A Retrospective Reappraisal of Labetalol's Hemodynamic Effects.

Background: Labetalol is an adrenergic antagonist used to manage blood pressure. Current package labeling and drug databases describe intravenous (IV) labetalol's hemodynamic effects to have an alpha-to-beta potency relationship of 1:7, denoting a predominantly negative cardiotropic effect, which differs from our clinical experience. Objective: The purpose of this study was to describe the hemodynamic effects of IV labetalol in clinical practice and compare those results to official references. Methods: This was a retrospective, observational cohort study of patients undergoing evaluation and

Effects of maternal labetalol on the newborn infant.

Forty eight neonates, born to mothers suffering from pregnancy induced hypertension and receiving labetalol for control of blood pressure, were studied for the possible adverse effects of the drug. These were compared with eighty one neonates matched for gestation and weight and born to mothers with pregnancy induced hypertension treated with drugs other than labetalol. Incidence of birth asphyxia and intrauterine growth retardation (IUGR) in the study population was 10.4 and 22.9%, respectively and in the control group 5 and 19.7%, the difference between two groups was not statistically signi

Usefulness of labetalol in chronic atrial fibrillation.

Beta-adrenergic blocking agents are useful in controlling excessive ventricular rate in chronic atrial fibrillation (AF) but often reduce exercise capacity. To investigate the advantage of labetalol--a unique beta blocker with alpha-blocking property--in chronic AF, 10 patients without underlying structural heart disease were studied with treadmill test, 12-minute walk and 24-hour ambulatory electrocardiographic monitoring. Patients were randomized and crossed over to receive 4 phases of treatment (placebo, digoxin, digoxin with half-dose labetalol, and full-dose labetalol). Exercise durations

Comparison of labetalol and propranolol in treatment of hypertension.

Labetalol 100 mg is equivalent to propranolol 40 mg with regard to antihypertensive effect. The resting heart rate is faster with labetalol than with an equivalent dosage of propranolol. Orthostatic hypotension on a therapeutic dosage of labetalol appears to be unimportant clinically. During one year of labetalol treatment in 18 hypertensive patients, biochemical and haematological monitoring has not disclosed any toxic side effects.

Use of labetalol in moderate to severe hypertension.

An open study was undertaken in 31 patients with moderate to severe hypertension to compare labetalol (a competitive antagonist of both alpha-adrenoreceptors and beta-adrenoreceptors) alone and in combination with a thiazide diuretic, with a combination of thiazide, beta-adrenergic blocking agent and the alpha-adrenoreceptor blocker prazosin. After introduction of labetalol, six patients withdrew because of intolerable side effects. All 25 remaining patients were controlled on diuretic and labetalol. On labetalol alone, 14 patients were controlled, two withdrew because of side effects and nine

Labetalol-induced Peyronie's disease? A case report.

Peyronie's disease (induratio penis plastica) has been observed in a 58-year-old man 8 months after initiation of treatment with the new combined alpha- and beta-blocking agent, labetalol. During the last 2 months before onset of symptoms he had received 2400 mg labetalol daily. He showed no other signs of abnormal fibrous tissue production and the ANF test was negative. Cessation of the drug revealed no improvement. Peyronie's disease has also been observed in relation to treatment with propranolol, practolol and metoprolol and might be due to an impaired balance between alpha- and beta-recep

Treatment of hypertension with labetalol in neurosurgical practice. Influence of labetalol on cerebral perfusion pressure in dogs without and with intracranial mass lesions.

In neurosurgical patients autoregulation of cerebral perfusion is often lost. Therefore, a sudden increase in blood pressure may lead to an increase in cerebral blood flow and cerebral oedema may follow. The influence of labetalol, a new alpha- and beta-adrenoceptor blocking agent, on intracranial pressure and cerebral perfusion pressure was investigated in dogs without and with mass lesions. During hypotension with labetalol the intracranial pressure remained unchanged and the cerebral perfusion pressure decreased to the same extent as mean arterial pressure (30%). Labetalol seems to be suita

Precipitation of sugammadex with nicardipine and labetalol: A laboratory research.

There is a paucity of clinical data about whether sugammadex forms precipitates with other medications. This laboratory experimental study was performed to determine the drugs that produce precipitates with sugammadex. Samples of 1 ml of sugammadex were prepared in transparent cylinders, to which 1 ml of test drugs (rocuronium, neostigmine, glycopyrrolate, atropine, nitroglycerin, dobutamine, dopamine, epinephrine, vasopressin, norepinephrine, phenylephrine, ephedrine, esmolol, nicardipine, and labetalol) was added. The precipitation reaction was observed visually and via light microscope. The

Maternal and fetal effects of labetalol in pregnant ewes.

Labetalol has been advocated to rapidly decrease blood pressure in preeclamptic women and to blunt the hemodynamic response to tracheal intubation. To assess labetalol's actions in a setting of uterine vasoconstriction, saline or labetalol (1 mg/kg) was infused into maternal venous catheters in 12 pregnant ewes receiving continuous maternal intravenous infusions of norepinephrine. Norepinephrine increased maternal mean arterial pressure (MAP) to 120 +/- 4% of control and decreased uterine blood flow to 45 +/- 6% of control. Labetalol, but not saline, altered these parameters: it decreased MAP

Does Labetalol Trigger False Positive Drug Testing Results?

OBJECTIVES: Labetalol, an α- and β-adrenergic antagonist used to treat hypertension in pregnancy has been blamed for causing false-positive amphetamine and methamphetamine results. In this study, we tested 3 concentrations of labetalol prepared with 4 specimen types (urine, plasma, meconium, and umbilical cord tissue), for amphetamine, methamphetamine, and several other drugs with screen and confirmation tests. METHODS: Residual drug-free specimens were pooled. Labetalol hydrochloride dissolved in methanol was used to prepare spikes in triplicate per specimen type (2.7, 50, and 100 μM), which

Intrapartum labetalol for the treatment of maternal and fetal thyrotoxicosis.

Maternal thyrotoxicosis complicates approximately 0.2% of pregnancies. Simultaneous occurrence of maternal and fetal thyrotoxicosis during labor is rare, and control of maternal tachycardia and hypertension, as well as fetal manifestations of thyrotoxicosis, are cornerstones of management. An 18-year-old nulliparous female at 33 weeks gestational age presented in labor with thyrotoxicosis. Fetal tachycardia was present as well. Labetalol therapy resulted in a decrease in maternal pulse and blood pressure, and resolution of fetal tachycardia. Vaginal delivery occurred. Subsequent evaluation dem

The evaluation of labetalol in the treatment of hypertension complicating pregnancy.

1 Eighty-five women with severe hypertension complicating pregnancy were treated with oral labetalol (Trandate). Six of these had a twin pregnancy and 54 had proteinuria. 2 Effective control of the blood pressure was achieved in all but six patients. The maximum dose of labetalol prescribed was 1200 mg daily. There were no significant maternal or foetal side-effects. 3 Foeto-placental function was carefully monitored in all patients. Twenty-four of the 89 infants born alive showed evidence of intra-uterine growth retardation, the highest incidence occurring in the group of patients with essent

Labetalol disposition and concentration-effect relationships during pregnancy.

1 Labetalol pharmacokinetics and concentration-effect relationships following intravenous administration have been studied in 10 women during the last trimester of pregnancies complicated by hypertension, in seven of these women postpartum (when normotensive) and in 10 normotensive female volunteers. 2 The clearance and volume of distribution of labetalol were the same in all three groups. Clearance values (1 min-1) were: pregnant, 2.16 (range 1.54-3.14); postpartum 2.35 (1.56-3.85); controls 1.96 (0.98-2.61). Volumes of distribution (1) were: pregnant 274 (250-547); postpartum 407 (248-701);

Neonatal adrenergic blockade following single dose maternal labetalol administration.

A single 30 mg intravenous dose of labetalol given 20 minutes prior to cesarean delivery at 35 weeks of gestation for severe pregnancy-induced hypertension was associated with symptoms of beta-adrenergic blockade (hypoglycemia, bradycardia, hypotension) in preterm twins. The infants were subsequently found to have therapeutic labetalol concentrations (180 and 150 ng/mL) in umbilical cord blood. The pharmacology of transplacental labetalol is reviewed and potential mechanisms for neonatal beta-adrenergic blockade are discussed.
