Prolonged Second Stage of Labor and Postpartum Pelvic Floor Dysfunction.

IMPORTANCE: Parity and mode of delivery are common risk factors for pelvic floor dysfunction. The prolonged second stage of labor, defined as the time between complete cervical dilation and the expulsion of the fetal head, may be related to these changes and no pooled evidence has been tabulated to seek this association. OBJECTIVE: The aim of the study was to carry on a systematic review on the association between pelvic floor dysfunction symptoms and prolonged second stage of labor (PSSL). STUDY DESIGN: A systematic search was conducted in the databases PubMed, Cochrane Library, Embase, SCO

Cesarean delivery outcomes after a prolonged second stage of labor.

OBJECTIVE: We hypothesized that prolonged second stage of labor increases the incidence of unintentional hysterotomy extensions at cesarean delivery. STUDY DESIGN: A retrospective cohort of term pregnant women who underwent primary cesarean delivery after failed second stage of labor at Stanford University was assessed for hysterotomy extensions and other maternal and neonatal morbidities. Groups included second stage length of 1-3 hours and >4 hours. Data were analyzed with the use of chi-square and Fisher's exact tests. RESULTS: Of the 239 women who were studied, the second stage of labor

Length of the Second Stage of Labor in Women Delivering Twins.

OBJECTIVE: To evaluate the length of the second stage of labor in twin deliveries and to compare the length of the second stage in twin and singleton gestations. METHODS: This is a retrospective cohort study from three large hospitals in Israel. Clinical data were collected from the electronic medical record. The primary outcome was the length of the second stage (the time from documented 10-cm dilation until spontaneous vaginal delivery of the first twin). Multivariable linear regression was used to examine the association of clinical factors with the length of the second stage. The length o

Vibroacoustic stimulation of the fetus entering the second stage of labor.

OBJECTIVE: To evaluate the fetal heart rate (FHR) response to vibroacoustic stimulation of fetuses entering the second stage of labor as a predictor of neonatal outcome. METHODS: Three hundred sixteen cases and 316 controls were studied during the second stage of labor. All cases had vibroacoustic stimulation on entering the second stage of labor using an electronic artificial larynx. For control patients, the artificial larynx was not activated. The patients were stratified into groups based on the quality of FHR response: acceleration (n = 124), acceleration followed by deceleration (n = 12

Maternal half-sitting position in the second stage of labor.

The maternal half-sitting and supine position during the second stage of fullterm labor was compared in 100 women who, after identical opening phases in supine position, randomly delivered in half-sitting (50 degrees, n = 50) or supine position (n = 50). The whole duration of the second stage of labor or the time spent in active pushing did not differ between the groups. Vacuum extraction was needed twice (4%) in the group delivering in half-sitting and six times (12%) in the group delivering in supine position. Vaginal tear occurred in one mother in both groups. Early decelerations in fetal c

Caesarean Section During Second Stage of Labor in a Tertiary Centre.

BACKGROUND: There is an alarming rise in caesarean section leading to increased adverse outcomes for both the mother and fetus when compared with vaginal delivery. Within this increasing caesarean section rate, there is a concerning increase in the rate of second stage caesarean section. This study highlight the feto-maternal outcome of caesarean section in second stage of labour. METHODS: This was a retrospective cohort review of all women with a singleton, cephalic fetus at term delivered by caesarean section in the second stage of labor between April 1, 2013 and March 30, 2017 at Patan Aca

Prolonged second stage of labor is associated with low Apgar score.

There is no consensus on the effects of a prolonged second stage of labor on neonatal outcomes. In this large Swedish population-based cohort study, our objective was to investigate prolonged second stage and risk of low Apgar score at 5 min. All nulliparous women (n = 32,796) delivering a live born singleton infant in cephalic presentation at ≥37 completed weeks after spontaneous onset of labor between 2008 and 2012 in the counties of Stockholm and Gotland were included. Data were obtained from computerized records. Exposure was time from fully retracted cervix until delivery. Logistic regres

Intrauterine pressure during the second stage of labor in obese women.

OBJECTIVE: The perception that obese women have longer labors and a higher frequency of operative delivery because they are "poor pushers" persists despite the absence of objective study. We tested the hypothesis that obese women generate inadequate intrauterine pressure during the second stage of labor. METHODS: Intrauterine pressure was prospectively measured in 71 women during the second stage of labor. Obesity was defined as a body mass index (BMI) greater than 29 (n = 17). A BMI below 25 was normal (n = 40). Women with a BMI between 26 and 29 (n = 14) were considered overweight. All wome

Use of fundal pressure during second-stage labor. A pilot Study.

Fundal pressure is a controversial obstetric technique used by some practitioners in second-stage labor. In this preliminary study, 34 deliveries in which fundal pressure was used to expedite birth were matched with 34 deliveries that occurred spontaneously. Several parameters were compared between the two groups. In the group of women who were delivered with use of fundal pressure, second-stage labor was longer and a higher incidence of third- and fourth-degree perineal lacerations was observed compared with those women who delivered spontaneously. This article discusses the possible reasons

FHR variability and other heart rate observations during second stage labor.

Seventy-four fetal heart rate (FHR) records that were continued to vaginal delivery were selected for study from more than 2000 intrapartum FHR tracings. Thirty-six of the births were associated with neonatal depression and Apgar scores of 3 or less and/or 6 or less at 1 and 5 minutes, respectively; 38 patients exhibited normal Apgar scores (7 and 10 at 1 and 5 minutes, respectively). Twenty minutes of heart rate activity immediately prior to birth was the basis of analysis. All categories of loss of short-term beat-to-beat variability (BBV), both baseline and with decelerations, were observed

Duration of second stage of labor and factors associated with prolonged second stage: A national population-based study in France.

OBJECTIVES: Prolonged second stage of labor could be associated with an increase in maternal and neonatal morbidity. However, in France, no consensual definition of prolonged 2nd stage of labor exists and therefore factors associated with prolonged second stage have not yet been studied. From national population-based data, we aimed at defining the prolonged 2nd stage of labor in France and identifying its associated factors for women with epidural. STUDY DESIGN: From the French national perinatal survey of 2016, we included all women who reached 2nd stage of labor and delivered at term of a
