Bilateral asymmetrical traumatic sternoclavicular joint dislocations.

Unilateral and bilateral sternoclavicular joint (SCJ) dislocations are rare injuries. The difficulty in assessing this condition often leads to delay in diagnosis and treatment. We report a rare case of bilateral asymmetrical traumatic SCJ dislocations in a 45-year-old male. The right anterior SCJ dislocation was reduced in the emergency room (ER) and resulted in residual instability. The left posterior SCJ dislocation was asymptomatic and unnoticed for six months. It is important for ER physicians and orthopaedic surgeons to be able identify and treat this condition. All suspected SCJ disloca

Complex dislocations of the metacarpophalangeal joint.

A dorsal longitudinal incision provides excellent exposure for reducing complex dislocations of the metacarpophalangeal joints of the thumb and index finger. Within the past four years, we have successfully treated five patients with irreducible dislocations of the metacarpophalangeal joint of the thumb and four with dislocations of the index finger by open reduction through a dorsal incision. This approach exposes the joint and the major structure blocking reduction, the volar plate. The digital nerves, which are tented volarly over the metacarpal head, are not prone to injury and osteochondr

Surgical management of chronic temporomandibular joint dislocations.

INTRODUCTION: Temporomandibular joint dislocation is described as the movement of mandibular condyle out of the fossa beyond its anatomical and functional boundaries causing pain and discomfort. It is often managed by conservative methods, but in long-standing, chronic conditions, surgical treatment is the only option. The goal of surgical treatment is to reposition the condyle and prevent further recurrences. MATERIALS AND METHODS: This retrospective analysis involving a single center and a surgeon with 19 patients and 23 joint surgeries performed over a 10-year period. Patients who fulfille

Closed reduction of multiple MCP joint dislocations.

We report a young man in his 20s who fell from the roof of his house and sustained injury to his left hand. Physical examination and radiographic evaluation revealed closed dislocations of the second, third, and fourth metacarpophalangeal (MCP) joint with fractures of the bases of the third and fourth metacarpals and the base of the proximal phalanx of the middle finger. The injury was managed with closed reduction and splintage with an uneventful outcome. Multiple MCP joint dislocations are rare, and there are only a few reports of closed reduction with a good outcome.

Metacarpophalangeal Joint Dislocations Two Cases, Two Stories.

INTRODUCTION: Metacarpophalangeal (MCP) joint dislocation and its pathoanatomy was first discussed in detail in an article written by Kaplan et al . ,in 1957. They had identified certain features of complex, that is irreducible MCP joint dislocation through closed method to differentiate from a simple MCP joint dislocation, that is MCP joint dislocation that can be reduced by closed method. The complex MCP joint dislocations (mostly involving the index finger) are called Kaplan dislocations. Here, we describe two cases of MCP joint dislocations to highlight the differences between complex and s

Management of Proximal Interphalangeal Joint Fracture Dislocations.

Fracture dislocations of the proximal interphalangeal (PIP) joint of the finger are often caused by axial load applied to a flexed joint. The most common injury pattern is a dorsal fracture dislocation with a volar lip fracture of the middle phalanx. Damage to the soft-tissue stabilizers of the PIP joint contributes to the deformity seen with these fracture patterns. Unfortunately, these injuries are commonly written off and left untreated. A late-presenting PIP joint fracture dislocation has a poor chance of regaining normal range of motion. The provider must be suspicious of these injuries.

Acute dislocations of the distal radioulnar joint.

An injury to the DRUJ can occur in association with almost any fracture of the forearm or as an isolated phenomenon. A dislocation of this joint may be simple or complex. Constant awareness must be maintained during treatment of injuries of the wrist, forearm, and elbow if common pitfalls are to be avoided. Failure to diagnose and treat a complex DRUJ dislocation will lead to chronic, persistent subluxations or dislocations, or both, and to symptomatic osteoarthrosis. Failure to recognize a simple dislocation of the DRUJ associated with a fracture of the forearm may result in inappropriate or

[Concomitant injuries after upper ankle joint dislocations].

Functional treatment with the Air Stirrup Ankle Brace recommended by C. N. Stover in 1979 can reduce pathological inversion of the ankle joint. In our retrospective study of 109 patients treated by this kind of ankle brace we found 96 patients (88%) with excellent results. Only 13 patients (12%) reported moderate to good results. To detect and characterize their painful conditions of ankles we did a clinical, radiological and MRI-Investigation. In only 2 cases we found a moderate instability after clinical investigation, anterior stress roentgenogram and talar tilt. By using the MRI-investigat

[Modern ideas on the treatment of dislocations and fracture-dislocations in the elbow joint].

The authors describe the causes and surgical methods of treatment of fresh and long-standing dislocations and fracture-dislocations in the elbow joint. The indications to using different operative methods and technical-surgical interventions are described in details as well as long-term results of treatment by the literature data. The current methods and the authors' methods of prophylactics and treatment of postoperative complications are analyzed. The methods of rehabilitation of such patients are also described.

Current Concepts: Lesser Digit Carpometacarpal Joint Fracture-Dislocations.

Carpometacarpal fractures and dislocations are commonly unrecognized injuries. Treating physicians need to maintain a high index of suspicion regarding this particular injury as delay in diagnosis and treatment can lead to a suboptimal outcome. Mechanism of injury often entails high-energy trauma or an axial blow to a closed fist. Thorough physical examination and adequate imaging are necessary during initial evaluation. Closed management and immobilization can be considered if the injury is recognized early; however, surgical reduction and stabilization are more commonly warranted. This artic

Outcomes of open dorsal proximal interphalangeal joint dislocations.

We report the outcomes of 25 open dorsal proximal interphalangeal joint dislocations treated without palmar plate reinsertion. Stiffness was frequent, but no residual dorsal instability was observed. These findings suggest that routine palmar plate reinsertion may not be necessary.

Complications following dislocations of the proximal interphalangeal joint.

Dorsal fracture-dislocations of the proximal interphalangeal joint are challenging injuries to treat and are associated with many complications. The determination of stability is crucial to appropriate management. Stable injuries can usually be treated nonsurgically, whereas unstable injuries typically require surgical stabilization. Many surgical techniques have been used, including extension block pinning, volar plate arthroplasty, open reduction and internal fixation, external fixation, and hemihamate autografting. Because stiffness and flexion contracture are frequent complications, every

Management of acute acromioclavicular joint dislocations: current concepts.

Acromioclavicular joint (ACJ) injuries represent a common injury to the shoulder girdle. In the management algorithm of acute ACJ injuries complete radiological evaluation represents the key to a successful therapy. According to the classification of Rockwood the presence of a horizontal component in addition to vertical instability has to be detected. Using axillary functional views or Alexander views dynamic horizontal ACJ instability can be diagnosed in a simple, efficient and cost-effective manner reducing the number of mis-/underdiagnosed ACJ injuries. MRI should not be the imaging modali

The Ilizarov technique in joint contractures and dislocations.

This study was designed to evaluate the results of treatment of joint contractures (6 patients) and dislocations (2 cases) with an Ilizarov device. Six patients with restricting joint contractures were successfully treated and after gradual scar expansion they achieved a significant improvement in their range of motion. One patient with elbow instability after closed dislocation achieved 5 degrees to 130 degrees of motion, while one patient with an open dislocation achieved a range of motion from 15 degrees to 100 degrees. These results suggest that a hinged Ilizarov external fixation frame ca
