Corneal ectasia after photorefractive keratectomy.

BACKGROUND: Corneal ectasia after photorefractive keratectomy (PRK) has only been occasionally reported, and its incidence has not been assessed. METHODS: In a retrospective, non-comparative case series, incidence of corneal ectasia was evaluated in a personal consecutive 6453-case series of myopic PRK with a minimum follow-up of 18 months. Features and prognosis were evaluated in all patients with ectasia after PRK (in both personal and referred cases). RESULTS: Ectasia was detected in three eyes of two patients (patients 1 and 2) 3 years and 1 year after PRK performed elsewhere. In additio

Phototherapeutic keratectomy for bullous keratopathy.

AIMS: This study was designed to investigate the therapeutic potential of excimer laser for recurrent painful erosions in patients not suited to treatment with penetrating keratoplasty. METHODS: Phototherapeutic keratectomy (PTK) with the excimer laser was performed prospectively on a series of 13 eyes of 12 patients with recurrent corneal erosions in connection with bullous keratopathy of varied aetiology. The main complaint of the patients before the treatment was frequent attacks of pain. The patients selected either refused corneal grafting or could not for a variety of reasons expect to

Photorefractive keratectomy in pediatric patients.

This article reports the results of photorefractive keratectomy in nine patients between 10 and 15 years of age who were treated for the following refractive errors: unilateral myopia, post intraocular lens myopia, and unequal hypermetropia with amblyopia. Visual acuity improved and spectacle correction decreased in all patients. There was significant visual improvement with pleoptic treatment in one amblyopic hypermetropic patient.

PURPOSE: To assess the safety, efficacy, and refractive predictability of half-fluence accelerated corneal collagen crosslinking (CXL) applied concurrently with photorefractive keratectomy (PRK), and compare the results to standard PRK. SETTING: Refractive Services, The Eye Foundation, Coimbatore, India. DESIGN: Interventional comparative case series. METHODS: Patients seeking refractive correction for myopia or myopic astigmatism were included in the study. Pain management after photorefractive keratectomy.

PURPOSE OF REVIEW: To review current concepts regarding the mechanisms of postoperative pain after photorefractive keratectomy (PRK) and review available treatment options. RECENT FINDINGS: Many clinical studies have established the safety and efficacy of different topical and systematic therapeutic agents and techniques for the treatment of postoperative pain after PRK, especially topical nonsteroidal anti inflammatory agents and oral nonsteroidal and narcotic medications. New therapeutic agents and techniques are continuously studied, introducing new agents and comparing teh efficacy of dif

Phototherapeutic Keratectomy: Science and Art.

PURPOSE: To describe, with videos, the principles of excimer laser phototherapeutic keratectomy (PTK) for the treatment of corneal scars, corneal surface irregularity, and recurrent corneal erosions. METHODS: Depending on the pathology in a treated cornea, the epithelium is removed either by transepithelial PTK ablation with the excimer laser or thorough scraping with a scalpel blade. Stromal PTK can be performed with or without photorefractive keratectomy (PRK), depending on the refractive status of both eyes. Residual surface irregularity is treated with masking-smoothing PTK. Typically, 0.

Orthoptic Changes following Photorefractive Keratectomy.

PURPOSE: To report orthoptic changes after photorefractive keratectomy (PRK). METHODS: This interventional case series included 297 eyes of 150 patients scheduled for PRK. Complete ophthalmologic evaluations focusing on orthoptic examinations were performed before and 3 months after PRK. RESULTS: Before PRK, 2 (1.3%) patients had esotropia which remained unchanged; 3 (2%) patients had far exotropia which improved after the procedure. Of 12 cases (8%) with initial exotropia at near, 3 (2%) cases became orthophoric, however 6 patients (4%) developed new near exotropia. A significant reduction

PURPOSE: To report the clinical features, management, and outcome of a patient who developed bilateral ectasia after photorefractive keratectomy (PRK). METHODS: Case report of a 35-year-old man who underwent bilateral PRK. Preoperative uncorrected visual acuity was 20/200 in the right eye and 20/100 in the left eye. The patient's history was unremarkable and he denied a family history of ocular disorders. RESULTS: Two weeks after surgery, the patient presented with loss of visual acuity in both eyes. Uncorrected visual acuity was 20/80 in the right eye and 20/200 in the left eye. Objective r

SETTING: Gimbel Eye Centre, Calgary, Alberta, Canada. METHOD: In a retrospective study, 3 patients (5 eyes) with unusually shaped corneal topography before surgery were selected as keratoconus suspects. Zonal photorefractive keratectomy for presbyopia.

PURPOSE: We performed zonal excimer laser photorefractive keratectomy in three eyes of three presbyopic patients using a specially designed mask, with a minimum follow-up of 24-months. METHODS: Two females (ages 59 and 48) and one male (age 55) were included in the study. The procedure was performed with a mask designed by one of the authors (GMN), applied to the Aesculap-Meditec Mel 60 excimer laser. The mask consists of a mobile diaphragm formed by two blunt blades. The aim in all the eyes was a presbyopic correction of 3.00 D. RESULTS: After an initial regression of 1.00 D, the presbyopic

Bacterial keratitis following phototherapeutic keratectomy.

BACKGROUND: Phototherapeutic keratectomy (PTK) is effective in the treatment of many superficial corneal disorders. The incidence of bacterial keratitis following PTK has not been assessed in a large, prospective clinical trial. METHODS: We report three cases of bacterial keratitis that occurred in a prospective study of 258 consecutive PTK procedures at King Khaled Eye Specialist Hospital. RESULTS: Three (1.2%) of 258 eyes developed bacterial keratitis during a period of follow up ranging from 1 to 24 months. All three cases were in 183 eyes (1.6%) with a diagnosis of climatic droplet kerat

Making sense of... photorefractive keratectomy.

The technique of photorefractive keratectomy (PRK) represents a major development in ophthalmology. Using lasers, PRK involves reshaping the cornea so that its refractive power is increased. Currently PRK can only be used to correct myopia and astigmatism, but hypermetropia may also be corrected by PRK in the near future.

Superficial lamellar keratectomy using an automated microkeratome to excise corneal scarring caused by photorefractive keratectomy.

Corneal haze is a recognized complication of excimer laser photorefractive keratectomy. When severe, it can result in the formation of a dense corneal scar, with a reduction in best corrected visual acuity. In extreme cases, medical treatment and repeated phototherapeutic excimer ablations may fail to restore corneal transparency. In these patients, penetrating keratoplasty (PKP) has been used to restore visual function. We describe a technique for excising a superficial corneal scar using an automated microkeratome. The resultant corneal surface is quite smooth, and good visual acuity may be

PURPOSE: To evaluate whether laser-assisted subepithelial keratectomy (LASEK) and photorefractive keratectomy (PRK) achieve effective targeted correction and the extent of post-treatment corneal haze after corneal transplantation. SETTING: Nonhospital surgical facility, Calgary, Alberta, Canada. DESIGN: Evidence-based manuscript.
