[Pulmonary function in patients with chronic kidney failure].

Changes in the function of the respiratory system are among the frequent complications of chronic renal failure whereby their clinical impact is manifested above all in critically ill patients. A typical finding during ventilometric examinations is the limited airflow in the distal airways, and even the clinical picture of obstructive ventilation disorder is not rare. A reduced diffusion capacity of the lungs for CO was found in uraemic patients in long-term dialyzation program as well as after successful renal transplantation. The function of respiratory muscles is characterized by a reductio

Heart Failure in Patients with Chronic Kidney Disease.

The function of the kidney is tightly linked to the function of the heart. Dysfunction/disease of the kidney may initiate, accentuate, or precipitate of the cardiac dysfunction/disease and vice versa, contributing to a negative spiral. Further, the reciprocal association between the heart and the kidney may occur on top of other entities, usually diabetes, hypertension, and atherosclerosis, simultaneously affecting the two organs. Chronic kidney disease (CKD) can influence cardiac function through altered hemodynamics and salt and water retention, leading to venous congestion and therefore, no

Epidemiology of chronic kidney disease in heart failure.

Heart failure is common and is associated with a poor prognosis. Chronic kidney disease is common in heart failure and shares many risk factors with heart failure, such as age, hypertension, diabetes, and coronary artery disease. Over half of all patients who have heart failure may have moderate-to-severe chronic kidney disease. The presence of chronic kidney disease is associated with increased morbidity and mortality, yet it is also associated with underuse of evidence-based heart failure therapy that may reduce morbidity and mortality. Understanding the epidemiology and outcomes of chronic

Chronic Kidney Disease as a Comorbidity in Heart Failure.

Heart failure (HF) is one of the greatest problems in healthcare and it often coexists with declining renal function. The pathophysiology between the heart and the kidneys is bidirectional. Common mechanisms leading to the dysfunction of these organs result in a vicious cycle of cardiorenal deterioration. It is also associated with difficulties in the treatment of aggravating HF and chronic kidney disease (CKD) and, as a consequence, recurrent hospitalizations and death. As the worsening of renal function has an undeniably negative impact on the outcomes in patients with HF, searching for new

[A combination of chronic kidney failure with hereditary angioedema].

A case of a woman with a hereditary angioedema in combination with chronic renal failure is presented. The chronic renal failure is a late consequence of complications which had occurred in the course of a difficult parturition. The difficult parturition was probably the result of an expressed edema of the vaginal wall. The therapeutic difficulties in this case are pointed out.

Multicystic transformation of kidneys in chronic renal failure.

With real-time sonography, 120 nondialyzed uremic patients prior to hemodialysis, 108 patients on maintenance hemodialysis and 9 patients postdialysis after successful homotransplantation were examined for the presence of renal cysts. Even in incipient renal failure, multiple cysts were demonstrable in some patients (at a serum creatinine of 3 mg/dl in 22% of patients), particularly in patients with analgesic nephropathy. When hemodialysis was started (serum creatinine approximately 10 mg/dl), 35% of the patients had multiple cysts. On hemodialysis, the prevalence, number and size of cysts ros

[The problem of sodium metabolism in chronic kidney failure].

The endogenous 24-hour-clearance and the sodium excretion during 24 hours in the urine were investigated on 12 healthy persons with chronic nephropathies, 5 patients with condition after acute renal failure and 75 patients with condition after transplantation of a kidney. A correlation between the quantity of urine and the excretion of sodium in 24 hours could be proved in patients with a retention to 884.0 mumol/l creatinine in the serum. Only with further restriction of the renal function a reduction of the sodium excretion during 24 hours below 150 mmol becomes significant. In 16 of 25 pati

Inflammation-based scores predict chronic kidney disease progression in patients with chronic kidney disease and chronic heart failure.

BACKGROUND: Inflammation is associated with adverse outcomes of chronic kidney disease (CKD) or chronic heart failure (CHF), but few large data exist. We aimed to explore the clinical associations, and prognostic consequences of inflammation-based scores in patients with CKD and CHF. METHODS: This work was a retrospective cohort study. Glasgow Prognostic Score (GPS), modified Glasgow Prognostic Score (mGPS), Prognostic Nutritional Index (PNI) and Prognostic Index (PI), were used to explore its relationship with CKD progression in patients with CKD stage 1-3b and CHF from the China Renal Data

Optimal patient care in advanced chronic kidney disease progressing to kidney failure.

The transition from advanced chronic kidney disease (CKD) to kidney failure requires comprehensive management to optimize patient outcomes. This crucial period, in practical terms defined by CKD stages G4 and G5, involves complex decision-making regarding kidney replacement therapy, pre-emptive kidney transplantation and conservative kidney management. Patient preferences, quality of life, and comorbidities, especially cardiovascular disease, are essential considerations when making treatment decisions. Importantly, nephroprotective therapies should be continued even at advanced stages of CKD

Treatment of diabetes in patients with advanced chronic kidney disease or kidney failure.

Chronic kidney disease (CKD) is common in both type 1 and type 2 diabetes and complicated glycemic control due to fluctuating insulin resistance and metabolism. In advanced CKD (stage 4 and stage 5, or estimated glomerular filtration rate <30 mL/min/1.73 m2), reduced kidney clearance of insulin can lower or eliminated insulin needs, but glycemic control remains challenging. This article reviews optimal glycemic targets, monitoring and current therapeutic recommendations for patients with severe CKD or on dialysis, addressing the lack of efficacy data for many antihyperglycemics in this populat

Hypertension in chronic kidney disease and future heart failure.

PURPOSE OF REVIEW: Hypertension and chronic kidney diseases (CKDs) are known risk factors for the development or worsening of heart failure. In last years, several new therapeutic approaches for the management of people with diabetic and nondiabetic CKD and hypertension have been investigated. In this brief review, the most recent findings regarding the ability of SGLT-2 inhibitors and nonsteroidal mineralocorticoid receptor antagonists (nsMRA) and GLP-1 receptor agonists to prevent heart failure in patients with hypertension and CKD will be discussed. RECENT FINDINGS: In the last 3 years, se

Heart failure in advanced chronic kidney disease: treatment rationale.

Patients with chronic kidney disease (CKD) exhibit an increased risk to develop heart failure and the presence of heart failure in patients with CKD leads to a worse prognosis. The following overview article summarizes the current standard of medical heart failure therapy and discusses the treatment characteristics in patients with CKD.

Intravenous iron in heart failure and chronic kidney disease.

Intravenous iron therapy is increasingly being used worldwide to treat anemia in chronic kidney disease and more recently iron deficiency in heart failure. Promising results were obtained in randomized clinical trials in the latter, showing symptomatic and functional capacity improvement with intravenous iron therapy. Meanwhile, confirmation of clinical benefit in hard-endpoints such as mortality and hospitalization is expected in large clinical trials that are already taking place. In chronic kidney disease, concern about iron overload is being substituted by claims of direct cardiovascular b

[Probability of acute heart failure in chronic kidney disease].

BACKGROUND: In Mexico 130,000 people live with chronic kidney disease (CKD). Heart conditions are the most frequent clinical problems; 45% of the deaths of patients in replacement therapy have a cardiac origin. OBJECTIVE: To identify the probability of presenting acute heart failure (AHF) in patients with CKD in the Adult Emergency Department (AED) of a second-level hospital. MATERIAL AND METHODS: Case-control study with 111 cases and 103 controls. Cases were defined as patients with CKD admitted to AED for AHF compared with controls: patients with CKD who were admitted for a different diagn

Acute kidney injury in acute on chronic liver failure.

Acute on chronic liver failure (ACLF) is a distinct clinical entity; however, there is still debate in the way it is defined in the East as compared to the West, especially with respect to incorporation of kidney dysfunction or failure in the definition of ACLF. Kidney dysfunction is defined as serum creatinine between 1.5 and 1.9 mg/dl and kidney failure as serum creatinine of more than 2 mg/dl or requirement of renal replacement therapy according to the EASL-CLIF Consortium. Kidney dysfunction or failure is universally present in patients with ACLF according to the definition by the EASL-CLI

Association of race, heart failure and chronic kidney disease.

Heart failure and kidney disease are two important emerging epidemics. The importance of pre-end stage kidney disease was introduced in the 2002 publication of the National Kidney Foundation's Chronic Kidney Disease Guidelines. One in nine US adults has some degree of kidney disease, many of whom also have heart failure. Among all patients with heart failure, approximately half have significant kidney disease. The distribution of etiologies of these conditions varies among races; blacks tend to have heart and kidney disease predominantly due to hypertension, while whites tend to be affected by

The interplay between heart failure and chronic kidney disease.

Chronic kidney disease (CKD) and heart failure (HF) are two globally prevalent, independent, long-term conditions, which often coexist in an individual and display a bidirectional yet interconnected relationship. The presence of CKD often leads to the development of HF and vice versa, which propagates the worsening of each disease, reflecting an intertwined disease cycle. Both HF and CKD share common risk factors, such as increasing age, diabetes, high blood pressure, obesity and smoking. Data show that approximately half of all people with HF also have CKD, which impacts patient burden and qu
