OBJECTIVES: This study aimed to assess the results of knee capsulotomy for correcting fixed knee flexion contracture in children with cerebral palsy (CP).
METHODS: Thirty-five children (20 boys, 15 girls; mean age 13.5+/-2.5 years) with CP underwent posterior knee capsulotomy for 59 knees. Eleven patients had diplegia, one patient had hemiplegia, and 23 patients had quadriplegia. There were two community ambulators (3 knees), 19 household ambulators (33 knees), and 14 nonambulators (23 knees). Posterior knee capsulotomy was combined with hamstring lengthening (50 knees, 84.8%), rectus femoris

This study determined knee joint proprioception in below-knee amputees using two methods of measurement of knee joint proprioception. The threshold for detection of passive motion and the ability to reproduce specific limb positions were recorded in nine below-knee amputees. A significant difference was observed between the prosthetic and sound limbs in the amputees in the threshold of detection test. The large angles (5 degrees to 25 degrees) involved in position reproduction tests may have provided cues through the cuff suspension. Different strategies are used by below-knee amputees to disc

PURPOSE: Knee function, whether anterior cruciate ligament (ACL)-deficient or ACL-reconstructed, is related to many conditions, and no single biomechanical variable can be used to definitively assess knee performance. The purpose of this study was to investigate the relationship between extension and flexion muscle strength and knee function in patients prior and following ACL reconstruction.
METHODS: 44 ACL-deficient patients with a mean age of 26.6 years were tested between 3 and 6 months following an acute injury and 2 years following ACL reconstruction. All reconstructed patients underwen

Spontaneous osteonecrosis of the knee (SON) is an osteonecrosis that mainly affects the medial femoral condyle. In endstage SON, knee arthroplasty is the therapy of choice. Because of the unicompartimental nature of the knee, unicondylar knee arthroplasty is considered an ideal implant for treatment of this condition. The purpose of this study was to prove that the long-term results of unicondylar implants are better than the results of bicondylar implants for the treatment of SON. All patients treated for SON between 1984 and 2000 have been recorded. Two groups were formed according to the im

BACKGROUND: Reduced knee confidence is common in people with knee osteoarthritis and may influence how they engage with physical activity. A lack of confidence may lead an individual to adopt compensatory movement strategies, such as reduced vertical knee loading or movement in the sagittal plane, which may contribute to disability. As such, reduced confidence may be an important treatment target for clinicians working to improve function in people with knee osteoarthritis.
RESEARCH QUESTION: Is there an association between knee confidence and vertical knee loading or sagittal-plane knee move

Accurate lateral knee radiographs are essential for assessing pathology and planning surgery. However, achieving adequate femoral condyle overlap is technically challenging because of individual variations in lower limb alignment. We analyzed the alignment-dependent bone morphology and proposed practical X-ray tube angles to optimize lateral imaging. Full-length lower limb radiographs of 212 normal and 191 knees with osteoarthritis (KOA) were examined. The lateral distal femoral angle (LDFA) and medial proximal tibial angle were measured to classify the alignment into varus, neutral, and valgu

Chronic knee pain is common at all ages, particularly in the elderly, among whom it has its greatest impact. Chronic knee pain is often ascribed to osteoarthritis in adults and to chondromalacia patellae in children and adolescents. Pathological findings in both these conditions correlate poorly, however, with the severity of knee pain and disability. Psychometric variables correlate better with the impact of knee osteoarthritis, suggesting that this disorder has characteristics of a regional pain syndrome. This perception may reflect our lack of understanding of the biological mechanisms in t

Results of open reduction and internal fixation for complex articular fractures around the knee are poor, particularly in elderly osteoporotic patients.Open reduction and internal fixation may lead to an extended hospital stay and non-weight-bearing period. This may lead to occurrence of complications related to decubitus such as thrombo-embolic events, pneumonia and disorientation.Primary arthroplasty can be a valuable option in a case-based and patient-specific approach. It may reduce the number of procedures and allow early full weight-bearing, avoiding the above-mentioned complications.Ther

BACKGROUND: Osteotomy around the knee and unicondylar knee replacement can both produce excellent results in unilateral knee osteoarthritis. The indications for these procedures differ significantly and are discussed in this article.
METHODS: A weight-bearing long-leg radiograph, clinical stability tests and varus-valgus-stress radiographs are mandatory for a preoperative analysis. Osteotomy is a safe and reproducible procedure when a biplanar cutting technique is used and fixation is achieved with a plate fixator. Unicondylar knee replacement can be performed minimally invasive with a quadri

BACKGROUND: Primary total knee arthroplasty (TKA) in severe valgus knees may prove challenging, and choice of implant depends on the severity of the valgus deformity and the extent of soft-tissue release. The purpose of this study was to review 8 to 11 years (mean, 10 years) follow-up results of primary TKA for varient-III valgus knee deformity with use of different type implants.
METHODS: Between January 2002 and January 2005, 20 women and 12 men, aged 47 to 63 (mean, 57.19 ± 6.08) years old, with varient-III valgus knees underwent primary TKA. Of the 32 patients, 37 knees had varient-III de

AIM: Outcome of primary total arthroplasty for osteoarthritis of the knee with valgus deformity.
MATERIALS AND METHODS: Between 2005 and 2007, 28 primary total knee replacements were performed for osteoarthritis of the knee with valgus deformity. 21 cases were women and 7 men with a mean age of 66.6 years (extremes 54-81). The clinical and radiological evaluations were done considering the knee range of motion, Knee Society Score (KSS) and femorotibial angle measured on the frontal standing long leg X-rays. Preoperatively, the knee valgus deformity angle was 6 to 15 degrees in 14 cases, 15 to

OBJECTIVES: In this study, the factors causing stiff knee after primary total knee arthroplasty (TKA) in patients with medial compartment osteoarthrosis were evaluated.
PATIENTS AND METHODS: Primary TKA surgery was performed in fifty-three knees in 48 patients (42 females, 6 males; mean age 67.2±8.5; range 59 to 76 years) diagnosed with medial compartment osteoarthrosis. Five patients (10%) out of forty-eight underwent bilateral primary TKA. In the patients who had bilateral TKA, the second operation was performed six months later. Patello-femoral arthroplasty was performed in none of the pat

This prospective study aimed to establish the pattern of knee skin temperature following uncomplicated primary total knee replacement. Thirty-two patients were included. The skin temperature of operated and contralateral knees was measured preoperatively and daily during the first 6 weeks postoperatively. Measurements were also taken at 3, 6, 12 and 24 months following surgery. The difference in temperature between the two knees had a mean value of (+)2.9 degrees C at 7 days. This mean value decreased to (+)1.6 degrees C at 6 weeks, (+)1.3 degrees C at 3 months, (+)0.9 degrees C at 6 months (+

We present the case of a 75-year-old woman who, following a below-knee amputation, developed worsening osteoarthritis of her knee in the same limb. A total knee replacement was subsequently performed in the amputated limb. The patient successfully mobilized following the procedure and remained pain-free on review after 8 months. The Knee Society score improved from 53 to 85, and the function score increased from 0 to 40. Modifications to the standard technique of total knee arthroplasty used in this case and the postoperative rehabilitation methods are described. Total knee replacement should

We reviewed 17 cases of posttraumatic arthritis after fractures about the knee. All patients underwent total knee replacement; one had a distal femoral osteotomy to correct a deformity prior to arthroplasty. Thirteen patients had a minimum one-year follow-up, with an average follow-up of 27 months (range, one to four years). Of these, eight were considered to have a successful clinical result. All five patients with unsuccessful results had major intraoperative and/or postoperative complications. In malunions of intra-articular fractures of the proximal tibia, the important technical considera

PURPOSE: In the last couple of years, a significant amount of studies comparing the UKA and TKA for unicompartmental knee osteoarthritis have been published. However, there is a lack of recent meta-analysis comparing the two implants. Since the number of performed UKAs is currently increasing and the indications are obsolete, it becomes important to update current evidences and outcomes. With these premises, a meta-analysis of clinical trials comparing UKA versus TKA was conducted.
METHODS: In October 2018, the following databases were accessed: Cochrane Systematic Reviews, Scopus, PubMed and

A key factor in successful knee arthroplasty is restoration of correct knee alignment. Although traditional total knee arthroplasty effectively restores alignment, it is unclear whether similar results can be consistently obtained with more conservative procedures. The purpose of this study was to assess alignment after implantation of the Journey Deuce Bi-Compartmental Knee System (Smith & Nephew, Inc, Memphis, Tenn). Mechanical axis was assessed in 137 cases. Preoperatively, 77 (56%) of 137 patients had a mechanical axis passing far medial to the center of the knee, consistent with varus def

This study was undertaken to investigate whether high tibial osteotomy (HTO) still had a role in the treatment of osteoarthritis of the knee joint. The author has performed photoelasticity studies which confirmed abnormal stress distribution over the joint, as soon as its mechanical axis was deviated and the joint line had an obliquity over 10 degrees. High tibial osteotomy to correct varus or valgus deformity restores a symmetrical stress distribution and represents the only etiological treatment of secondary osteoarthritis of the knee. Two hundred and fifty HTO's were performed between 1971
