[Total esophagoplasty using a double pedicled free graft from the jejunum: the last resort].

After repeated failures of classic oesophagoplasties (gastroplasty and coloplasty) a last chance reconstruction could rely on a free jejunal transplant. We report an exceptional and original case where digestive tract was re-established between the orostoma and the epigastric antrum. The patient has four neoplasms in the throat, thoracic oesophagus. After circular pharyngolarynectomy and total oesophagectomy, all the usable vascularized plasties had necrosed. The ultimate reconstruction was performed with a free jejunal transplant carrying two vascular bundles revascularized with the cervical

Testicular Metastasis of Jejunal Neuroendocrine Tumor on 68Ga-DOTATATE PET/CT.

Neuroendocrine tumors are slow-growing tumors originating from neuroendocrine cells and capable of metastasis. Most of them are found in the gastrointestinal tract; however, they can also be rarely seen in other organs. Testicular neuroendocrine tumors account for less than 1% of all testicular neoplasms. They may present as primary testicular or secondary tumors from extratesticular sources. Jejunal neuroendocrine tumor metastasis to the testis is extremely rare. We present the case of a 61-year-old man with a jejunal neuroendocrine tumor and metastases to bilateral testicles revealed on Gall

Small intestinal metastasis from intrahepatic cholangiocarcinoma: report of a case.

Metastasis of the small intestine that derives from a primary hepatic neoplasm is rare. We encountered a case of intrahepatic cholangiocarcinoma (ICC) with jejunal metastasis after resection of a primary lesion. A 61-year-old male patient was referred to us with a diagnosis of liver tumors. Partial hepatectomy was performed, and the pathological diagnosis was ICC. Seventeen months after surgery, the patient was found to have a mass in the jejunum and lymph node swelling by positron emission tomography-computed tomography. The jejunal tumor was preoperatively diagnosed as a metastasis of ICC fr

An unusual case of pleuropulmonary blastoma in a child with jejunal hamartomas.

We report a rare case of 9-month-old girl who presented with a choking episode and was found to have an incidental finding of a lung cyst and iron deficiency anemia leading to the diagnosis of pleuropulmonary blastoma (PPB) and a jejunal hamartoma. Our patient is the eighth that has been reported with the association of PPB with jejunal hamartoma and the first one in the radiological literature. PPB is the pulmonary analog of other dysontogenetic neoplasms in childhood. A biological sequence has been described with the three types of PPB to be interrelated as part of pathologic progression. Massive bleeding of a jejunal gastrointestinal stromal tumour: a rare case of a life-threatening presentation.

A jejunal gastrointestinal stromal tumour (GIST) is a rare neoplasm of the gastrointestinal tract. Massive bleeding due to a jejunal GIST is a diagnostic challenge and could present as a life-threatening situation needing urgent intervention. A 54-year-old woman presented with episodes of melaena and haematochezia for the previous 3 days. An oesophagoduodenoscopy was inconclusive. A contrast computed tomography (CT) scan showed a well-defined extraluminal mass in the proximal jejunum suggestive of a bleeding GIST. Her haemodynamic state deteriorated despite initial supportive therapy including

Unusual case report of bifocal gastrointestinal stromal tumor in the first jejunal loop uncovered by severe hemorrhage.

INTRODUCTION AND IMPORTANCE: Gastrointestinal stromal tumors (GISTs) are rare mesenchymal tumors, constituting 1 % of gastrointestinal tumors. Jejunal GISTs are even rarer. This article describes a unique bifocal jejunal GIST case, revealed by an exceptionally rare instance of massive hemorrhage. CASE PRESENTATION: We present a 60-year-old female admitted for two days of melena. Despite being hemodynamically stable, severe anemia was noted. Initial endoscopy was normal, but further imaging and jejunoscopy revealed two jejunal GISTs. Surgical resection confirmed the diagnosis. Postoperative re

Multifocal jejunal histiocytic sarcoma presenting with dual intussusception.

Histiocytic sarcoma (HS) of the small intestine is exceedingly rare. We report a woman in her early 50s who presented with acute abdominal pain and was found to have two synchronous jejunal intussusceptions caused by multifocal HS. Emergency laparoscopy converted to open resection revealed multiple jejunal tumours; pathology confirmed HS with mesenteric nodal metastasis. Although 13 tumours were identified, only two lesions served as lead points for intussusception. All tumorous lesions were resected. The patient was recommended adjuvant therapy but declined and died 12 months later. This case

Ampullary somatostatinomas and jejunal gastrointestinal stromal tumor in a patient with Von Recklinghausen's disease.

Von Recklinghausen's disease is an autosomal dominant hereditary disease associated with a wide number of neoplasms. We report a case of a 47-year-old Caucasian male affected by Von Recklinghausen's disease who developed a malignant somatostatinoma of the papilla major and minor associated with jejunal gastrointestinal stromal tumour with uncertain behaviour. At laparotomy, multiple hepatic metastases were evident. Whipple pancreaticoduodenectomy, jejunal resection, extensive lymphadenectomy and multiple hepatic wedge resections were performed. The patient was alive without recurrence after 24

Massive Gastrointestinal Bleeding from Choriocarcinoma of the Ovary.

Choriocarcinoma of the ovary is a rare neoplasm and tends to metastasize early to involve the lungs, brain, and liver. Metastasis to the gastrointestinal tract is rare. We report the case of a young lady, who presented with life-threatening gastrointestinal bleeding, secondary to jejunal involvement. The site of bleeding was localized using a computed tomography angiogram following inconclusive upper and lower gastrointestinal endoscopies. The bleeding jejunal segment was resected and the metastatic disease responded to combination chemotherapy. Metastatic choriocarcinoma of the ovary should b

Diagnosis and Management of Gastrointestinal Neuroendocrine Neoplasms.

The latest WHO classification cleanly divides gastrointestinal neuroendocrine neoplasms into neuroendocrine tumor (NET; well-differentiated, any grade) and neuroendocrine carcinoma (NEC; poorly differentiated, high-grade by definition), along with mixed neuroendocrine-non-neuroendocrine neoplasms. NECs are always aggressive, with multiple mutations; they are treated with chemotherapy. NETs have widely different presentations, behavior, and management depending on site of origin. Esophageal examples are vanishingly rare. Most gastric and appendiceal tumors are indolent, as are many colonic and

Bleeding jejunal leiomyoma: a new approach.

Leiomyomas represent one of the most common benign neoplasms of the small intestine. Most of these are asymptomatic and are found incidentally at surgery or autopsy; however, leiomyomas may present with prominent signs and symptoms. Bleeding is the most common clinical problem and is occasionally life-threatening. We report the case of a 90-yr-old man who presented with rectal bleeding that became massive within hours, necessitating blood transfusion. An arteriogram revealed significant hemorrhage from the jejunal branch of the superior mesenteric artery (thought to be arteriovenous malformati

Synchronous resection of esophageal cancer and other organ malignancies: A systematic review.

BACKGROUND: Neoplasms arising in the esophagus may coexist with other solid organ or gastrointestinal tract neoplasms in 6% to 15% of patients. Resection of both tumors synchronously or in a staged procedure provides the best chances for long-term survival. Synchronous resection of both esophageal and second primary malignancy may be feasible in a subset of patients; however, literature on this topic remains rather scarce. AIM: To analyze the operative techniques employed in esophageal resections combined with gastric, pancreatic, lung, colorectal, kidney and liver resections and define posto

[A case of primary jejunal adenocarcinoma diagnosed by enteroscopy using pediatric colonoscope].

A 39-year-old man presented with dizziness and melena for 2 months. Abdominal CT scan showed constrictive wall thickening with enhancement and proximal loop dilatation of the jejunum. On endoscopic examination, there was large amount of bile stained fluid in duodenum. Enteroscopy using pediatric colonoscope demonstrated an encircling mass with obstruction approximately 20 cm distal to the ligament of Treitz. Endoscopic jejunal biopsy showed moderately differentiated adenocarcinoma. Small intestinal adenocarcinoma is uncommonly encountered in clinical practice. Because small intestine is relati

Colonoscopy Leads to A Diagnosis of A Jejunal Gastrointestinal Stromal Tumour (GIST).

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors in the gastrointestinal (GI) tract, but are the least common of small intestinal malignant neoplasms. While GI bleeding is the most common clinical presentation of GISTs, intussusception and obstruction are uncommon, as GISTs rarely grow into the lumen. We describe an unusual case of a 50-year-old male who presented with intermittent obscure, overt GI bleeding requiring multiple hospital admissions and blood transfusions. His work-up included abdominal CT imaging, small bowel follow-through, gastroscopies, push ente

An unusual lead point: jejunal leiomyosarcoma causing jejuno-jejunal intussusception - a case report.

INTRODUCTION: Adult intussusception is rare. Unlike in pediatric cases, adult intussusception typically arises from an underlying pathology. Primary jejunal leiomyosarcoma is an exceptionally rare malignant tumor originating from the smooth muscle cells of the small intestine. CASE PRESENTATION: A 52-year-old man presented with signs and symptoms of acute intestinal obstruction, including abdominal pain, constipation, vomiting, and abdominal distension of 2 days duration. Contrast-enhanced computed tomography revealed a jejunojejunal intussusception with a lead-point intraluminal mass. Explor

Small Bowel Adenocarcinoma: a Nationwide Population-Based Study.

PURPOSE: Small intestinal cancers have a non-specific clinical presentation and hence a delayed diagnosis. The prevalence of small intestinal cancers is low, and there are no cost-effective methods of screening. This study aimed to identify clinical characteristics of duodenal and jejunal adenocarcinomas that can assist in the early detection and diagnosis of disease. METHODS: Duodenal adenocarcinoma and jejunal adenocarcinoma in Explorys database (1999-2019) were compared using odds ratio (OR) with 95% confidence intervals. Data on demographic characteristics, risk factors, clinical features

Hepatic metastases of primary jejunal carcinoid tumor: A case report with radiological findings.

CONTEXT: Carcinoid tumors represent a group of well-differentiated tumors originating from the diffuse endocrine system outside the pancreas and thyroid. The overall prevalence of carcinoid tumors in the United States is estimated to be one to two cases per 100,000 persons. Various sites of origin of this neoplasm are appendix - 30-45%, small bowel - 25-35% (duodenum 2%, jejunum 7%, ileum 91%, multiple sites 15-35%), rectum 10-15%, caecum - 5%, and stomach - 0.5%. Liver metastases from jejunal and ileal carcinoids are generally hypervascular. CASE REPORT: Here we report a case of primary jeju

An unexpected extra-gastrointestinal stromal tumor (E-GIST) on the jejunal mesentery.

Extra-gastrointestinal stromal tumors (E-GIST) are group of soft tissue neoplasm, which originates from outside the gastrointestinal tract and comprises less than 5% of the total GIST. Most commonly, they originate from the omentum, mesentery, retroperitoneum or the solid organs like liver and the pancreas. E-GIST arising from the sac of the jejunal mesentery, and mimicking preoperatively as a colonic neoplasm is very rare. Here we report an interesting case of a 55-year-old male, who presented with pain abdomen for 4 months duration. Contrast enhanced computed tomography was suggestive of rig

Anatomical Variants of the Jejunal Veins and Their Technical Implications in Pancreaticoduodenectomy: A Systematic Review and Meta-Analysis.

INTRODUCTION: One of the most common causes of bleeding during pancreaticoduodenectomy is a dissection of the pancreatic head from the superior mesenteric vein and superior mesenteric artery. Knowledge of the anatomical variants of the veins draining the proximal jejunum may allow better control of bleeding during detachment of the uncinate process and pancreatic head from the mesenteric pedicle and division of the mesopancreas. The aim of this systematic review and meta-analysis was to evaluate the anatomical variations of the first jejunal vein (FJV) and jejunal trunk (FJT). METHODS: Fourte
