Cortical surface intracranial electrodes identify clinically relevant seizures missed on scalp EEG after traumatic intracranial hemorrhage.

Seizures and other electrophysiological disturbances are an under-recognized cause of coma, focal deficits, and prolonged encephalopathy following subdural hematoma evacuation. In these patients, it is possible that seizures remain unrecognized on scalp EEG. It has been shown that a high burden of seizures and other electrophysiological disturbances exist following surgical evacuation and underlie the encephalopathy commonly seen in this patient population, predisposing them to medical complications and confounding estimates of prognosis. As part of a research protocol, we are performing intra

Whole Blood-Based Resuscitation and Mortality in Patients with Traumatic Intracranial Hemorrhage.

BACKGROUND: Traumatic intracranial hemorrhage (tICH) is a major driver responsible for traumatic brain injury-related deaths. Studies have demonstrated an associated survival benefit with plasma administration among patients with tICH. The administration of whole blood (WB), from which plasma is derived, may provide similar or greater benefits. We hypothesize that WB, compared with plasma-based resuscitation, is associated with reduced 30-day mortality among patients presenting with tICH. STUDY DESIGN: This cohort study used the American College of Surgeons-TQIP databank from January 1, 2020,

Repeat Surgery After Decompressive Craniectomy for Traumatic Intracranial Hemorrhage: Outcomes and Predictors.

INTRODUCTION: Repeat surgery (RS) after decompressive craniectomy/craniotomy (DC) for traumatic intracranial hemorrhage (TICH) is a devastating complication. In patients undergoing DC for TICH, we sought to 1) describe the population requiring RS, 2) compare outcomes of those requiring RS with those who did not, and 3) discern RS predictors. METHODS: A single-institution retrospective case-control study was conducted from 2000 to 2015. Inclusion criteria were DC for acute supratentorial TICH (subdural hemorrhage, epidural hemorrhage, and intraparenchymal hemorrhage) and ≥7 day survival. Patie

A handheld near infrared scanner for the detection of acute traumatic intracranial hemorrhage.

The Archeoptix NIRD device is a handheld near infrared scanner for intracranial hemorrhage of at least 3 mL in volume located up to 3.5 cm from the scalp. It generates an image that approximates hemorrhage location on a schematic head. We report a prospectively collected cohort of scans from patients with traumatic brain injury and CT-confirmed intracranial hemorrhage, alongside healthy control subjects, to evaluate the utility and accuracy of this technology. Patients with hemorrhage were recruited from the neurosurgical service within 24 h of trauma for a single scan, and also controls with

[Visual evoked potentials in patients with a history of non-traumatic intracranial hemorrhage].

The authors present the results of preliminary investigations of visual evoked potentials in patients long after intracerebral haemorrhage. In 32 studied patients prolongation was observed of the latency of waves N1 and N2 as well as P1, and this prolongation was more pronounced in the case of stimulation of the eye contralateral to the hemisphere with the lesion. This asymmetry of latency occurred with similar irregularity in case with all locations of the lacuna, with the exception of the occipital lobe. In the latter case a much greater prolongation of Ni and Pi latency on the affected side

Retinal hemorrhages in children with traumatic intracranial pathology in a tertiary care center.

BACKGROUND: Retinal hemorrhages (RH) are commonly seen with abusive head trauma (AHT). This study evaluates pediatric patients who had neuroimaging and an ophthalmologic examination in an attempt to correlate the etiology of presentation with the presence and pattern of RH and intracranial pathology. METHODS: A retrospective review was performed on hospitalized patients under 5 years of age who underwent neuroimaging and ocular examination. The presence of intracranial pathology, RH, and diagnoses of AHT versus accidental trauma were recorded. Fundus photographs were evaluated with a publishe

Clinical Evaluation of a Microwave-Based Device for Detection of Traumatic Intracranial Hemorrhage.

Traumatic brain injury (TBI) is the leading cause of death and disability among young persons. A key to improve outcome for patients with TBI is to reduce the time from injury to definitive care by achieving high triage accuracy. Microwave technology (MWT) allows for a portable device to be used in the pre-hospital setting for detection of intracranial hematomas at the scene of injury, thereby enhancing early triage and allowing for more adequate early care. MWT has previously been evaluated for medical applications including the ability to differentiate between hemorrhagic and ischemic stroke

Predictors of Mortality in Traumatic Intracranial Hemorrhage: A National Trauma Data Bank Study.

Background/Objective: Traumatic intracranial hemorrhage (tICH) accounts for significant trauma morbidity and mortality. Several studies have developed prognostic models for tICH outcomes, but previous models face limitations, including poor generalizability and limited accuracy. The objective was to develop a prognostic model and determine predictors of mortality using the largest trauma database in the U.S., applying rigorous analytical methodology with true hold-out-set model validation. Methods: We identified 248,536 patients in the National Trauma Data Bank (NTDB) from 2012 to 2016 with a

Traumatic nondisplaced coronal suture fracture causing delayed intracranial hemorrhage in a pediatric patient.

Skull fracture after a head injury is relatively common in children younger than 2 years of age. The author reports the case of a 14-month-old girl who sustained a unilateral nondisplaced coronal suture fracture from a fall. She developed delayed intracranial hemorrhage from an underlying dural tear and cortical vein injury. Although an isolated skull fracture in a pediatric trauma patient typically portends a benign clinical course and may not require that the patient be hospitalized, a nondisplaced fracture across the coronal suture can lead to dural tear and intracranial injuries. High vigi

When a Pulmonary Embolism Precedes the Fall: Anticoagulation Challenges in Traumatic Intracranial Hemorrhage.

Pulmonary embolism (PE) is typically managed with urgent systemic anticoagulation or reperfusion therapy. However, management becomes complex when PE occurs concurrently with traumatic intracranial hemorrhage (ICH), where anticoagulation may worsen bleeding. A 74-year-old male patient presented after syncope and traumatic head injury. He was hypoxic and tachycardic on arrival with concern for right ventricular strain. Given persistent clinical concern, CT angiography of the chest demonstrated a massive saddle PE. CT of the head revealed traumatic subarachnoid hemorrhage and a small subdural he

Polygenic Vulnerability to Intracranial Hypertension, Hemorrhage Progression, and Outcome in Traumatic Brain Injury.

OBJECTIVE: Growing evidence underscores the importance of host-response/secondary-injury-likely influenced by genetics-in outcome variability post-traumatic brain injury (TBI). Intracranial hypertension and hemorrhage progression are critical secondary injuries in severe TBI; these are mediated by the SUR1-TRPM4 channel (a target in clinical trials). We aimed to deconstruct the complex network surrounding SUR1-TRPM4 and define the cumulative impact of key genetic variants on mechanistically connected secondary injuries/outcomes after severe TBI. METHODS: This exploratory study analyzed 492 pr

Clinical outcomes among patients with concurrent blunt cerebrovascular injury and traumatic intracranial hemorrhage.

BACKGROUND: Blunt cerebrovascular injury (BCVI) accounts for 1-3 % of patients with blunt trauma, which should be promptly diagnosed and managed due to risk of cerebral infarction and death. Antithrombotic therapy had been proven to reduce risk of stroke and mortality. However, due to concern of hematoma progression, treatment suggestion is still inconclusive for patients with concurrent traumatic intracranial hemorrhage. MATERIALS AND METHODS: We performed a retrospective, observational study from 2002 to 2020 at a Level I trauma center, all patients with BCVI and concurrent traumatic intrac

A Prehospital Triage System to Detect Traumatic Intracranial Hemorrhage Using Machine Learning Algorithms.

IMPORTANCE: An adequate system for triaging patients with head trauma in prehospital settings and choosing optimal medical institutions is essential for improving the prognosis of these patients. To our knowledge, there has been no established way to stratify these patients based on their head trauma severity that can be used by ambulance crews at an injury site. OBJECTIVES: To develop a prehospital triage system to stratify patients with head trauma according to trauma severity by using several machine learning techniques and to evaluate the predictive accuracy of these techniques. Early cognitive impairment is common after intracranial hemorrhage with mild traumatic brain injury.

BACKDROP: The incidence of early cognitive impairment (ECI) after traumatic brain injury (TBI) is unknown. We hypothesized ECI is common and can be predicted based on Glasgow Coma Scale (GCS) and Brain Injury Guideline (BIG) category. METHODS: A single-center, retrospective review of adult trauma patients (2014-2016) with intracranial hemorrhage (ICH) and mild TBI (GCS score, 13-15) was performed. The primary outcome was ECI, defined as a Rancho Los Amigos score less than 8. Routine cognitive evaluation is performed on all ICH patients at our institution. Comparisons between ECI and no-ECI gr

Traumatic laceration of the intracranial vertebral artery causing fatal subarachnoid hemorrhage: case report.

A 36-year-old man who had been drinking alcohol had a fatal subarachnoid hemorrhage immediately after suffering a moderate craniofacial injury. Autopsy revealed a 3-mm longitudinal laceration of the left intracranial vertebral artery proximal to the posterior inferior cerebellar artery. There was no finding of arterial dissection. We discuss the mechanisms of the traumatic laceration of the vertebral artery in relation to traumatic dissection of the vertebral artery.

Hospital admission of older patients with mild traumatic brain injury and traumatic intracranial hemorrhage: is it always necessary?

BACKGROUND AND IMPORTANCE: Traumatic intracranial hemorrhage (tICH) after mild traumatic brain injury (mTBI) is not uncommon in the elderly. Often, these patients are admitted to the hospital for observation. The necessity of admission in the absence of clinically important intracranial injuries is however unclear. OBJECTIVES: The objective of this study is to identify which factors additional to tICH affect the risk of this outcome and to evaluate the differences in the risk of adverse outcome in younger and older mTBI patients with tICH. DESIGN, SETTING, AND PARTICIPANTS: This retrospectiv

Cardiac surgery for a right atrial myxoma with traumatic intracranial hemorrhage: a case report.

BACKGROUND: The timing of cardiac surgery with cardiopulmonary bypass (CPB) for intracranial hemorrhage is controversial. CASE PRESENTATION: We report the case of an 82-year-old woman who was transferred to our hospital because of a head injury. Brain computed tomography (CT) revealed traumatic intracranial hemorrhage, and transthoracic echocardiography revealed a giant right atrial myxoma. After confirming the disappearance of intracranial hemorrhage on brain CT, cardiac surgery with CPB was performed, which was uneventful. CONCLUSIONS: For an uneventful surgery, the optimal timing of cardi
