Microchimerism decades after transfusion among combat-injured US veterans from the Vietnam, Korean, and World War II conflicts.

BACKGROUND: Blood transfusion after traumatic injury can result in microchimerism (MC) of donor white cells (WBCs) in the recipient as late as 2 to 3 years postinjury, the longest prospective follow-up to date. The purpose of this study was to determine how long transfusion-associated MC lasts after traumatic injury. STUDY DESIGN AND METHODS: A group of US combat veterans who received transfusions who responded to a recruitment notice was retrospectively evaluated. Their blood was sampled, and MC was assessed by quantitative allele-specific polymerase chain reaction detection of differences a

Possible effects of agent orange and posttraumatic stress disorder on hyperglycemia in Korean veterans from the US-Vietnam war.

This study was conducted to examine whether Korean veterans from the US-Vietnam War who had a diagnosis of type II diabetes mellitus (T2DM) as well as past history of exposure to agent orange (AO) are vulnerable to hyperglycemia when receiving intra-articular corticosteroid injection (IACI) for pain relief. The current study included a total of 49 patients (n = 49) who received an injection of triamcinolone 20 or 40 mg to the shoulder under sonographic guidance or did that of dexamethasone 10 mg or triamcinolone 40 mg combined with dexamethasone 20 mg to the spine under fluoroscopic guidance. Lead, Follow, or Get out of the Way-How Bold Young Surgeons Brought Vascular Surgery into Clinical Practice from the Korean War Battlefield.

The maturation of vascular surgery into widespread clinical practice was accelerated by events that took place in Korea during the conflict of 1950-1953. Early research and anecdotal clinical trials were just then resulting in publication of cases of the successful vascular repairs and replacements. Noncrushing vascular clamps were being developed and limited manufacture begun. The stage was set for a major advance in the treatment of arterial injury, just as war commenced in Korea, which provided a clinical laboratory. When the war on the Korean Peninsula erupted in June 1950, the policy of t

Unstable Expansion: The Development of the Military Medical Evacuation Chain in Korea, 1948-1953.

This research examines the expansion and characteristics of the Korean Army's chain of medical evacuation in 1948-1953. The most important goal of the chain of medical evacuation was to conserve fighting strength, which cannot be achieved only by sending the sick and wounded to the rear for treatment. It was more important to maintain as many mission-capable wounded soldiers on the frontline. Therefore, triage for conserving strength was the priority in the evacuation process, and military doctors conducting triage played a significant role. Focusing on military doctors, this article studies t

Association between exposure to combat-related stress and psychological health in aging men: the Atherosclerosis Risk in Communities (ARIC) Study.

Exposure to military combat has been associated with myriad adverse health effects. Mechanisms are not well understood, but may include negative emotional states. The authors investigated the relationship between combat and trait anger and vital exhaustion among 5,347 middle-aged male participants in the Atherosclerosis Risk in Communities Study. Combat was significantly associated with trait anger, although results were limited to men of the Korean and Vietnam War eras. Among men of the Korean War era, combat- and noncombat veterans, compared to their nonveteran counterparts, reported more tr

[Trade, wars and the venereal disease: VD epidemic and control of Korea in the late nineteenth and early twentieth century].

This paper examines the spread of venereal disease from the Opening of Korea to the early Japanese colonial period. It focuses on the formation and expansion of Japanese settlement in Korea, the influence of wars, and the state control of VD. The Opening of Korea made the foreign settlement, and Japanese licensed prostitution flourished in Japanese settlement. According to the First Annual Report of the Korean Government Hospital (1886) and Gyelimuisa (1887), VD patients occupied 18.3% of outpatients in Jejungwon hospital of Seoul and 8.9% of outpatients in Busan hospital. Waist-to-arm ratio is a simple and effective index for screening the sarcopenic obesity in older adults.

OBJECTIVE: The purpose of this study was to evaluate the diagnostic performance of the waist-to-arm ratio (WAR) for screening the sarcopenic obesity (SO) in older adults. METHODS: A cross-sectional study was conducted among Korean older adults aged over 65 years. Data were extracted from the Korean National Health and Nutrition Examination Survey (KNHANES) 2023 years. Sarcopenia was defined as handgrip strength < 28 kg for males and < 18 kg for females and appendicular skeletal muscle mass index (ASMI) < 7.0 kg/m2 for males and < 5.7 kg/m2 for females. Obesity was defined as waist circumferen

The long-term consequences of war: the experience of World War II.

Seven hundred and thirty-one World War II and Korean War veterans completed a questionnaire about their experiences and their current psychological reactions to the war. Nineteen percent scored above the cut-off points for both the General Health Questionnaire and the (war-related) Impact of Event Scale, demonstrating that, even over 50 years after the event, many veterans still experience problems relating to their war experiences. Psychological distress was in part directly related to particular experiences, but intrusion and avoidance both played an important role as mediating variables. Ot

[Chinese dentists in the War to Resist US Aggression and Aid Korea].

This article reviews the history of the treatment of oral and maxillofacial injuries during the War to Resist US Aggression and Aid Korea from three aspects: dental medical services in the Korean battlefield, oral and maxillofacial plastic surgery in the rear treatment bases in Northeast China, and oral medical services in rear bases across the country. It shows that Chinese dentists were not afraid of sacrifice or difficulties, and had contributed their lives and wisdom to the oral health of Chinese and Korean military and civilians during the War to Resist US Aggression and Aid Korea, as wel

Wound shock: a history of its study and treatment by military surgeons.

The treatment of wounds has received considerable attention from the time of the Trojan War. However, it was not until the American Civil War that shock was described as an entity distinct from the wounds themselves and that efforts were directed at more than just treatment of the wound. The need for fluid resuscitation in the treatment of hemorrhagic shock was first recognized in the Spanish American War, as was the association of sepsis with shock. World War I showed the need for blood in the treatment of "wound shock," a lesson that had to be relearned in World War II through bitter experie

U.S. military enlisted accession mental health screening: history and current practice.

Through the stimulus of war and concerns about neuropsychiatric disability, the U.S. military developed methods to rapidly screen the mental health of World War I and II draftees. Intelligence testing and brief psychiatric screening expanded the accession physical examination and underwent revision to identify only gross mental health disability. Supplemental psychiatric evaluations and written psychological screening tools were abandoned after postwar assessments; they demonstrated poor predictive power in evaluating recruit service capacity for combat environments. Currently, only three ment

The urologic status of the Vietnam war paraplegic: a 15-year prospective followup.

In a 15-year prospective followup of the Vietnam War spinal cord injury patient the corrected death rate was 20 per cent. This rate is not significantly better than the World War II and Korean War veterans 15 years after injury. However, the percentage of renal deaths 15 years after injury in the Vietnam War group is approximately half that of the other 2 groups, which possibly could be owing to better and more successful efforts to maintain a status free of a catheter. These patients were managed during the era of external sphincterotomy, an important therapeutic advancement in the treatment

The Evolution of Reconstructive Microsurgery in Warfare.

War has been a powerful accelerant of innovation in reconstructive surgery. From the storytelling of Greek heroes of war to modern warfare, each conflict expanded on what was surgically possible. The World Wars introduced pedicled flaps and advancements in burn care and skin grafting. The Korean and Vietnam Wars established arterial and venous repair, halving amputation rates and priming the microsurgical revolution by Jacobson, Suarez, Malt, McKhann, and Buncke. Post-9/11 conflicts drove damage control reconstruction, negative pressure wound therapy, and clinical translation of vascularized c

Anaesthetic and other treatments of shell shock: World War I and beyond.

Post-traumatic stress disorder (PTSD) is an important health risk factor for military personnel deployed in modern warfare. In World War I this condition (then known as shell shock or 'neurasthenia') was such a problem that 'forward psychiatry' was begun by French doctors in 1915. Some British doctors tried general anaesthesia as a treatment (ether and chloroform), while others preferred application of electricity. Four British 'forward psychiatric units' were set up in 1917. Hospitals for shell shocked soldiers were also established in Britain, including (for officers) Craiglockhart War Hospi

Changing dogmas: history of development in treatment modalities of traumatic pneumothorax, hemothorax, and posttraumatic empyema thoracis.

Development of treatment modalities for chest wounds and traumatic empyema thoracis is reviewed in the light of war experience. Mortality from thoracic injury was more than 50% before World War I and was about 25% during World War I. It came down to 10% in World War II and was about 5% during the Korean War. It improved further during the Vietnam War, until it ranged at 2% to 4%, where no further improvement could be imagined. Thoracic surgery was born in the field hospitals of World War I. Established drainage methods and standardized anesthesia made thoracotomy a standard procedure in World

Tube thoracostomy: the struggle to the "standard of care".

Tube thoracostomy for thoracic injuries has been standard for only the last 40 years. Its theoretic roots trace back to World War II, where the goal of treatment was restoration of intrathoracic organ function. Thoracentesis was used to evacuate the hemopneumothorax resulting from chest trauma and that compromised pulmonary function. Experience gained in military and civilian hospitals contributed to the development of tube thoracostomy as an alternative treatment for patients with chest trauma. Progress stalled due to technologic problems and unacceptable complications associated with tube th
