Nebulized ipratropium bromide in ventilator-assisted patients with chronic bronchitis.

Whether nebulized ipratropium bromide is of benefit to mechanically ventilated patients with chronic bronchitis is not well defined. The objective of the study was to determine the effect of ipratropium bromide as a nebulized solution on ventilatory function in patients with severe airflow limitation and under ventilator treatment because of respiratory failure. The design was a randomized, double-blind, controlled trial. Forty-two ventilated patients (43 to 83 years old) with acute airflow obstruction and wheezing or coughing were chosen. The patients were randomly allocated to treatment ever

Control of the hypersecretion of vasomotor rhinitis by topical ipratropium bromide.

A randomized, double-blind, placebo-controlled trial of ipratropium bromide nasal spray was performed with 25 patients with vasomotor rhinitis. Criteria for selection of patients were (1) clear watery nasal discharge more than 1 hour each day, (2) absent or mild nasal obstruction, (3) no known allergic cause, and (4) no satisfactory response to previous alternative medications. Ipratropium bromide, two sprays (20 micrograms per spray) in each nasal cavity four times daily, for 3 weeks, produced a major reduction in nasal discharge severity and duration (p less than 0.00005 for daytime reductio

Ipratropium bromide in chronic bronchitis/emphysema. A review of the literature.

Many studies have evaluated the efficacy of ipratropium bromide in chronic obstructive pulmonary disease (COPD). Single-agent studies have shown ipratropium to be both safe and effective in COPD. Several studies have compared ipratropium with sympathomimetic agents or methylxanthines in patients with chronic bronchitis or emphysema; all of these studies have shown at least an equal, and in most instances a superior, bronchodilator action with ipratropium in terms of duration of action as well as peak bronchodilator effect in patients with COPD. In some patients with COPD, beta agonists, theoph

Ipratropium (Atrovent) in the treatment of vasomotor rhinitis of elderly patients.

Ipratropium is a parasympatholytic agent with high topical activity. The effect of intranasal ipratropium was studied in 34 patients who were over 60 years of age and who suffered from chronic watery vasomotor rhinorrhoea. The study was a double-blind cross-over placebo controlled trial. Trial parameters were: daily nasal symptom and side effect scores, nasal methacholine testing before and after treatment, and clinical evaluation. The active drug clearly reduced nasal discharge. Nasal methacholine testing showed reduction of nasal secretion after the active treatment period. The majority of t

[Benefits of ipratropium bromide in the management of asthmatic crises in the emergency department].

BACKGROUND: To determine if the addition of ipratropium bromide in the emergency department (ED) for the treatment of childhood asthma reduces rates of hospitalization and relapses for moderate and severe exacerbations. METHODS: Patients were given an oral corticosteroid treatment (2 mg/kg) and received every 20 minutes either three nebulizations with albuterol (0.15 mg/kg) and ipratropium bromide (250 micrograms) or six nebulizations with albuterol alone (control group). The primary end point was the need for hospitalization, additional nebulizations or a relapse during the following week. S

[Experiences in therapy of bradycardic cardiac arrhythmias in childhood with ipratropium bromide].

Ipratropiumbromid is used in adults for a variety of bradyarrhythmias. In contrast this therapy has not yet been established in childhood, where symptomatic bradyeardias are less frequent. Since Ipratropiumbromid has less side effects (especially concerning the central nervous system) and a longer half time than atropin, we investigated the efficacy of Ipratropiumbromid in 12 children (mean age 7.2 yrs.) in the course of 2 1/2 years. We found a significant increase of the minimal and mean heart rate in Holter-Eeg-recordings. Only in 7 of 12 patients we obtained a sufficient therapeutic effect.

Olfactory stimuli provoke diffuse esophageal spasm: reversal by ipratropium bromide.

Diffuse esophageal spasm (DES) is a motor disorder of the esophageal smooth muscle characterized by multiple spontaneous contractions and by swallow-induced contractions that are of simultaneous onset, large amplitude, long duration, and repetitive occurrence. Although the pathogenesis of DES is unknown, provocative studies with cholinergic stimulation, esophageal balloon distention, or acid instillation have suggested involvement of both sensory and motor mechanisms. This report describes a patient with DES who would predictably become symptomatic with dysphagia and chest pain upon inhalation

Long-term safety and efficacy study of intranasal ipratropium bromide.

Forty patients presenting with watery rhinorrhoea as their main complaint were entered in an open trial of long-term intranasal ipratropium bromide. Fourteen of these (35 per cent) found it to be beneficial and completed one year of treatment. Side-effects were seen in 28 patients (70 per cent) but these were local and mild in nature. All resolved during the trial as patients adjusted the dosage to individual severity and frequency of symptoms. No significant systemic effects were noted during the course of therapy. Ipratropium is a useful drug in the treatment of watery rhinorrhoea and appear

Oxitropium bromide, ipratropium bromide and fenoterol in exercise-induced asthma.

Oxitropium bromide (Ba 253) is a new inhaled bronchodilating agent with anticholinergic properties. The effect of this compound (100 micrograms) was compared to that of ipratropium bromide (40 micrograms), the beta 2-receptor stimulant fenoterol (400 micrograms) and placebo in 8 patients with exercise-induced asthma. The drugs were administered by metered dose inhalers, after which exercise test were performed on ergometer cycles at the times of the drugs' maximal effects. Forced expiratory volume during 1 S and vital capacity were recorded basally, repeatedly during 20 min after exercise and

The influence of ipratropium bromide in the recovery phase of methacholine induced-bronchospasm.

BACKGROUND: The protective effect of Ipratropium Bromide (IB) in the methacholine-induced bronchospasm is well known from some time. The objective of the present study was to assess whether a pretreatment with IB may influence the subsequent phase of methacholine-induced bronchospasm relief. METHODS: Sixteen patients with bronchial hyper-reactivity (PD20 FEV1 < 200 microg) were randomly assigned to three methacholine challenge tests at a 48 to 72 hours interval apart. In the first test IB was inhaled before the challenge (pre-IB), in the second IB was administered soon after the PD20 FEV1 (po

[The effect of ipratropium bromide on lung function in patients with cystic fibrosis].

The main manifestation of cystic fibrosis (CF) of the lungs is an obstructive ventilation disturbance. The fact that the administration of atropine improves pulmonary function in patients with CF suggests a vagal mechanism for the development of bronchal obstruction. In a single-blind, placebo-controlled study, we investigated the effect of 250 micrograms of an inhalation solution of the anticholinergic substance ipatropium bromide (IB) on the pulmonary function of 11 patients with CF (age range: 8 to 29 years). The mean figures for the changes in FEV1.0, FEF25-75%, RV and TLC after inhalation

Effect of ipratropium bromide on respiratory mechanics in infants with acute bronchiolitis.

The effect of nebulized ipratropium bromide in 14 infants (mean age: 20 weeks, range: 4-41) with acute respiratory syncytial virus bronchiolitis was examined. A modified rapid chest compression technique was used to obtain partial expiratory flow-volume curves and maximum flow at functional residual capacity. Passive respiratory mechanics were assessed by brief occlusion at end inspiration. Thoracic gas volume was measured in a body plethysmograph. No significant difference was found in forced and passive respiratory mechanics pre- and post-ipratropium bromide. No subgroups could be identified

The effect of ipratropium bromide (Atrovent) on the air conditioning capacity of the nose.

The air conditioning capacity of the nose was assessed by a method which permits measurements in both nasal cavities without blockage of the passage between the nasopharynx and oropharynx. By this method the degree of warming of the air and its humidification during the passage through the nasal cavity can be calculated. The effect of intranasal application of ipratropium bromide (Atrovent) on the air conditioning capacity of the nasal mucosa was evaluated in a group of normal subjects and a group of patients with vasomotor rhinitis. In this short term trial Atrovent was found not to have any

Bronchial responsiveness to hyperventilation in children with asthma: inhibition by ipratropium bromide.

Isocapnic hyperventilation dose response curves were constructed for 11 asthmatic children before and after pretreatment with placebo or ipratropium bromide, 40-1500 micrograms given by inhalation, on three separate days. The response before and after placebo was highly reproducible (within subject coefficient of variation 7.5%, 18%, and 22% for intervals of two hours, within two weeks, and over two weeks). It was independent of baseline lung function. Complete protection against hyperventilation induced asthma was achieved by ipratropium bromide 40 micrograms in six children and by 200 microg

Double-blind study of intranasal ipratropium bromide in nonallergic perennial rhinitis.

We undertook this trial to determine whether ipratropium bromide nasal spray 0.03% (IB) reduced the nasal hypersecretion associated with nonallergic perennial rhinitis (NAPR) without causing excessive dryness or irritation of the nasal mucosa. We compared two drug doses of IB (21 micrograms and 42 micrograms per nostril) to a placebo, administered as two sprays to each nostril twice daily. The study design consisted of a 1-week screening period without treatment, a 1-week single-blind placebo period, a 4-week double-blind treatment comparison period, and a 1-week follow-up period without medic

Effects of intranasal xylometazoline, alone or in combination with ipratropium, in patients with common cold.

BACKGROUND: Common cold is one of the most prevalent conditions that family doctors encounter. One of the first symptoms to occur is nasal congestion, which can have a negative impact on daily life and prompts many patients to seek treatment for relief. Xylometazoline nasal spray (Otrivin*) is a topical decongestant that has been used successfully for many years and is generally recognized as an effective and safe therapy. However, most studies have investigated its clinical efficacy in healthy patients and few have included patients with common cold. SCOPE: To review the published clinical e

Effect of ipratropium bromide on the interfacial organization of model lung surfactant membranes.

The interactions of inhaled drugs with lung surfactant are important for understanding their behavior at the first biological barrier of the respiratory tract. In this work, we investigated the effect of the anticholinergic bronchodilator, ipratropium bromide (IPB), on model lung surfactant membranes composed of dipalmitoylphosphatidylcholine (DPPC), dipalmitoylphosphatidylglycerol (DPPG), and their 8:2 molar mixture. Langmuir monolayer studies showed that ipratropium bromide has little influence on zwitterionic DPPC, while it interacts strongly with negatively charged DPPG, leading to concent
