The distal radioulnar joint is a complex structure necessary for forearm motion and force transmission across the wrist. Anatomic and biomechanical advances have revealed broad contributions to distal radioulnar joint stability and refined our understanding of the forces acting across it. Instability often co-occurs with other modes of pathology, such as arthrosis or malunion; and appropriate diagnosis and treatment require a comprehensive understanding of all contributing factors. Distal radioulnar joint instability can be broadly categorized as primary, post-traumatic or post-surgical. Treat

Injury to the proximal tibiofibular joint is typically seen in athletes whose sports require violent twisting motions of the flexed knee. Instability of this joint may be in the anterolateral, posteromedial, or superior directions. With acute injury, patients usually complain of pain and a prominence in the lateral aspect of the knee. A closed reduction should be attempted in patients with acute dislocation. If this is unsuccessful, open reduction and stabilization of the joint with repair of the injured capsule and ligaments can be done. Patients with chronic dislocation or subluxation report

Acute lateral ankle ligament ruptures are successfully treated nonoperatively with physiotherapy, peroneal strengthening, and coordination training. About 10-20% of patients may develop functional instability despite adequate nonoperative treatment. Chronic functional instability is not always a severe disability, but reconstruction of the lateral ankle ligaments may be necessary for patients with high demands on ankle stability. More than 50 different surgical procedures for the treatment of chronic lateral ankle joint instability have been described. Most of these are tenodeses where one of

Acute instability of the distal radioulnar joint (DRUJ) is often underestimated but can cause devastating loss of forearm and wrist form and function. These traumatic injuries can be caused by disruption of the triangular fibrocartilage complex, the interosseous membrane, or fractures of the radius or ulna, and a comprehensive understanding of the anatomy involved is paramount to the adequate treatment of these injuries. While stable DRUJ instability can be treated with immobilization, often surgical intervention is required and is based on the anatomical restraints to the DRUJ that are disrup

Distal radioulnar joint (DRUJ) instabilities are common and often combined with other injuries of the interosseous membrane and/or the proximal radioulnar joint. Once they are diagnosed and the treatment is chosen, physiotherapists have limited choices due to the lack of validated protocols. The benefits of proprioception and neuromuscular rehabilitation have been brought to light for the shoulder, knee and ankle joints, among others. However, no program has been described for the DRUJ. The purpose of this article is to study the muscular elements responsible for active DRUJ stability, and to

The purpose of this paper is to assess the results obtained with the arthroscopy-assisted surgical technique for the treatment of chronic acromioclavicular joint instability (CACJI), based on non-rigid coracoclavicular (CC) fixation and anatomical CC reconstruction with a tendinous allograft. Patients with CACJI who underwent surgery between 2008 and 2012 were included in the study. Clinical assessments included SF36, VAS and DASH, applied at the preoperative visit (POV) and at the last follow-up visit (LFUV). The Constant score and the General Satisfaction Score (0-10) were applied at the las

Closed injuries affecting the metacarpophalangeal and interphalangeal joints and their stabilizers in the thumb and fingers occur very commonly in athletes, possibly leading to finger joint instability and long-standing or permanent disability. This article reviews the spectrum of joint injuries of the thumb and fingers that are common in the athletic population with a main focus on the ultrasound features of collateral ligament tears, palmar plate injuries, and thumb sesamoid fractures. A thorough understanding of the complex anatomy, mechanism of injury, soft tissue abnormalities, and imagin

Metatarsophalangeal (MTP) instability is a common cause of metatarsalgia of the lesser toes. Instability often presents as pain in the plantar forefoot, progressing to coronal and transverse plane malalignment. MTP joint dislocation can develop in some cases. The plantar plate plays an integral role in MTP joint stabilization, and injury to the plantar plate is the main contributor to the instability noted at the MTP joint.

Disorders of the patellofemoral joint are commonly seen in musculoskeletal clinics. In recent years, the expansion of imaging techniques, improvements in correlative injury anatomy, and more focused physical examinations have resulted in new knowledge about patellofemoral disorders. To achieve optimal patient outcomes, it is helpful for orthopaedic surgeons who treat knee problems to review the management of patellar dislocations and isolated patellofemoral arthritis, including treatment algorithms.

We describe an alternative method for treating chronic trapeziometacarpal (TM) joint instability after acute injury or chronic repetitive use of the thumb by performing a dorsoradial capsulodesis procedure. The procedure is done by imbricating the redundant TM joint dorsoradial ligament and capsule after reducing the joint by pronating the thumb. The dorsoradial capsulodesis is a reasonable reconstructive option for chronic TM joint instability and subluxation.

There has been extensive research and investigation into the subtalar joint (STJ), yet the biomechanics that relate to its anatomic function, especially instability after injury, are surrounded by controversy. With a mechanism of injury closely related to the classic inversion ankle sprain, chronic instability can result following trauma to the lateral ligamentous support network of either joint. Over the past decades there have been countless examples in the literature challenging the current standard of evaluation and treatment of the "subtalar sprain." New technologies have offered varied a

Instability of the ulnar side of carpus centers around the triquetrum, which is suspended by the ulnar triquetral ligaments and supported proximally by the TFCC. The triquetrum guides the lunate by an interosseous membrane and stout palmar ligaments that provide a relatively rigid connection between the two bones. Disruption of the LT ligament is frequently associated with pathology in the ulnar carpal area and may progress to triquetral instability, VISI, and finally, degenerative arthritic changes on the ulnar side of the carpus. The diagnosis of LT injuries is made by stress radiographs, ar

Generalized ligamentous hyperlaxity and glenohumeral joint instability are common conditions that exhibit a spectrum of diverse clinical forms and may coexist in the same patient. No single diagnostic test can confirm the presence of these disorders, and a careful clinical assessment is important. Unlike patients with traumatic shoulder instability, patients with hyperlaxity and instability are more likely to experience episodes of recurrent subluxation than they are to have recurrent dislocation. They are more likely to have instability in more than one anatomic plane, and they usually do not

Successful treatment of elbow instability is aided by fundamental biomechanical understanding. This allows for clinical instability to be traced back to structural injuries, which can be addressed with targeted therapy. Acute instability of the elbow joint after frequent posterolateral dislocations tends to affect the radial ligamentous complex. In addition to simple ligamentous injuries, fractures can also contribute to elbow instability; comminuted fractures of the radial head often correlate with a rupture of the ulnar collateral ligament. Improperly healed acute injuries can result in chro

The article gives an analysis of results of conservative treatment of 503 patients with a primary traumatic dislocation of the shoulder. High frequency of the development of chronic instability of the shoulder joint was shown to be 31.2%. The using of arthroscopy, noninvasive methods of diagnosis (rentgenography, USI) allowed to diagnose the typical complex of morphological injuries following dislocations of the shoulder. General and local factors responsible for the development of chronic instability of the shoulder joint were established. Arthroscopy and early surgical stabilization of the j

The value of magnetic resonance imaging (MRI) in shoulder instability depends on its ability to demonstrate pathologic lesions within the joint. Traumatic dislocation in particular often results in capsulo-labral-avulsion, stretching of the joint capsule and impression fracture at the humeral head. Various MR weightings, including spin echo and gradient echo, are useful at the shoulder. A surface coil should be used. Image planes are oriented at the scapula. A capsulo-labral-avulsion can not be reliably demonstrated on non-contrast imaging in the absence of intra-articular effusion. With indir

The diagnosis of glenohumeral instability is based on the patient's history and physical examination. The main objective in using imaging techniques is to define the type and extent of the underlying pathology for therapeutic planning. Plain radiography is the initial step in the diagnostic procedure but can miss typical lesions of the glenohumeral joint structures. CT arthrography and MR imaging offer great sensitivity and accuracy for the detection of traumatic abnormalities of the labral-capsular complex, fractures of the glenoid rim and Hill-Sachs lesions. The advantages and pitfalls of bo
