Safe and effective treatment of seborrheic dermatitis.

Seborrheic dermatitis is a common chronic inflammatory skin disorder that can vary in presentation from mild dandruff to dense, diffuse, adherent scale. The disorder occurs throughout the world without racial or geographic predominance; it is more common in males than females. Its precise etiology remains unknown, but the condition is strongly associated with lipophilic Malassezia yeasts found among the normal skin flora and represents a cofactor linked to several risk factors, including T-cell depression, increased sebum levels, and activation of the alternative complement pathway. The goal o

Facial seborrheic dermatitis: a report on current status and therapeutic horizons.

Seborrheic dermatitis, characterized by erythema and/or flaking or scaling in areas of high sebaceous activity, affects up to 5% of the US p … eroids, the immunosuppressant agents tacrolimus and pimecrolimus, azoles and other antifungals, and keratolytic agents. A recent exploratory study supports the efficacy and safety of azelaic acid 15% gel in seborrheic dermatitis. Azelaic acid may be especially valuable in this application because of its efficacy in treating concomitant rosacea and acne.

[Acute salicylate poisoning].

Although aspirin (acetylsalicylic acid) has become widely available without prescription, cases of self-poisoning due to overdose of salicylates are quite uncommon, with a low reported mortality. However, severe poisoning with these preparations is life threatening. Besides the aspirin, there are other sources of salicylate poisoning, such as an excessive application of topical agents, ingestion of salicylate containing ointments, use of keratolytic agents or agents containing methyl salicylate (e.g. oil of wintergreen). Most of these preparations are liquid, highly concentrated and lipid solu…

Isolated Plantar Vein Thrombosis Resembling a Corn with a Bruise.

Plantar vein thrombosis, rarely-reported disease, is usually accompanied by pain and tenderness in the plantar region and should be differentiated from other dermatological conditions causing plantar pain, such as hemorrhagic corn/callus, plantar epidermal cyst, verruca, or plantar fibromatosis. A 52-year-old man presented with a violaceous tender subcutaneous nodule overlying a hyperkeratotic plaque on his sole. Initially, he thought it was a corn and applied keratolytic agents, which failed to work. Sonography revealed a well-demarcated mass with increased peripheral vascularity. His pain wa…

Bilateral Tinea Nigra Plantaris with Good Response to Isoconazole Cream: A Case Report.

Tinea nigra is a superficial fungal infection caused by Hortaea werneckii. It typically affects young individuals as an asymptomatic unilateral macule, from light brown to black on the palms and soles, mainly in tropical and subtropical regions. In 1997, Gupta et al. [Br J Dermatol 1997;137:483-484] described the dermoscopic characteristics of tinea nigra. Topical antifungals with or without keratolytic agents can be used for the treatment. The authors report a case of a 47-year-old man with asymptomatic light brown macules bilaterally on the plantar regions. Dermoscopic examination revealed b…

Antifungal effects of Allium sativum (garlic) extract against the Aspergillus species involved in otomycosis.

Otomycosis due to saprophytic keratolytic fungi represents a small percentage of clinical external otitis. Although there are certain antibacterial and antifungal agents available, they usually are very caustic, potentially ototoxic and cannot be used if the ear drum is perforated. Garlic is utilized as a folk medicine in many countries for its antimicrobial and other beneficial properties. In response to a lack of otic preparations, the authors studied the efficacy of garlic extracts against the fungi belonging to the genus Aspergillus which are the most common cause of this infection. Atypical scabies in HIV-positive patients.

Five cases of Norwegian or keratotic scabies in HIV-positive patients are described. One of these patients was the source of an outbreak in a hospital, ultimately involving 72 persons. Three of our patients had a markedly pruritic eruption. This is unusual in crusted scabies in which pruritus is usually slight or absent. Two of the five patients had unusual CD4 counts of more than 200 cells per cubic millimeter. All our patients responded to lindane and keratolytic agents. When generalized papular, crusted, or eczematoid lesions are observed in HIV-positive patients, particularly if the CD4 co…

Effective Management of Mosaic Epidermolytic Ichthyosis in a Pediatric Patient With Urea 30% Cream: A Case Report.

Epidermolytic ichthyosis (EI) is a rare autosomal dominant keratinization disorder characterized by blistering at birth followed by progressive hyperkeratosis. Treatment is primarily supportive, involving emollients, keratolytics, and occasionally systemic agents. We report a pediatric case of clinically and histologically confirmed linear EI that showed marked improvement with topical urea 30% cream. Therapy led to substantial reduction in scaling, smoother skin texture, and improved comfort, with no adverse effects. This case highlights the potential role of high-concentration urea as a safe

[Keratoplastic effect of salicyclic acid, sulfur and a tensio-active mixture].

The right lower inner arms of 22 healthy test persons in total have been treated either with 6% salicylic acid solution in 70% isopropyl alcohol, or with 5% precipitated sulphur mixture in white petrolatum, or with a surfactant mixture. The basic preparations without active agents have been applied to the corresponding skin area on the left arms. The thickness of the corneal layer on both arms was measured by means of the cantharidin blister method. Salicylic acid, sulphur, as well as the surfactant mixture all led to a more significant reduction of the corneal layer than the basic preparation

Regression of subcorneal pustular dermatosis type of IgA pemphigus lesions with azithromycin.

We present our experience with a 56-year-old man who complained for generalized dermatopathy, manifested by skin lesions with diameter from 0.5 to 5 cm. The lesions did not respond to a 20-day systemic steroid regimen. The results of biopsies of three excised lesions, in combination with the clinical manifestations, led to the diagnosis of subcorneal pustular dermatosis type of IgA pemphigus. An unexpected improvement was noted after treatment with azithromycin (which was provided for management of concurrent non-specific urethritis) and local non-potent steroid plus keratolytic agent ointment

Local wound care and topical management of hidradenitis suppurativa.

Hidradenitis suppurativa (HS) is a chronic, recurrent, debilitating disease predominantly involving apocrine gland-bearing skin. The folliculoinfundibular dysfunction and an aberrant cutaneous immune response to commensal bacteria are recognized as potential contributors. Topical antibiotics, such as clindamycin, and keratolytic agents have been used in the management of early stages of HS. Proper wound care is a key part of management, particularly in patients with advanced HS. The evidence for the optimal topical therapy or optimal local wound care is limited. As such, a multidisciplinary ap…

Intralesional pingyangmycin treatment for resistant plantar warts.

BACKGROUND: The diagnosis of viral warts is often straightforward, but their treatment is difficult and always takes a long time. Nowadays warts are usually treated with conventional methods including topical keratolytic agents, cryotherapy, electrosurgery and laser surgery. However, such treatments cause different complications, and the warts become recalcitrant. OBJECTIVE: To investigate the effect of intralesional pingyangmycin injection for resistant plantar warts. METHODS: From June 2006 to September 2008, we treated 66 patients with plantar warts by intralesional pingyangmycin injectio…

Verrucas. Guidelines for management.

Most people will experience infection with human papilloma virus (HPV) at some point in their life. Diagnosis, based on clinical examination, is usually straight forward. Treatment can, however, be challenging. Indications for treatment include pain, interference with function, cosmetic embarrassment, and risk of malignancy. Clearance rates are highest in young, healthy individuals with short duration of infection. Treatment may be with destructive agents (keratolytics, cryotherapy, curettage and cautery, laser, photodynamic therapy), with antimitotic agents (podophyllin, bleomycin, retinoids)

Salicylic acid as a peeling agent: a comprehensive review.

Salicylic acid has been used to treat various skin disorders for more than 2,000 years. The ability of salicylic acid to exfoliate the stratum corneum makes it a good agent for peeling. In particular, the comedolytic property of salicylic acid makes it a useful peeling agent for patients with acne. Once considered as a keratolytic agent, the role of salicylic acid as a desmolytic agent, because of its ability to disrupt cellular junctions rather than breaking or lysing intercellular keratin filaments, is now recognized and is discussed here. Salicylic acid as a peeling agent has a number of in

Treatment of recalcitrant periungual warts with cimetidine in pediatrics.

Viral warts, caused by human papilloma virus (HPV), are commonly seen in dermatology clinics. However, treatment for warts can be challenging, and there is no single method of treatment that is universally effective. Conventional therapies involve the physical destruction of lesions, including cryotherapy, electrodesiccation, CO2 laser, and topical keratolytic agents. Successful treatment of recalcitrant warts, especially periungual lesions and verruca plana of the face, had been limited with the conventional modalities. These painful procedures resulting in poor compliance, cosmetic disfigura…
