MI remains a highly lethal entity. Improving survival requires an aggressive, multidisciplinary approach. High-risk patients with severe abdominal pain and a paucity of physical findings should be undergo emergent imaging in a search for this disease. Improvements in laboratory tests and advances in imaging techniques may improve the ability to diagnose MI earlier in its course, before irreversible damage has occurred. Many treatment modalities are available and should be tailored to each individual case. By recognizing and preventing ischemia-reperfusion injury, the cycle of protracted complic

CONTEXT: As rejection in renal transplantation has become better controlled, gastrointestinal complications have become increasingly important. Ischemic colitis and colonic perforation are the most common of these lesions, contributing to morbidity and mortality in the early postoperative period.
OBJECTIVE: We undertook this study to identify factors contributing to the risk of intestinal ischemia in patients undergoing renal transplantation and to define circumstances that may affect that risk.
METHODS: We studied 356 patients undergoing renal transplantation during a 40-month period. We re

Colon ischemia is expressed in a broad clinical spectrum, from mild, reversible ischemia to intestinal infarction and gangrene. In most cases, the precipitating cause is unknown, and colonic blood flow usually has normalized by the time the patient seeks medical attention. Satisfactory treatment begins with accurate diagnosis, which depends on serial colonoscopic or roentgenographic studies and the exclusion of other disorders that may mimic colon ischemia. Prognosis typically is good, and most patients require only supportive care with close follow-up. More aggressive therapy, including surge

Papilloarterial ischemia (PI) is the designation for a clinical picture characterized by circulatory insufficiency of the optic disk and the retinal arteries. Optic disk ischemia leads to visual loss, altitudinal field defects, pale papilledema and fluorescein angiographic filling defects of the disk capillaries. Retinal ischemia manifests itself by ophthalmoscopic and angiographic signs of caliber variations, focal constrictions or even occlusions of peripapillary retinal arteries. PI occurs more frequently in young females, in whom an inflammatory origin must be suspected in view of the angi

The posttraumatic syndrome of Volkmann's ischemia and infarction is the end result of compromise of muscle perfusion within the osteofascial compartments of the forearm. The "closed space" arrangement as well as the particular neurovascular anatomy make the flexor forearm muscles particularly susceptible to the cyclic derangement of blood flow, which ultimately results in muscle ischemia and infarction. Only strong clinical awareness will allow appropriate early diagnosis and treatment by surgical decompression. Fasciotomy and epimysiotomy serve to interrupt the cycle. The successful treatment

Hand ischemia is an uncommon, but potentially catastrophic, clinical entity. The large number of etiologic factors and lack of universal agreement regarding treatment have created a confusing clinical situation. Diagnosis is most frequently made by a detailed history and physical examination. Both invasive and noninvasive tests are employed for diagnosis and in planning treatment. Growing experience and improved surgical techniques have enabled vascular surgeons to deal more effectively with the ischemic hand.

The reports of 8 patients with acute or subacute abdominal pain related to venous mesenteric ischemia were reviewed. None of the patients presented local or regional predisposing factors for venous thrombosis. In 4 patients, a localized segment of ischemic small bowel (median length 125 cm; range: 30-350) was resected without immediate anastomosis and postoperative anticoagulation therapy was given. Two of these patients developed recurrent ischemia involving the bowel adjacent to the stoma, treated successfully in 1 case by a repeat resection. The 4 other patients hospitalized with intestinal

BACKGROUND: Inhibiting SDH (succinate dehydrogenase), with the competitive inhibitor malonate, has shown promise in ameliorating ischemia/reperfusion injury. However, key for translation to the clinic is understanding the mechanism of malonate entry into cells to enable inhibition of SDH, its mitochondrial target, as malonate itself poorly permeates cellular membranes. The possibility of malonate selectively entering the at-risk heart tissue on reperfusion, however, remains unexplored.
METHODS: C57BL/6J mice, C2C12 and H9c2 myoblasts, and HeLa cells were used to elucidate the mechanism of sel

The prognosis of 55 patients with silent ischemia (group I: asymptomatic ST segment depression of greater than or equal to 0.1 mV on symptom-limited ergometer exercise) was compared with that of 25 patients with angina and ST depression (group II), 22 patients with angina but without ST depression (group III) and 94 patients without angina and without ST depression (group IV) on ergometer testing in the first post-infarction month. Patients for whom PTCA or coronary artery bypass graft surgery was planned for the next months following discharge were excluded. Groups were well matched in terms

Ischemic heart diseases are the leading cause of death with increasing numbers of patients worldwide. Despite advances in revascularization techniques, angiogenic therapies remain highly attractive. Physiological ischemia training, which is first proposed in our laboratory, refers to reversible ischemia training of normal skeletal muscles by using a tourniquet or isometric contraction to cause physiologic ischemia for about 4 weeks for the sake of triggering molecular and cellular mechanisms to promote angiogenesis and formation of collateral vessels and protect remote ischemia areas. To investigate the effects of ischemia/reperfusion on rat submandibular glands without denervation and the possible protective effects of ischemia preconditioning on the glands that experienced ischemia/reperfusion, in-situ ischemia/reperfusion and ischemia preconditioning experimental models of submandibular glands of healthy male Wistar rats were conducted. For ischemia/reperfusion groups, the glands were subjected to 90 min of ischemia without denervation, followed by 1, 12, 24, or 72 h of reperfusion. Ischemia preconditioning was achieved by 3 min of ischemia following 3 min of reperfusion

Acute mesenteric ischemia (AMI) presents variably based on etiology-arterial embolism (often from atrial fibrillation), arterial thrombosis (due to atherosclerosis), mesenteric venous thrombosis (linked to hypercoagulability), or nonocclusive ischemia (from low-flow states in critically ill patients). Diagnosis is confirmed with biphasic multidetector CT angiography. Early recognition is essential, as delayed diagnosis worsens outcomes. Treatment varies from anticoagulation to surgery or endovascular intervention. Resuscitation, rapid diagnosis, and revascularization ("3 R"s) are key. AMI shou

We introduced a case of a 69-year-old female patient with mitral valve prolapse with severe regurgitation who was transferred from the ICU to a regular ward after undergoing mitral valve replacement surgery. Routine ECG examination showed that the precordial leads were affected by the apical pulsation, leading to pseudo-ST segment depression and prolonged QT interval. In clinical practice, ECG artifacts caused by apical pulsation, if not carefully distinguished, may lead to unnecessary examinations and treatments for patients. Our case emphasizes the importance of accurately identifying ECG ar

Acute mesenteric ischemia is a surgical emergency commonly caused by embolic or thrombotic occlusion of the superior mesenteric artery. Prompt diagnosis, fluid resuscitation, systemic anticoagulation, and mesenteric revascularization are key tenants to the treatment of this lethal condition. Revascularization can be performed via open thromboembolectomy or surgical bypass, endovascular techniques, or a hybrid approach of the 2. Despite technological advancements, mortalities remain high, and the plan of care and revascularization should be based on the patient's clinical status and available m

Despite major advances in the contemporary management of peripheral arterial occlusive disease, acute ischemia of the lower limb is still characterized by an important morbidity, limb threat, and mortality, and continues to pose a challenge to the vascular surgeon. Ageing of the population increases the prevalence of acute lower limb ischemia. The two principal etiologies of acute ischemia of the lower limbs are arterial embolism and in situ thrombosis of an atherosclerotic artery or of a bypass graft. Popliteal aneurysm thrombosis and vascular trauma are less common causes of severe limb isch

BACKGROUND: Acute vascular occlusion within the mesenteric circulation leads to ischemic damage of the corresponding bowel segment, which starts on the mucosal level and progresses transmurally.
OBJECTIVES: Report on pathogenesis, clinical picture and treatment of various forms of intestinal ischemia.
MATERIALS AND METHODS: Analysis of the available literature taking into consideration our own experience.
RESULTS: Frequently, predisposing diseases and risk factors are present (e.g., cardiac diseases, hypercoagulability, status post cardiac surgery, circulatory failure, or administration of

Acute mesenteric ischemia is a life-threatening condition that occurs when an abrupt decrease in blood flow results in inadequate supply to the intestines, leading to bowel infarction. Etiologies may include arterial embolus, arterial or venous thrombosis, and nonocclusive mesenteric ischemia. Other causes include vasculitis, traumatic injury, dissection of the aorta, cholesterol emboli, and intestinal obstruction. Mortality rates can approach 90% in the setting of bowel infarction. As such, it is important to establish an early diagnosis before the onset of bowel infarction. Treatment has tra
