Long-term treatment of involuntary facial spasms using botulinum toxin.

Botulinum toxin, a powerful pre-synaptic neurotoxin produced by Clostridium botulinum, interferes with the release of acetylcholine from nerve terminals. Since September 1985, we have been using this toxin to treat altogether 62 patients with benign facial spasms. Most of the patients had been on drugs or psychotherapy, 2 had received alcohol injections, 2 had undergone surgery of the orbicular branch, and 2 electrocoagulation of the facial nerve. In essential blepharospasm the duration of the beneficial effect after each treatment with botulinum toxin was about 3 1/2 months. In patients with

Anorexia nervosa and the law: Legal mechanisms for involuntary treatment in Ireland.

Anorexia nervosa is a form of eating disorder associated with significant morbidity and mortality, such that patients can become physically unwell and need medical treatment. Body image can be distorted, meaning that underweight people may believe they need to lose weight, leading to treatment refusal in some cases. Consequently, involuntary treatment is sometimes used in severe cases of anorexia, which may include nasogastric (tube) feeding to restore weight. Wardship is used in Ireland to obtain the court's consent for treatment of unwilling patients with anorexia nervosa, as it is legally u

Involuntary Psychotropic Treatment in the Correctional System: Revisiting the Legal Standards.

Correctional facilities are obligated to provide psychotropic medications, as part of standard psychiatric care, to inmates with serious mental illness. The right to refuse such medication treatment has become one of the most important and contested areas of legal regulation of correctional mental health. This article will focus on the three cases that have come before the U.S. Supreme Court thus far, as well as their implications for future medicolegal directions in pursuing involuntary treatment.

A Danish register-based study on involuntary treatment in anorexia nervosa.

OBJECTIVE: Involuntary treatment is controversial and widely debated, but remains a significant component of treatment for severe anorexia nervosa. Given how little is known about this topic, we describe the frequency of various involuntary measures in a national cohort of all patients diagnosed with anorexia nervosa. In a subsample of patients, we explored predictors of the first involuntary measure recorded. METHOD: Descriptive statistics and Cox proportional hazard analyses were conducted using the national registers of Denmark covering the total population. Data from the National Patient

Treatment of Involuntary Movements with Carbamazepine in Dystonic-Choreoathetoid Cerebral Palsy.

Choreoathetoid movements are quite common in cerebral palsy (CP). This is the first report of a patient with choreoathetoid CP who was successfully treated with carbamazepine. Therefore, clinicians should try carbamazepine for involuntary movements in CP patients before pursuing other procudures.

[Involuntary movements as a complication of treatment of extrapyramidal disorders with L-dopa].

The frequency of dyskinesias connected with L-DOPA treatment of certain extrapyramidal system diseases was studied. Among 111 studied patients with Parkinson's disease drug-induced dyskinesias were observed in 56 cases (that is 50% of the treated patients) while in the group of other extrapyramidal system diseases (torsion dystonia, Hallervorden-Spatz disease, Huntington's chorea with increased muscular tonus) they were observed in only 8 cases (of 73 treated ones - about 11%). Two groups of patients with Parkinson's disease were isolated - differing in the susceptibility to dyskinesia develop

The benefits and harms of inpatient involuntary psychiatric treatment: a scoping review.

This scoping review examined quantitative research comparing involuntary inpatient groups with voluntary inpatients or other comparator group. Ten themes were identified: patient knowledge of legal status, experienced or perceived coercion, effects on medication use, clinical effects measured on outcome scales, effects on psychiatric readmission, use of restraints and seclusion, effects on suicide and deaths, patient satisfaction, length of stay and carer experiences. The review found that involuntary inpatient admission was associated with harms including increased subjective and objective co

Involuntary inferior and multidirectional instability of the shoulder: etiology, recognition, and treatment.

Multidirectional and inferior instability of the shoulder is not rare. Etiological factors include various combinations of (a) repetitive injuries, (b) inherent joint laxity, and (c) one or more major injuries. It is seen in athletic and active patients without generalized joint laxity and as well in sedentary patients with hypermobile joints. Standard operations for unidirectional anterior or posterior dislocations fail to correct multidirectional instability because they do not correct inferior instability and they may displace the head in fixed subluxation to the opposite side leading to se

Involuntary discharge from drug or alcohol treatment programs in Vancouver, Canada.

BACKGROUND: Retention in substance use treatment is essential to treatment success. While programmatic factors are known to influence retention, less is known about the role of involuntary discharges from drug or alcohol treatment programs. Therefore, we sought to identify the prevalence of and factors associated with involuntary discharge due to ongoing substance use. METHODS: Data were derived from two community-recruited prospective cohort studies of people who use drugs in Vancouver, Canada. Generalized estimating equation (GEE) analyses were used to identify variables associated with inv

Longitudinal investigation of patients receiving involuntary treatment for extremely severe anorexia nervosa.

INTRODUCTION: Involuntary treatment may be a life-saving option for extremely severe anorexia nervosa (AN) in the context of life-threatening conditions and refusal of care. The long-term outcomes of patients undergoing involuntary treatment for AN are poorly understood. This study aims to explore quality of life, long-term outcomes and attitudes towards involuntary treatment in patients involuntarily treated for extremely severe AN. METHODS: 23 patients involuntarily admitted for extremely severe AN (I-AN), and 25 voluntarily admitted patients (V-AN) were compared for body mass index (BMI),

Involuntary treatment in patients with anorexia nervosa: utilization patterns and associated factors.

BACKGROUND: A subgroup of patients with anorexia nervosa (AN) undergoing involuntary treatment (IT) seems to account for most of the IT events. Little is known about these patients and their treatment including the temporal distribution of IT events and factors associated with subsequent utilization of IT. Hence, this study explores (1) utilization patterns of IT events, and (2) factors associated with subsequent utilization of IT in patients with AN. METHODS: In this nationwide Danish register-based retrospective exploratory cohort study patients were identified from their first (index) hosp

Involuntary treatment of anorexia nervosa in Ireland: Challenges and changes in the legal framework.

Anorexia Nervosa is an eating disorder typified by low body weight, restrictive eating behaviours, and body image distortion. It is associated with significant risk of medical complications, with one of the highest mortality rates of any mental illness. While the majority of patients receive treatment on a voluntary basis, a small proportion of severely ill patients refuse treatment and are treated involuntarily. The legal mechanisms used for involuntary treatment vary between jurisdictions, including mental health law, capacity-based law, guardianship, and use of inherent jurisdiction, the po

A Bitter Pill: The Ethics of Involuntary Treatment of Adolescents With Severe Eating Disorders.

Severe eating disorders (EDs) are a common and increasing threat to normal adolescent health and development. Major clinical challenges include longstanding malnutrition potentially complicated by emergent electrolyte disorders and cardiac dysfunction. The care of adolescents with severe EDs can lead to challenging decisions regarding the initiation of involuntary feeding with restraints. After describing a representative case, we present pertinent ethical challenges involved in the use of involuntary feeding with restraints in adolescents with severe EDs. These include the following: (1) the

The therapeutic relationship in the context of involuntary treatment orders: The perspective of nurses and patients.

WHAT IS KNOWN ON THE SUBJECT?: Involuntary treatment orders are increasingly being used around the world to allow the treatment of individuals living with a mental illness deemed incapable of giving consent and who are actively refusing treatment. The use of involuntary treatment orders can impact the nurse-patient therapeutic relationship, which is essential to offer quality care and promote recovery. WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE?: Nurses and patients do not agree on the possibility to develop a therapeutic relationship, with nurses believing they can build a bond with the patien

Involuntary treatment in dementia care at home: Results from the Netherlands and Belgium.

AIMS AND OBJECTIVES: To gain insight into the request, use and associated factors of involuntary treatment in people with dementia (PwD) receiving professional home care in the Netherlands and Belgium. BACKGROUND: Most of the PwD remain living at home as long as possible. Due to complex care needs, this can result in an increased risk for care provided against the wishes of the client and/or to which the client resists, referred to as involuntary treatment. DESIGN: Secondary data analyses of two cross-sectional surveys. METHODS: Dementia case managers and district nurses filled in a questio

[Clinical criteria of involuntary psychiatric treatment: a literature review and a synthesis of recommendations].

Though the carrying out of involuntary psychiatric care (IPC) mainly follows from a subjective appraisal of the patient's state, some specific clinical criteria have been proposed on regards to the underlying disorder. French national recommendations are synthesized hereby, and completed by a literature review. In psychotic disorders, the level of insight and the impact of delusion(s) are the essential criteria that should recommend to carry out IPC. In mood disorders, the appraisal is different depending on the underlying clinical state. In depressive or mixed states, IPC should be considered
