Balanced electrolyte solutions versus isotonic saline in adult patients with diabetic ketoacidosis: A systematic review and meta-analysis.

BACKGROUND: Current guidelines suggest the use of isotonic saline (IS) infusion as the preferred resuscitation fluid in the management of diabetic ketoacidosis (DKA). However, balanced electrolyte solutions (BES) have been proposed as an alternative due to a lower propensity to cause hyperchloremic metabolic acidosis. Evidence regarding the use of BES in DKA remains limited. OBJECTIVES: To determine if the use of BES in fluid resuscitation leads to faster resolution of DKA compared to IS. METHODS: The study involves a comprehensive search of literature from PubMed, Cochrane CENTRAL, Google S

A current conflict: use of isotonic sodium chloride solution on endotracheal suctioning in critically ill patients.

The use of isotonic sodium chloride on endotracheal suctioning is still commonly performed in intensive care units (ICUs). According to the studies, isotonic sodium chloride instillation may decrease oxygen saturation, increased intracranial pressure, arterial blood pressure, and cause cardiac dysrhythmias, cardiac arrest, respiratory arrest, and nosocomial infection. Endotracheal suctioning should not be used as a routine or standard clinical practice because of these negative effects. This article reviews effects of isotonic sodium chloride solution before endotracheal suctioning of mechanic

Differential effects of isotonic and hypotonic 4% albumin solution on intracranial pressure and renal perfusion and function.

OBJECTIVES: Albumin is used to resuscitate trauma patients but may increase intracranial pressure (ICP). Its effects on renal blood flow and function are unknown. Our aim was to examine the effects of hypertonic albumin on ICP and renal function, and if any effects are due to the hypotonicity of the solution containing albumin or to albumin itself. DESIGN, SETTING AND SUBJECTS: Cross-over, randomised controlled experimental study of six adult Merino ewes in the animal facility of a research institute. METHOD: Sheep were implanted with flow probes around the pulmonary and renal arteries and a

Isotonic saline solution as maintenance intravenous fluid therapy to prevent acquired hyponatremia in hospitalized children.

OBJECTIVES: This review aims to evaluate if the use of hypotonic saline solutions as maintenance intravenous fluid therapy in hospitalized children increases the risk of hyponatremia, if the administration of isotonic fluids is able to protect against acquired hyponatremia and if the isotonic solutions increase the risks of deleterious effects such as hypernatremia or fluid overload. SOURCES: We researched the relevant literature on the PubMed (Jan 01 1969 to Jul 13 2011), EMBASE (1989 to 2011) and Cochrane Library (1989 to 2011) databases. Furthermore, references of selected studies were inc

Balanced electrolyte solution vs isotonic saline in the resuscitation of children with diabetic ketoacidosis: A randomized controlled trial.

BACKGROUND: Fluid therapy is a cornerstone in the management of diabetic ketoacidosis (DKA). Normal saline (NS, 0.9%) has traditionally been the fluid of choice; however, concerns about its association with hyperchloremic metabolic acidosis and possible development of acute kidney injury (AKI) have led to an increased interest in balanced electrolyte solutions (BES) as a potentially safer alternative. AIM: To compare BES with NS in the initial resuscitation of children with DKA. METHODS: This double-blind, parallel, randomized controlled trial (RCT) was conducted in the Pediatrics department

Stabilization of Graphene Oxide Dispersion in Plasma-like Isotonic Solution Containing Aggregating Concentrations of Bivalent Cations.

UNLABELLED: Graphene oxide's (GO) intravascular applications and biocompatibility are not fully explored yet, although it has been proposed as an anticancer drug transporter, antibacterial factor or component of wearable devices. Bivalent cations and the number of particles' atom layers, as well as their structural oxygen content and pH of the dispersion, all affect the GO size, shape, dispersibility and biological effects. Bovine serum albumin (BSA), an important blood plasma protein, is expected to improve GO dispersion stability in physiological concentrations of the precipitating calcium a

Hemodynamic effects of hypertonic hydroxyethyl starch 6% solution and isotonic hydroxyethyl starch 6% solution after declamping during abdominal aortic aneurysm repair.

Fluid resuscitation with hypertonic hydroxyethyl starch solutions (HES) is effective in haemorrhagic shock due to the rapid mobilisation of fluids into the intravascular compartment. Declamping of the abdominal aorta with acute redistribution of blood into the vessels of the lower body half causes declamping-induced hypotension. Usually large amount of fluids or vasopressors are necessary to restore hemodynamic stability. Therefore, infusion of a hypertonic colloid solution may be an attractive option to achieve hemodynamic stability. This study was conducted to determine the amount of fluid o

Effects of intravenous rehydration protocols based on an acetate-containing electrolyte solution, isotonic saline, and an isotonic mixture of saline/sodium bicarbonate on acid-base status and plasma electrolyte concentrations in calves with diarrhoea.

Strong ion (metabolic) acidosis, dehydration, and electrolyte imbalances are common in neonatal calves with diarrhoea. Balanced crystalloids may provide advantages for rehydration purposes in those animals due to their polyionic composition and higher buffer capacity compared to conventional buffer-free fluids such as 0.9 % NaCl. This randomized clinical trial aimed to compare the effects of three infusion solutions - an acetate-containing balanced electrolyte solution (Sterofundin® ISO; Stero), 0.9 % saline (NaCl), and an isotonic saline-sodium bicarbonate mixture (NaClBic) - on acid-base sta
