Obstetric Lacerations: Prevention and Repair.

Obstetric lacerations are a common complication of vaginal delivery. Lacerations can lead to chronic pain and urinary and fecal incontinence. Perineal lacerations are defined by the depth of musculature involved, with fourth-degree lacerations disrupting the anal sphincter and the underlying rectal mucosa and first-degree lacerations having no perineal muscle involvement. Late third-trimester perineal massage can reduce lacerations in primiparous women; perineal support and massage and warm compresses during the second stage of labor can reduce anal sphincter injury. Conservative care of minor

Cervical lacerations: some surprising facts.

OBJECTIVE: Our aim was to calculate the incidence of cervical lacerations after vaginal delivery and to study its associated risk factors. STUDY DESIGN: A retrospective chart review of all patients with a cervical laceration after vaginal delivery during a 5 year period was performed. Their risk factors were studied. A chi2 test and Bonferroni's correction were applied. The relative risk, P values, and confidence intervals were calculated. RESULTS: Thirty-two patients of 16,931 vaginal deliveries had a cervical laceration for an incidence of 0.2%. Cervical cerclage was associated with an 11.

Nonobstetric lacerations of the vagina.

Although obstetrically related trauma remains the most common cause of injury to the female genital tract, trauma of nonobstetric origin is not uncommon. Reports of traumatic injuries to the vagina, especially lacerations, have been infrequent in the literature and offer only a generalized approach to this problem. Severe vaginal lacerations may result in life-threatening blood loss. The authors report their recent experience with treating patients who have this type of trauma, review mechanisms of injury, and provide an organized treatment protocol for the nonobstetric patient with suspected

Repair of obstetric perineal lacerations.

Family physicians who deliver babies must frequently repair perineal lacerations after episiotomy or spontaneous obstetric tears. Effective repair requires a knowledge of perineal anatomy and surgical technique. Perineal lacerations are classified according to their depth. Sequelae of obstetric lacerations include chronic perineal pain, dyspareunia, urinary incontinence, and fecal incontinence. With lacerations involving the anal sphincter complex, particular attention must be given to anatomy and surgical technique because of the high incidence of poor functional outcomes after repair. An ove

Homicidal cardiac lacerations in children.

We report six cases of intentionally inflicted cardiac laceration. The victims ranged in age from 9 weeks to 2 1/2 years. Five victims were girls and in five cases the right atrium was lacerated. The left ventricle was lacerated in the other case. In the three cases with a confession, one victim each was struck with a fist, stomped, and kicked. Four patients had rib fractures, with at least two fractures in each case. Cardiac rupture from blunt trauma most commonly results from compression of the heart between the sternum and vertebral column, but may also occur from compression of the abdomen

Pulmonary laceration.

INTRODUCTION: Pulmonary laceration is usually caused by penetrating trauma but it can also be found in non-penetrating trauma. It usually resolves quickly and with few problems if proper and timely treatment is given for which an adequate diagnosis is mandatory. In some cases it may require a surgical approach. CASE: A 21 year-old male was admitted after a car accident with multiple trauma lesions. He presented with tachycardia and tachypnea. Thorax CT revealed evident pulmonary lacerations. After placing two chest tubes he evolved favorably. CONCLUSION: Since we can usually find different k

Renal Laceration.

In brief A male skateboarder experienced chest wall pain and splinting after striking a curb with the right side of his torso. X-rays revealed a nondisplaced fracture of the right ninth rib. Two days later, however, the patient returned with hematuria, and CT showed a renal laceration. This case illustrates that minor symptoms can accompany serious injury after abdominal trauma. Also, as skateboarding gains popularity, such injuries may become more frequent.

Leech Therapy for Complex Facial Lacerations.

Facial trauma can pose challenging reconstructive obstacles in both maintaining tissue viability and restoring aesthetic appearance. Medicinal leech therapy can help to promote vascular decompression in the setting of venous congestion. A retrospective chart review was conducted to identify patients who underwent medicinal leech therapy following venous stasis secondary to repair of a complex facial laceration. Three patients were identified; 2 suffered auricular avulsion, while 1 suffered a lip avulsion. All patients suffered from venous congestion and underwent medicinal leech therapy for 48

Birthing postures and birth canal lacerations.

OBJECTIVE: This study was performed to assess the differences in the birth canal lacerations following the lateral and fours posture deliveries compared with those following the supine posture deliveries. METHODS: We examined the birth canal lacerations of our "low risk" pregnant women under the midwife-led delivery care at Japanese Red Cross Katsushika Maternity Hospital between April 2006 and March 2015. RESULTS: There were 3826, 1754 and 719 women who delivered with supine, lateral and fours postures. The rate of no laceration in the women who delivered with lateral posture was significan

Flexor tendon lacerations in zone V.

Twenty-three patients with zone V flexor tendon lacerations rehabilitated by the Kleinert protocol were studied at an average of 46 months after trauma. Hand function was subjectively normal in only eight. Of fourteen patients who were employed at the time of injury, eight returned to their original occupations. Eight others were working at other jobs, and we considered six more capable of employment. Only one had a poor functional result that precluded occupational use of the hand. Pinch/grip strengths recovered to 85%/79% of the uninvolved side. Independent flexor digitorum superficialis/fle

Silicone intubation of traumatic canalicular lacerations.

To assist in the repair of canalicular injuries, identification of the medial aspect of the canalicular laceration can easily be accomplished by retrograde probing and intubation of the medial canalicular remnant. A dacryocystotomy along the medial side of the nasolacrimal sac will allow direct visualization of the internal nasolacrimal sac anatomy and common internal punctum of the common canalicular system. A retrograde passage of a modified Quickert probe can then be accomplished through the dacryocystotomy incision into the common internal punctum and present in the traumatic wound, identi

Epikeratophakia in children with corneal lacerations.

Epikeratophakia grafts were used to optically correct aphakia in six children who suffered traumatic cataracts and corneal lacerations which were greater than 3 mm in length. The ages of the patients at the time of corneal laceration ranged from nine months to 3 3/12 years. Five of the six grafts were successful. The average increase in corneal curvature was 16.11 diopters with an average over-refraction of -0.73 diopters. The deprivation interval in the five children with successful grafts ranged from two months to two years, and the average visual acuity for the four verbal patients with suc

Cervical Lacerations: A Review of Risks.

Cervical laceration (CL), although infrequent, is an often-unrecognized complication of vaginal birth and can cause significant blood loss in the immediate postpartum period. The rate of clinically significant CL ranges from 0.14% to 0.2% of births. Nulliparity, operative vaginal birth, occiput posterior position of the fetus, induction of labor, and episiotomy have been cited as possible risk factors. Much of the available literature regarding CL, however, is dated or anecdotal, and there are varying and inconsistent risk associations with its occurrence. Given this unpredictability, CL shoul
