Influence of the renal lower pole anatomy and mid-renal-zone classification in successful approach to the calices during flexible ureteroscopy.

PURPOSE: The aim of this paper is to analyze if the anatomy type of the collector system (CS) limits the accessibility of flexible ureteroscopy (FUR) in the lower pole. METHODS: We analyzed the pyelographies of 51 patients submitted to FUR and divided the CS into four groups: A1-kidney midzone (KM) drained by minor calices (Mc) that are dependent on the superior or on the inferior caliceal groups; A2-KM drained by crossed calices; B1-KM drained by a major caliceal group independent both of the superior and inferior groups, and B2-KM drained by Mc entering directly into the renal pelvis. Correlation analysis between renal anatomical factors and residual stones after an ultrasound-guided PCNL.

INTRODUCTION: To predict the factors of residual stones after percutaneous nephrolithotomy (PCNL) by analyzing the characteristics of the renal anatomical structure in intravenous urography, so as to make a reasonable operation plan, reduce the risk of residual stones in PCNL, and improve the stone-free rate (SFR). METHODS: A retrospective study was performed between January 2019 and September 2020 for patients treated with PCNL. According to the results of a kidney ureter bladder review after PCNL, 245 patients were divided into a residual stone group (71 patients, stone size >4 mm) and a st

Is pelvicaliceal anatomy a risk factor for stone formation in patients with solitary upper caliceal stone?

OBJECTIVES: To investigate the effect of pelvicaliceal anatomy on stone formation in patients with solitary upper caliceal stones. METHODS: The records of patients with solitary upper caliceal stones between 1996 and 2004 were reviewed. After the exclusion of patients with hydronephrosis, major anatomic abnormalities, noncalcium stones, metabolic abnormalities, history of recurrent stone disease, multiple stones, and previous renal surgery, 42 patients (24 male, 18 female) and 42 healthy subjects (22 male, 20 female) with normal results on intravenous pyelography (IVP) were enrolled into the

[A case of esophageal cancer metastatic to the kidney presenting as renal pelvic cancer].

A 50-year-old man, who had undergone operation of esophageal carcinoma 2 years earlier, was admitted with the complaints of right flank pain and macroscopic hematuria. Intravenous urogram showed right unvisualized kidney. Right retrograde pyelography showed the dilatation of calices and irregularity of middle and lower calices. Computed tomography revealed severe hydronephrosis of the right kidney. Right nephrectomy was performed under the diagnosis of either renal pelvic cancer or esophageal cancer metastatic to the kidney. Pathological examination revealed metastatic squamous cell carcinoma

Anatomic predictors of formation of lower caliceal calculi: is it the time for three-dimensional computed tomography urography?

OBJECTIVE: To evaluate the various anatomic factors, using computed tomography urography (CTU), that predispose to a lower-pole kidney stone formation on one side compared with the other. MATERIALS AND METHODS: The study included 51 patients with a solitary lower-pole stone. Lower-pole infundibulopelvic angle (IPA), infundibular width (IW), infundibular length (IL), caliceal volume (CV), and number of minor calices of the affected lower calyx and normal contralateral kidney were measured based on CTU. The IPA was measured according to the methods of Sampaio and Elbahnasy. A comparison was mad

Accuracy of ultrasound in the diagnosis of nonfunctioning kidneys.

The accuracy and reliability of ultrasound were assessed in 246 patients with 338 radiologically nonfunctioning renal units. The causes of nonfunction included hydronephrosis, pyonephrosis, renal parenchymal disease, renal agenesis, renal neoplasm and polycystic disease. Obstruction was proved in 158 patients, with an accuracy of 98.1 per cent. Retrograde pyelography was avoided. The diagnosis of a small end stage kidney was correct in 42 patients. In 9 patients with a unilateral nonfunctioning kidney less than 9 cm. long a thin coarsely textured cortex with dilatation of 1 or more calices was

Two-part silicone mold. A new tool for flexible ureteroscopy surgical training.

INTRODUCTION AND OBJECTIVES:: Flexible ureteroscopy is a common procedure nowadays. Most of the training programs use virtual reality simulators. The aim of this study was to standardize the building of a three-dimensional silicone mold (cavity) of the collecting system, on the basis of polyester resin endocasts, which can be used in surgical training programs. MATERIALS AND METHODS:: A yellow polyester resin was injected into the ureter to fill the collecting system of 24 cadaveric fresh human kidneys. After setting off the resin, the kidneys were immersed in hydrochloric acid until total co

Renal collecting system anatomy: its possible role in the effectiveness of renal stone treatment.

The anatomy of the kidney collecting system may play a role in the selection of the best method of kidney stone treatment for a specific patient. Also, an analysis of the collecting-system anatomy would indicate the likely effectiveness of each method of treatment. For stones located in the lower pole, the clearance rate after shockwave lithotripsy has been uniformly low relative to that of calculi elsewhere. Some special anatomical findings suggest that retention of what are considered to be 'passable stone fragments' (4 mm in diameter or less) in the inferior pole might be a consequence not

[The collector system of the kidney. Applied anatomy based on the analysis of 3-dimensional casts].

The kidney collector system were studied in 120 three-dimensional casts obtained by the injection-corrosion method. To the procurement of the first hundred casts, we made use of a polyester resin; however on the preparation of the last twenty we made use of a mixture containing latex and radiographic contrast as we intended to obtain, besides the casts, the roentgenograms. Two anatomic details, which have a great importance for endourology, were observed: a. the calices' cross draining the meso-renal region and the consequent inter-pelvis-calice (space (IPC) formation which is seen in the roen

Pan-caliceal ileoneocystostomy: indications, modifications and further evaluation.

During the last 4 years we treated 11 patients with obstructed kidneys and intrapelvic strictures, which were owing to tuberculosis in 9 and renal stone surgery in 2. Pan-caliceal ileoneocystostomy was performed in all 11 cases. The entire caliceal system was anastomosed to an ileal ureter and, thus, relieved the obstruction. The result of this operation was most gratifying. Although 1 patient died of anastomotic leakage and sepsis the morbidity was low.

The clinical significance of renal caliceal diverticulum in children: relation to vesicoureteral reflux.

Of 250 children with vesicoureteral reflux, 23 had a renal caliceal diverticulum in one or both kidneys. In a third of these 23 children this was the only renal change noted and in the remaining two-thirds other changes of chronic pyelonephritis were seen. The presence of a caliceal diverticulum is evidence that renal damage of some degree is associated with reflux and urinary tract infection, and should be added to existing criteria for investigation of reflux in a child.

Intrapelvic pressure and caliceal dilatation in the evaluation of intermittent hydronephrosis.

We investigated 10 patients with suspected unilateral hydronephrosis and normal or minimal widening of the calices or renal pelvis on routine urography by high rate perfusion of the renal pelvis and simultaneous intrapelvic pressure measurement. All affected kidneys had reduced renal function (26 +/- 7 ml. per minute per 1.73 body surface area) evaluated from total and separate glomerular filtration rate studies using 51chromium-ethylenediaminetetraacetic acid clearance technique and isotope renography. Four patients without signs of obstruction operated upon because of renal calculi were used

[Ureterorenoscopy with a flexible instrument].

Flexible ureterorenoscopy should be routinely used at the Urological Centers that deal with urinary stones. Flexible instruments should be used for both diagnostic and therapeutic purposes, allowing a safe exploration of the whole upper urinary tract. Thanks to their flexibility and to the active and passive deflection of their distal part, these instruments allowed to successfully treat several difficult situations, such as renal caliceal calculi in the lower calices or even in some middle/upper calices or in horseshoe kidneys. The therapeutic potential of this approach is enhanced by the lar

Xanthogranulomatous pyelonephritis associated with a congenital caliceal diverticulum.

We report a rare case of xanthogranulomatous pyelonephritis occurring in a caliceal diverticulum in a 1-year-old child. She presented with recurrent urinary tract infections. A computed tomography scan demonstrated an enhancing cystic lesion in the left kidney. At open exploration, a caliceal diverticulum was found with a very stenotic opening into the renal pelvis. The diverticulum was enucleated, and the pathologic examination confirmed xanthogranulomatous pyelonephritis in the parenchyma surrounding the diverticulum. The child did well postoperatively with no recurrence of her urinary tract

The pig kidney as an endourologic model: anatomic contribution.

We present detailed anatomic findings on collecting system anatomy and renal morphometry in the pig and compare these findings with previous findings in humans. We studied three-dimensional polyester resin corrosion endocasts of the pelviocaliceal system obtained from 100 kidneys (50 pigs). Eighty kidneys were evaluated morphometrically, considering length, cranial pole width, caudal pole width, thickness, and weight. The pig collecting system was classified into two major groups (A and B). Group A (40%) was composed of kidneys in which the mid-zone is drained by calices dependent on the crani
