Knee dislocations with vascular injuries.

Any knee that is dislocated or that has both anterior and posterior cruciates and one collateral ligament torn should be carefully evaluated for neurologic and vascular injury. Reduction and immobilization should be performed. If the pulses are anything but normal, arrangements should be made to proceed to the operating room immediately. A one-shot arteriogram can be obtained in the operating room, but proceeding directly with popliteal artery exploration is reasonable if the pulses have not returned to normal. If the pulses have returned to normal, an arteriogram should be obtained. After the

[Open injuries of the knee].

The authors discuss the therapeutic approach to 9 cases with open injuries of the knee which they have encountered during the past 5 years. All patients were hospitalized in a state of serious traumatic shock and all had multiple injuries. All 5 cases were operated on within 2--3 hours of the injury. The authors used the following postulates in treating this type of trauma: prevention of traumatic shock, early meticulous primary surgical treatment of the wound, closure of the joint with the drainage of the synovial cavity and periarticular space, strict immobilization, reinacting the extensor

Overuse Injuries of the Knee.

Overuse injuries of the knee represent a broad spectrum of disorders resulting from repetitive mechanical loading and failed tissue adaptation. These conditions commonly affect athletes and physically active individuals and may involve tendons, cartilage, bone, and periarticular soft tissues. Imaging plays a central role in diagnosis, with radiographs and ultrasound providing initial assessment, and MRI offering the highest sensitivity for early and subtle abnormalities. A wide range of entities may be encountered, including extensor mechanism tendinopathies, traction apophysitis in the immatu

Acute sport-related knee injuries.

BACKGROUND: Australia has a high rate of sporting participation. Although this has many benefits, knee injuries are common, particularly in pivoting and contact sports. OBJECTIVE: This article reviews the management of common sport-related knee injuries, including injuries to the collateral and cruciate ligaments, menisci and the extensor mechanism. DISCUSSION: Some sport-related knee injuries can be treated non-operatively with physiotherapy-led rehabilitation. These include the majority of isolated medial collateral ligament, lateral collateral ligament and posterior cruciate ligament inju

Stress Injuries of the Knee.

Stress injuries to the bone and physis of the knee are common in the active adolescent patient and can be broken down into bone stress injuries (BSIs) and chronic physeal stress injuries. BSIs result from prolonged, repetitive bone loading, whereas chronic physeal stress injuries develop from repetitive loading to the apophysis or epiphysis. Most stress injuries of the knee resolve with relative rest but will occasionally need surgical intervention in more severe cases. Early and accurate identification is paramount for optimal management and to avoid long-term consequences.

Multiligamentous injuries and knee dislocations.

Complex capsular ligamentous structures contribute to stability of the knee joint. Simultaneous injury of two or more knee ligaments, aside from concurrent tears involving the anterior cruciate and medial collateral ligaments, is considered to be associated with femorotibial knee dislocations. Proximal tibiofibular joint dislocations are not always easily recognized and may be overlooked or missed. Patellofemoral dislocations can be transient with MR imaging sometimes required to reach the diagnosis. In this article, the authors describe the mechanism of injury, ligamentous disruptions, imagin

Sports injuries of the knee.

In spite of considerable research efforts, knee injuries continue to plague athletes. The rate of occurrence may actually be increasing. Particularly vulnerable to injury because of its anatomic location, the knee sustains a tremendous amount of force during sports. Physicians caring for these patients must be familiar with the mechanism of these injuries. This report aims to familiarize rheumatology physicians with an overview of the anatomy, function, physical examination, and treatment of the various anatomic structures that are commonly injured.

Arthroscopy in acute knee injuries.

The diagnostic value of arthroscopy was evaluated in 148 patients with acute hemarthrosis and/or instability of the knee. The treatment planned after clinical examination was compared with the treatment given after arthroscopy. Seventy-nine per cent of the patients had ligamentous injuries; 59 per cent of tears were combined with other injuries, and 71 per cent were complete ruptures. Stability testing under anesthesia was most inaccurate for the anterior cruciate ligament, with 13 per cent false positive and 30 per cent false negative results. The planned treatment was altered as a consequenc

Knee ligament injuries in children.

Nine children who were less than fourteen years old and had open physes and ligament injuries of the knee were studied. Despite an initial thorough physical and roentgenographic evaluation, the full extent of the lesion was determined only at operation in seven of the nine patients. The intercondylar eminence of the tibia was avulsed in five patients, four of whom had associated collateral ligament injuries and a positive anterior drawer sign. Despite surgical repair, all nine patients demonstrated some degree of postoperative ligament instability. This was increased in those patients in whom

Neurologic and vascular injuries associated with knee ligament injuries.

BACKGROUND: Multiple ligament knee injuries are commonly associated with neurovascular complications such as popliteal artery, common peroneal nerve, and tibial nerve injuries. HYPOTHESIS: While the importance of identifying these complications in association with bicruciate and triligamentous injuries has been well established, it is important to recognize that any biligamentous and some single ligament knee injuries present with neurological and vascular complications as well. STUDY DESIGN: Literature review. CONCLUSION: Popliteal artery injuries require immediate intervention to help pre

Paediatric Knee Injuries: A Narrative Review.

The prevalence of paediatric knee injuries is increasing due to higher participation in competitive sports, larger build and more use of imaging in diagnosis. A thorough history of injury (including mechanism and symptoms of pain and instability), together with a comprehensive examination (presence of an effusion, tender areas, range of movement and joint laxity) will dictate the need for radiological investigations. Magnetic Resonance Imaging (MRI) is indicated in most paediatric patients with a history of injury and an acute knee effusion, which indicates damage to an intra-articular structu

Nerve Injuries in Total Knee Arthroplasty.

Nerve injury is one of the potential complications of total knee arthroplasty. The extent of the injury includes motor and sensory dysfunction, either temporary or permanent. Although the consequences of nerve injury may be dramatic, the probability of occurrence during the course of primary knee arthroplasty is low, around 0.12% to 0.4%. Local dressing removal and knee flexion are imperative, and the initial investigations include ultrasound or MRI and nerve conduction studies. The extent of recovery depends on the type and severity of the initial nerve palsy; however, most patients are expec

Overuse Knee Injuries in Young Athletes.

In brief Because of their skeletal immaturity, children and adolescents are subject to a somewhat different set of overuse knee injuries than are adults. Described here are the diagnosis and treatment for the most common growth-related causes of knee pain in active young people: Osgood-Schlatter disease, Sinding-Larsen-Johansson disease, osteochondritis dissecans, bipartite patella, medial plica syndrome, and discoid meniscus. In most cases these conditions respond to conservative treatment.
