Traumatic intracranial aneurysm in infancy.

An anterior cerebral artery traumatic aneurysm in a 15-month-old infant is described. The diagnosis was confirmed by computed tomographic angiography. The patient recovered fully after successful clipping of the aneurysm. As the signs of traumatic intracranial aneurysm may be subtle especially in young children, a high degree of suspicion is essential in patients presenting with delayed neurological deterioration after head trauma.

Neoplastic aneurysm and intracranial hemorrhage.

A case of fatal intracerebral hemorrhage due to rupture of a neoplastic aneurysm in a 68-year-old man with a history of bronchogenic carcinoma is reported. Intracranial neoplastic aneurysms resulting from metastatic tumor embolization of the cerebral vessels are more often associated with cardiac myxoma and choriocarcinoma. Of the 23 reported cases, eight ruptured and caused intracerebral or subarachnoid hemorrhage. Like mycotic aneurysm, they are small and located in the peripheral branches of cerebral artery, and may easily be obscured by hemorrhage when ruptured.

Intracranial aneurysms in adolescents.

PURPOSE: Intracranial aneurysms are extremely uncommon in adolescents. This study was undertaken to assess the clinical and radiological characteristics and clarify the choice of therapeutic strategies of intracranial aneurysms in adolescents with age range from 15 to 18 years. METHODS: From our dedicated aneurysmal databank between October 1985 and July 2008, we reviewed 16 consecutive adolescents who had 20 intracranial aneurysms. RESULTS: Ten boys and six girls (male/female ratio = 1.67:1; mean age 16.78 ± 1.18 years) were included in the present study. Intracranial aneurysms in adolescen

Intracranial aneurysms in Korea.

We have 32 university hospitals and 18 general hospitals for neurosurgical training in Korea. Recently I have analyzed intracranial aneurysm cases which had an operation in 28 university hospitals and nine general hospitals. I have reported 2863 aneurysm operation cases. The location of aneurysm consisted of 865 aneurysms (30.2%) in the anterior communicating artery, 724 aneurysms (25.3%) in the internal carotid artery, 687 aneurysms (24.0%) in the middle cerebral artery, 129 aneurysms (4.5%) in the anterior cerebral artery, 152 aneurysms (5.3%) in the posterior circulation, and 306 (10.7%) mu

Pregnancy and intracranial aneurysms.

Aneurysmal subarachnoid hemorrhage during pregnancy is rare but is an important cause of maternal mortality. Physiologic changes in pregnancy may predispose to aneurysm formation and rupture. In general, neurosurgical considerations take precedence over obstetric considerations. Ruptured aneurysms should be treated as they would be in patients who are not pregnant. Unruptured aneurysms should be treated if they are symptomatic or enlarging. Other aneurysms should be treated on an individual basis.

Familial intracranial aneurysms.

The authors report seven individuals from two families, all of whom had aneurysmal subarachnoid hemorrhage. These cases and all reported cases of familial aneurysms (243 aneurysms in 177 patients from 74 families) were submitted to computer-aided multivariate analysis to determine if the aneurysms or the patients who harbor them differ from sporadic aneurysm cases. Familial aneurysms rupture at a smaller size (mean diameter 10.5 mm), and when the patient is younger (mean age 42.3 years and decennial age at peak incidence 40 to 49 years). There is a similar sex distribution (male to female rati

Familial intracranial aneurysms.

OBJECTIVE: Numerous studies have compared the characteristics of familial intracranial aneurysms with those of non-familial aneurysms. To better understand familial subarachnoid hemorrhage (SAH), we studied a series of patients with SAH who had at least one first-degree relative with SAH, and compared our results with those of previous studies. METHODS: We identified patients treated for SAH at our hospital between January 1993 and October 2006 and analyzed those patients with one or more first-degree relatives with SAH. We retrospectively collected data from patients with a family history an

Traumatic intracranial aneurysms.

The long-held hope that vaccination strategies might be effective against cancer has motivated numerous attempts over the past century to put the idea to test in the clinic. Although the generally disappointing results have cast a long shadow over the field, advances in cancer immunology growing on the remarkable insights from basic immunology provide a strong foundation and powerful new tools to guide current attempts to fashion effective cancer vaccines. This review covers the scientific basis and rationale for cancer vaccines, the challenges involved in assembling the many ingredients going

Inflammation and intracranial aneurysms.

OBJECTIVE: An intracranial aneurysm is an important acquired cerebrovascular disease that can cause a catastrophic subarachnoid hemorrhage. Despite modern therapy, most patients die or are left disabled as a direct result of a severe initial hemorrhage. The development of more effective treatment strategies depends on understanding the fundamental biology of cerebral aneurysms. The purpose of the present study is to determine whether inflammation or immunological reaction occurs in cerebral aneurysms. METHODS: Aneurysm tissue was collected at the time of microsurgical repair from 23 unrupture

Familial intracranial aneurysms.

We describe a 53-year-old mother and a her 34-year-old son who suffered from spontaneous subarachnoid hemorrhage (SAH) and/or intracerebral hemorrhage due to a ruptured intracranial aneurysm. Diagnosis of the intracranial aneurysm was made by CT scan of the brain, and cerebral angiography. These two cases received surgical intervention and returned to their previous social activity after discharge, even though the mother had arrived in a comatose condition. The literature is reviewed and the characteristic expressions of familial intracranial aneurysms are described. The etiology of cerebral a

Genetics of intracranial aneurysms.

The etiology and pathogenesis of intracranial aneurysms are clearly multifactorial, with genetic factors playing an increasingly recognized role. Intracranial aneurysms have been associated with numerous heritable connective tissue disorders, which account for at least 5% of cases. Of these disorders, the most important are Ehlers-Danlos syndrome Type IV, Marfan's syndrome, neurofibromatosis Type 1, and autosomal dominant polycystic kidney disease; the association with intracranial aneurysms, however, has been firmly established only for polycystic kidney disease. Familial intracranial aneurys

Intracranial aneurysms and heredity.

The occurrence of intracranial aneurysms (IAs) in the families of 579 consecutive patients with subarachnoid hemorrhage (SAH), of whom 485 had verified IAs, was studied retrospectively. IAs occurred in the families of 6.7% of the IA patients, but only 0.4% of their siblings had IAs. However, there were differences between the familial and nonfamilial IA patients, indicating that the familial patients are a specific small subpopulation of IA patients. The familial patients were younger, often had multiple aneurysms, and had aneurysms frequently located on arteries other than those in the nonfam

Dissecting intracranial aneurysms.

The clinical, operative and pathological characteristics of a dissecting aneurysm of the vertebral artery are presented. A review of the literature suggests that this type of intracranial aneurysm is being recognized with increasing frequency and can be characterized by its symptomatology and radiological patterns. A set of guidelines for the management of dissecting intracranial aneurysms is proposed.

Experimental intracranial aneurysms.

Methods and results of creation of an experimental intracranial aneurysm on the canine basilar artery using an autogenous vein graft are presented. Aneurysms were produced either on the wall of the basilar artery or at a lingual-basilar artery anastomosis site. Postoperative angiography revealed no success in the first group and 55% success in the second group, with better results occurring later in the series. Mortality was higher in the first group. Hemodynamic and technical factors are discussed and compared with earlier work on experimental aneurysms in the rat. Construction of an aneurysm

Site-Specific Relationship Between Intracranial Aneurysm and Aortic Aneurysm.

BACKGROUND AND PURPOSE: The high prevalence of intracranial aneurysms (IAs) in patients with a bicuspid aortic valve or coarctation of the aorta suggests a link between IA and aortic pathology. However, studies reporting this link do not sufficiently address the heterogeneity of IAs arising from different anatomic locations. This study aimed to explore whether a location-specific relationship exists between the 2 kinds of aneurysms. METHODS: Retrospective institutional analysis of patients aged ≥18 years with both IA and an aortic aneurysm (AA) was performed from 2005 to 2014. IAs were catego

Screening for intracranial aneurysms? Prevalence of unruptured intracranial aneurysms in Hong Kong Chinese.

OBJECT The objective of this study was to generate data on the local prevalence of unruptured intracranial aneurysms (UIAs) in asymptomatic Hong Kong Chinese individuals. First-degree relatives of patients with aneurysmal subarachnoid hemorrhage (aSAH) were recruited as surrogates of the general population and to explore the potential role of screening in this locality. METHODS The authors identified first-degree relatives of consecutive patients with subarachnoid hemorrhage from a ruptured aneurysm who were admitted to a university hospital in Hong Kong from June 2008 to December 2010.
