

INTRODUCTION: Previous studies demonstrated that interatrial block (IAB) is associated with atrial fibrillation (AF) in different clinical scenarios. The aim of our study was to determine whether IAB could predict silent ischemic brain lesions (sIBL), detected by magnetic resonance imaging (MRI). METHODS: Patients presented to a neurology clinic with transient ischemic attack (TIA) symptoms and underwent brain MRI were included to the study. sIBL were defined as lesions without corresponding clinical symptoms regarding lesion localization evaluated by two neurologists. A 12-lead surface ECG w

Presystolic wave as a predictor of interatrial block in patients with supraventricular tachycardia.

The identification of interatrial block (IAB) through electrocardiography (ECG) has been correlated with an elevated likelihood of developing atrial fibrillation (AF) and stroke. IAB is diagnosed by evaluating P-wave prolongation on a surface ECG. The presystolic wave (PSW) is an echocardiographic marker determined by pulse-wave examination of the aortic root during late diastole. As IAB and PSW share similar pathophysiological mechanisms, we speculated that PSW, as a component of the P wave, might be useful in predicting IAB. In the present study, we aimed to determine the relationship betwee

Interatrial block and interatrial septal thickness in patients with paroxysmal atrial fibrillation undergoing catheter ablation: Long-term follow-up study.

BACKGROUND: Interatrial block (IAB) is a strong predictor of recurrence of atrial fibrillation (AF). IAB is a conduction delay through the Bachman region, which is located in the upper region of the interatrial space. During IAB, the impulse travels from the right atrium to the interatrial septum (IAS) and coronary sinus to finally reach the left atrium in a caudocranial direction. No relation between the presence of IAB and IAS thickness has been established yet. OBJECTIVE: To determine whether a correlation exists between the degree of IAB and the thickness of the IAS and to determine wheth

Interatrial Conduction Block in Pediatric Patients with Ostium Secundum Atrial Septal Defect.

Background: Atrial arrhythmias represent a frequent long-term complication in patients with atrial septal defects (ASDs). Interatrial block (IAB), reflecting delayed or impaired conduction across Bachmann's bundle, has been proposed as an electrophysiological substrate predisposing to atrial arrhythmogenesis. However, evidence regarding its prevalence and clinical correlates in pediatric patients with ASD remains limited. The present study aimed to characterize interatrial conduction patterns and assess the occurrence of IAB in children with large secundum ASD undergoing percutaneous closure.

Cardiovascular Complications of Interatrial Conduction Block: JACC State-of-the-Art Review.

Interatrial block (IAB) is an electrocardiographic pattern describing the conduction delay between the right and left atria. IAB is classified into 3 degrees of block that correspond to decreasing conduction in the region of Bachmann's bundle. Although initially considered benign in nature, specific subsets of IAB have been associated with atrial arrhythmias, elevated thromboembolic stroke risk, cognitive impairment, and mortality. As the pathophysiologic relationships between IAB and stroke are reinforced, investigation has now turned to the potential benefit of early detection, atrial imagin

Preoperative interatrial block is associated with postoperative atrial fibrillation after cardiac surgery.

OBJECTIVES: Atrial fibrillation is a common complication after heart surgery, potentially leading to chronic atrial fibrillation, heart failure and mortality. The aim of this study was to explore the relationship between preoperative interatrial block and the occurrence of postoperative atrial fibrillation. METHODS: Perioperative 12-channel electrocardiographies of patients in sinus rhythm scheduled for heart surgery, including bypass and/or valve surgery, were analysed. Patients with pre-existing atrial fibrillation, amiodarone therapy, atrioventricular block II or III, or pacemaker were exc

DDD pacing and interatrial conduction block: importance of optimal AV interval setting.

The case of a 83-year-old patient undergoing DDD pacemaker implantation for sick sinus syndrome with postimplant detection of advanced interatrial conduction block is described. At nominal AV interval programming values (175 ms), absence of P wave following an atrial spike was observed, and the presence of an interatrial conduction disturbance was demonstrated by a Doppler transmitral flow pattern analysis and transesophageal ECG recording. AV interval lengthening up to 300 ms resulted in proper timing of atrial and ventricular contractions. Awaiting for conclusive data about biatrial pacing,

Ischemic stroke in a patient with interatrial block: Is it only a coincidence or a clear relationship?

Atrial fibrillation (AF) can be detected in nearly 25% of all patients with stroke by sequentially combining different electrocardiographic methods. Prediction of early cardio-embolic stroke remain a permanent challenge in everyday practice. The early identification of an increased risk for atrial fibrillation episodes (which are frequently asymptomatic) is essential for the prevention of cardioembolic events. One of the noninvasive modalities of atrial fibrillation prediction is represented by the electrocardiographic P-wave analysis. This includes study and diagnosis of interatrial conductio

Advanced interatrial block independently predicts appropriate and inappropriate ICD therapies in hypertrophic cardiomyopathy.

BACKGROUND: Implantable cardioverter-defibrillators (ICDs) reduce the risk of sudden cardiac death in hypertrophic cardiomyopathy (HCM); however, both appropriate and inappropriate shocks carry important clinical implications. Advanced interatrial block (IAB) is an electrocardiographic marker of atrial conduction disease, but its prognostic impact on ICD shocks in HCM remains unclear. METHODS: We retrospectively analyzed consecutive patients with HCM who underwent ICD implantation between 2014 and 2024. Advanced IAB was defined as a P-wave duration of ≥ 120 ms with biphasic morphology in the

Bachmann Bundle Pacing Combined with ICD in Hypertrophic Cardiomyopathy with Interatrial Conduction Block.

This case presents a 67-year-old man with familial hypertrophic cardiomyopathy, severe heart failure (LVEF 31%, NYHA III), and significant interatrial conduction delay. After ruling out cardiac resynchronization therapy due to unsuitable ventricular activation characteristics, a dual-chamber implantable cardioverter-defibrillator was implanted, using Bachmann bundle pacing for the atrial lead to optimize interatrial synchrony. BBP successfully corrected the IACD, evidenced by P-wave shortening and normalization. This led to remarkable clinical improvements within 1 month: LVEF increased from 3

15-Year ECG Evolution of Interatrial Block and Effects of Atrial Septal Pacing.

BACKGROUND: Interatrial block (IAB) reflects atrial remodeling and an arrhythmogenic substrate; however, its long-term progression is poorly characterized. CASE SUMMARY: An 87-year-old woman with recurrent atrial tachyarrhythmias and sinus node dysfunction demonstrated progressive P-wave prolongation from 120 to 160 ms over a 15-year period, evolving from partial to atypical advanced IAB with a triphasic morphology. Intracardiac mapping revealed extensive biatrial scarring and markedly delayed interatrial conduction. High right atrial septal pacing shortened the P-wave duration to 126 ms and

Advanced interatrial block predicts ineffective cardioversion of atrial fibrillation: a FinCV2 cohort study.

AIMS: Rhythm control using electrical cardioversion (CV) is a common treatment strategy for patients with symptomatic atrial fibrillation (AF). To guide clinical decision making, we sought to assess if electrocardiographic interatrial blocks could predict CV failure or AF recurrence as the phenomenon is strongly associated with atrial arrhythmias. METHODS: This study included 715 patients who underwent a CV for persistent AF lasting >48 h. P-wave duration and morphology were analyzed in post-procedure or the most recent sinus rhythm electrocardiograms and compared with rates of CV failure and

Interatrial block predicts atrial high rate episodes detected by cardiac implantable electronic devices.

BACKGROUND: Interatrial block which is defined as P wave duration longer than 120 milliseconds is underappreciated but highly prevalent electrocardiographic abnormality and associated with atrial tachyarrhythmias and ischemic stroke. Impaired left atrial mechanics and atrial fibrillation are potential mechanisms contributing to the embolic events associated with interatrial block. Detection of atrial high rate episodes by cardiac implantable electronic devices provides clinicians to diagnose asymptomatic atrial fibrillation. The relation between interatrial block and asymptomatic atrial fibril

BACKGROUND: Interatrial block (IAB) has been associated with atrial fibrillation (AF) and stroke in broader cardiovascular populations. Whether these associations extend to hypertrophic cardiomyopathy (HCM) and how IAB relates to underlying left atrial (LA) remodeling remain unknown. OBJECTIVE: This study aimed to investigate the prevalence, morphofunctional correlates, and prognostic value of IAB in patients with HCM. Interatrial block can occur in the absence of left atrial enlargement: New experimental model.

We present the surface electrocardiogram of an open-chest anesthetized healthy adult swine after direct application of ice at the transversus sinus of the pericardium where the Bachmann's region is located. Gradual and transient interatrial block (IAB) in the absence of structural atrial disease is described. This new experimental model demonstrated that IAB is an independent entity from left atrial enlargement.
