Interosseous Membrane Reconstructions for Longitudinal Radioulnar Instability: A Cadaveric Comparison.

BACKGROUND: Multiple options exist for reconstruction of the interosseous membrane (IOM) including biologic and nonbiologic materials. In this study, we present the biomechanical properties of reconstruction of the IOM using suture button suspension with and without pronator teres (PT) tendon rerouting. METHODS: Using 8 cadaveric specimens, radioulnar displacement was tested for the following successive conditions: intact specimen, IOM release with radial head (RH) resection, IOM reconstruction with PT tendon transfer alone, and PT transfer augmented with a suture button suspension construct

Reconstructing the interosseous membrane: a technique using synthetic graft and endobuttons.

Reconstruction of the interosseous membrane is an emerging procedure designed to help restore anatomic and biomechanical relationships within the forearm after a longitudinal instability injury. The indication for this reconstruction is proven acute or chronic longitudinal instability of the forearm. This technique uses a synthetic braided graft tied over endobuttons at radial and ulnar tunnels. It can also be combined with other procedures such as radial head replacement, wafer procedures, and even ulnar shortening osteotomies for comprehensive management of the condition. Early results indic

The role of the tibiofibular interosseous membrane in the repair of fractures of the tibia and fibula.

Anatomical studies were carried out to explore the relationship between the tibiofibular interosseous membrane and the long bones of the leg. Based on the data obtained from experimental fractures it appears that the interosseous membrane, which forms the so-called osteoperiosteal pocket, plays an important role in the repair of leg fractures. Further studies are being undertaken to validate this hypothesis and arrive at a more accurate interpretation of the phenomena observed.

The distal interosseous membrane: current concepts in wrist anatomy and biomechanics.

The distal interosseous membrane (DIOM) of the forearm acts as a secondary stabilizer of the distal radioulnar joint (DRUJ) when the dorsal and palmar radioulnar ligaments of the triangular fibrocartilage complex are cut. Recent anatomical studies revealed that thickness of the DIOM varies widely among specimens and the distal oblique bundle (DOB) exists within the DIOM in 40% of specimens. The DOB originates from the distal one-sixth of the ulnar shaft and runs distally to insert on the inferior rim of the sigmoid notch of the radius. The mean thickness of the DIOM without a DOB was 0.4 mm, w

A biomechanical study on the interosseous membrane and radial head in cadaveric forearms.

OBJECTIVE: Our aim was to evaluate the biomechanical properties of the interosseous membrane (IOM) and radial head and investigate the pathomechanics of the Essex-Lopresti injury. METHODS: Twelve adult fresh frozen upper limbs of human cadavers were chosen for test. First, the 12 intact specimens were mounted onto a materials testing machine to carry out biomechanical tests in pronation, supination and neutral positions, respectively. An axial load of 100 N was applied along the longitudinal axis of the forearm for 30 seconds in each position. Then, the twelve specimens were randomly divided

Reconstruction of the interosseous membrane of the forearm with a graft substitute: a cadaveric study.

PURPOSE: Longitudinal radioulnar dissociation occurs when traumatic axial loading through the wrist disrupts the interosseous membrane (IOM) of the forearm and fractures the radial head (Essex-Lopresti injury). Proximal migration of the radius results in a wrist with a positive ulnar variance, which leads ultimately to painful ulnar-sided wrist degeneration and wrist pain during grasping activities that involve axial loading or ulnar deviation of the wrist. In theory reconstruction of the IOM with a graft substitute can limit proximal migration of the radius, thereby preserving wrist function.

The interosseous membrane of the forearm: structure and its role in Galeazzi fractures.

A cadaveric study was performed to anatomically describe and mechanically document the interosseous membrane of the forearm using gross, histologic, scanning electron microscopic, and mechanical testing. The membrane was found to be a complex structure composed of nerves and vessels but mainly of collagen fibers that thicken to form bands coursing from radius to ulna. Strain-gauge studies demonstrated that the load transfer occurs from the radius to the ulna via the membrane and changes with supination or pronation. With specimens under a specific test load, sectioning of the membrane allowed

Role of the interosseous membrane in post-traumatic forearm instability: instructional review.

PURPOSE: In the last two decades, a strong interest on the interosseous membrane (IOM) has developed. METHODS: The authors present a review of the new concepts regarding the understanding of forearm physiology and pathology, with current trends in the surgical management of these rare and debilitating injuries. RESULTS: Anatomical and biomechanical studies have clarified the anatomy of forearm constrains and their role in forearm longitudinal and transverse stability. The radial pull test, a new intraoperative test, has been developed that might increase the detection on IOM injuries. The fo

Role of the interosseous membrane and annular ligament in stabilizing the proximal radial head.

HYPOTHESIS: The purpose of our study was to determine the relative contributions of the annular ligament, proximal band, central band, and distal band of the interosseous membrane in preventing dislocation of the proximal radius. METHODS: In part 1 of the study, 8 forearms were loaded transversely with the forearm intact, and the central band, proximal band, and annular ligament were sequentially sectioned to determine the percentage contribution of each structure in preventing transverse radial displacement. In part 2, 12 forearms were cyclically supinated and pronated while optical sensors

The effects of the interosseous membrane and partial fibulectomy on loading of the tibia: a biomechanical study.

The biomechanical basis for the treatment of delayed union of tibial fractures by partial fibulectomy has yet to be fully evaluated. To gain further insight into this problem, nine intact cadaveric lower extremities were instrumented with strain gauges on the surfaces of the tibia and fibula. The limbs were then subjected to axial loading with the ankle and subtalar joints placed in multiple positions. The specimens were loaded either through the distal femur or by direct loading of the tibial plateau. All specimens were first tested intact then after sectioning of the interosseous membrane an

Mason type-I radial head fractures and interosseous membrane lesions--a prospective study.

BACKGROUND: The role of the forearm interosseous membrane (IOM) in trauma remains not fully understood. Information from experimental models simulating sudden axial loading of the IOM is limited. The results from several cadaver studies are controversially discussed in literature. Pronation and supination of the forearm as well as varus and valgus position of the elbow joint have been suspected to have influence on the injury pattern. The purpose of the current study, therefore, was to further investigate the role of the IOM in trauma mechanism. METHODS: We prospectively screened the forearms

Antegrade Peroneal Flap through the Interosseous Membrane for Knee and Distal Femur Coverage.

BACKGROUND: Antegrade peroneal flaps can be rotated around the fibula to cover defects in the lower leg and lateral knee. However, these flaps cannot reliably cover the distal femur and anterior and medial knee. In the present article, the authors describe a novel technical modification that involves creating a tunnel through the interosseous membrane, through which the flap can be passed, circumventing the need to rotate around the fibula, allowing it to reach the entire knee and distal femur. METHODS: An anatomical study was performed in five cadaveric specimens to measure the gain in pedic

The intra-operative radius joystick test to diagnose complete disruption of the interosseous membrane.

Disruption of the interosseous membrane is easily missed in patients with Essex-Lopresti syndrome. None of the imaging techniques available for diagnosing disruption of the interosseous membrane are completely dependable. We undertook an investigation to identify whether a simple intra-operative test could be used to diagnose disruption of the interosseous membrane during surgery for fracture of the radial head and to see if the test was reproducible. We studied 20 cadaveric forearms after excision of the radial head, ten with and ten without disruption of the interosseous membrane. On each fo

Ultrasound diagnosis and surgical pathology of the torn interosseous membrane in forearm fractures/dislocations.

Ultrasound was used to effectively diagnose and precisely locate a torn interosseous membrane in 2 forearm fracture/dislocations. The ultrasound appearance of the torn interosseous membrane in cadavers and in the fractures/dislocations shows disruption of the anterior and posterior layers in midsubstance or adjacent to the radius or ulna. The surgical pathology in 3 forearm fractures/dislocations (2 Galeazzi injuries and 1 Essex-Lopresti injury) shows a longitudinal oblique tear of the interosseous membrane, parallel to its major palmar fibers, oriented from proximal radial to the distal ulnar
