[Diuretics and kidney diseases].

The main indications for diuretic treatment of renal patients are edema and hypertension. Pharmacokinetics and pharmacodynamics of diuretics are altered in patients with proteinuria and/or impaired renal function. These patients exhibit avid sodium retention. Diuretics are partially inactivated by binding to proteins in tubular fluid. The natriuretic response to diuretics is limited by counter-regulation, specifically increased proximal tubular reabsorption in response to hypovolemia and increased distal tubular sodium reabsorption in response to increased sodium load. At higher serum creatini

[Kidney diseases and hypertension].

Renal disease has been recognized as both a cause and a consequence of hypertension. Renal hypertension may be of vascular and nonvascular origin. Generally, the prevalence of hypertension increases with decreasing renal function, including more than 90% of patients with terminal renal failure. However, hypertension is more often found in glomerular than in interstitial disease. The pathomechanisms operative in renal hypertension are sodium retention with concomitant volume expansion, an increase in plasma renin activity or a combination of both factors. While mechanical intervention is usuall

Kidney organoids: a pioneering model for kidney diseases.

The kidney is a vital organ that regulates the bodily fluid and electrolyte homeostasis via tailored urinary excretion. Kidney injuries that cause severe or progressive chronic kidney disease have driven the growing population of patients with end-stage kidney disease, leading to substantial patient morbidity and mortality. This irreversible kidney damage has also created a huge socioeconomical burden on the healthcare system, highlighting the need for novel translational research models for progressive kidney diseases. Conventional research methods such as in vitro 2D cell culture or animal m

Gut-Kidney Axis: Novel Insights in Kidney Diseases.

Chronic kidney disease (CKD) is increasingly prevalent worldwide, and its management is an urgent issue. Understanding of its pathophysiology and the development of new treatment methods are critical. In recent years, gut microbiota reportedly contributes to the homeostasis of systemic organs and the pathogenesis of diseases. Also, in kidney disease, alterations in the gut microbiota (dysbiosis) have been observed and are deeply involved in pathophysiology. d-Amino acids derived from gut microbiota change in concentration in the body concomitant with dysbiosis. d-Serine and d-alanine have been

[Kidney function and kidney diseases in the elderly].

The significance of age-induced alterations in renal function is described in order to explain clinically important entities in older people, such as hypo- and hypervolaemia, hyper- and hyponatraemia, hyperkalaemia and metabolic acidosis. A sound knowledge of the age-dependent changes in renal function enables the physician to effectively prevent many of these metabolic disturbances. Renovascular disease, nephrotic syndrome, renal cysts and acute renal failure are renal diseases which typically have a higher incidence in older people and require special consideration when evaluating renal dise

Cell therapy for kidney diseases.

The prevalence of kidney disease continues to increase worldwide, with a considerable number of patients requiring long-term medication and renal replacement therapy. The pathogenesis and progression of kidney disease are associated with renal cellular injury and immune dysregulation. Conventional kidney disease treatment strategies, such as pharmacotherapy and renal replacement therapy, are unable to reverse the progression of kidney disease. To overcome this problem, kidney transplantation has been developed. However, its clinical application remains limited by challenges such as immune reje

Atmospheric stressors and kidney diseases.

The adverse effects of atmospheric conditions - including ambient air pollution from industry, wildfires and extreme atmospheric temperatures - on human health are well documented. However, their hazardous roles in kidney health have often been overlooked, despite the existence of plausible biological mechanisms for pathogenic effects. Emerging evidence from epidemiological studies indicates that exposures to ambient air pollutants such as particulate matter (PM2.5 and PM10), nitrogen dioxide, ozone, wildfire smoke and extreme hot and cold temperatures may increase the risks of acute kidney in

Autoinflammatory diseases and the kidney.

The kidney represents an important target of systemic inflammation. Its involvement in monogenic and multifactorial autoinflammatory diseases (AIDs) vary from peculiar and relatively frequent manifestations to some rare but severe features that may end up requiring transplantation. The pathogenetic background is also very heterogeneous ranging from amyloidosis to non-amyloid related damage rooted in inflammasome activation. Kidney involvement in monogenic and polygenic AIDs may present as renal amyloidosis, IgA nephropathy, and more rarely as various forms of glomerulonephritis (GN), namely se

Melatonin Treatment in Kidney Diseases.

Melatonin is a neurohormone that is mainly secreted by the pineal gland. It coordinates the work of the superior biological clock and consequently affects many processes in the human body. Disorders of the waking and sleeping period result in nervous system imbalance and generate metabolic and endocrine derangements. The purpose of this review is to provide information regarding the potential benefits of melatonin use, particularly in kidney diseases. The impact on the cardiovascular system, diabetes, and homeostasis causes melatonin to be indirectly connected to kidney function and quality of

Kidney Involvement in Autoinflammatory Diseases.

BACKGROUND: Autoinflammatory diseases (AIDs) were first proposed 20 years ago and caused by dysregulation of the innate immune system, leading to episodes of systemic inflammation. Advances in next-generation sequencing and biological technology have resulted in the identification of new monogenic diseases and the corresponding signaling pathways that may guide us in targeted therapy. The kidney is a major target organ of various inflammatory processes. SUMMARY: During systemic inflammation, increased pro-inflammatory cytokines, such as IL-6, IL-1β, and TNF, lead to over-transcription and rel

Kidney complications of parasitic diseases.

Parasitic agents have been known to cause human disease since ancient times and are endemic in tropical and subtropical regions. Complications of parasitic diseases, including kidney involvement, are associated with worse outcomes. Chagas disease, filariasis, leishmaniasis, malaria and schistosomiasis are important parasitic diseases that can damage the kidney. These diseases affect millions of people worldwide, primarily in Africa, Asia and Latin America, and kidney involvement is associated with increased mortality. The most common kidney complications of parasitic diseases are acute kidney

mTOR Signaling in Kidney Diseases.

The mammalian target of rapamycin (mTOR), a serine/threonine protein kinase, is crucial in regulating cell growth, metabolism, proliferation, and survival. Under physiologic conditions, mTOR signaling maintains podocyte and tubular cell homeostasis. In AKI, activation of mTOR signaling in tubular cells and interstitial fibroblasts promotes renal regeneration and repair. However, constitutive activation of mTOR signaling in kidneys results in the initiation and progression of glomerular hypertrophy, interstitial fibrosis, polycystic kidney disease, and renal cell carcinoma. Here, we summarize t

Autonomic dysfunction in kidney diseases.

Kidney diseases are associated with many cardiovascular risk factors, such as anaemia, inflammation and chronic volume overload. Changes in the sympathovagal balance are common findings in patients with end-stage renal disease (ESRD). In particular, sympathetic hyperactivity is linked with an increase in resting heart rate leading to myocardial hypertrophy and fibrosis. The latter increases the risk of sudden cardiac death from fatal arrythmias and therefore assessment of both sympathetic and parasympathetic tones could be clinically relevant in ESRD patients. Heart rate variability and other

Autophagy in Chronic Kidney Diseases.

Autophagy is a cellular recycling process involving self-degradation and reconstruction of damaged organelles and proteins. Current evidence suggests that autophagy is critical in kidney physiology and homeostasis. In clinical studies, autophagy activations and inhibitions are linked to acute kidney injuries, chronic kidney diseases, diabetic nephropathies, and polycystic kidney diseases. Oxidative stress, inflammation, and mitochondrial dysfunction, which are implicated as important mechanisms underlying many kidney diseases, modulate the autophagy activation and inhibition and lead to cellul

Stem cells in kidney diseases.

Circulating bone marrow-derived endothelial progenitor cells (EPCs) seem to play a crucial role in both vasculogenesis and vascular homeostasis. Chronic kidney disease is a state of endothelial dysfunction, accelerated progression of atherosclerosis and high cardiovascular risk. As a consequence, cardiovascular disorders are the main cause of death in end-stage renal disease (ESRD). It has been shown that patients with advanced renal failure have decreased number of bone marrow-derived endothelial progenitor cells and impaired EPCs function. Moreover, in kidney transplant patients, renal graft
