Imaging of pregnant and lactating patients with suspected adrenal disorders.

A high level of clinical suspicion is essential in the diagnosis and management of a suspected adrenal mass during pregnancy and the peripartum period. Timely recognition is important in order to improve fetal and maternal outcomes. Imaging is often performed to confirm a suspected adrenal lesion; however, increasing usage of diagnostic imaging during pregnancy and lactation has also increased awareness, concerns and confusion regarding the safety risks regarding fetal and maternal exposure to radiation and imaging intravenous contrast agents. This may lead to anxiety and avoidance of imaging

Sodium D-lactate infusion of panic disorder patients.

Nineteen patients with panic disorder received sodium D,L-lactate (racemic) and pure sodium D-lactate infusions in a pilot study. Sodium D-lactate, which is less metabolically active than L-lactate, produced panic attacks in half the patients. D-Lactate also produced hypocapnia and alkalosis, indicating hyperventilation. These findings suggest that metabolism of lactate is not necessary for the induction of panic in susceptible patients. D-Lactate appears similar to other agents that cause panic in its capacity to stimulate respiration in the preliminary study.

Lactate in bipolar disorder: A systematic review and meta-analysis.

Bipolar disorder (BD) is a debilitating mood disorder with no specific biological marker. No novel treatment has been developed specifically for BD in the last several decades. Although the pathophysiology of BD remains unclear, there is strong evidence in the literature supporting the role of mitochondrial dysfunction in BD. In this systematic review, we identified and investigated 12 studies that measure lactate, which is a direct marker for mitochondrial dysfunction, in BD patients and healthy controls. Six studies measured lactate levels in the brain through proton echo-planar spectroscopy

Psychopharmacological investigation of panic disorder by means of lactate infusion.

The contribution of research with lactate infusion to an understanding of agoraphobia, generalized anxiety disorder, and panic disorder (PD) was reviewed. Lactate-induced panic seems to differentiate panic disorder from generalized anxiety disorder. Panic disorder seems to have important links to depression; both respond to tricyclic antidepressant drugs and neither responds well to the benzodiazepine antianxiety drugs. Response to pharmacotherapy, epidemiological surveys, and familial studies support the distinction between panic disorder and generalized anxiety disorder and the overlap betwe

Panic disorder: response to sodium lactate and treatment with antidepressants.

Effective drugs for mental disorders have been found by serendipitous findings not supported by knowledge of psychopharmacology. Drug are assigned labels, such as "antidepressant" without knowledge that such a label delimits the utility of such agents. Many double-blind controlled studies have shown that imipramine effectively ameliorates panic attacks and agoraphobia. Epidemiological data show a relationship between Panic Disorder and Depression. Relatives of probands with Major Depression plus an Anxiety Disorder were at greater risk for both Major Depression and for an Anxiety Disorder. Pan

Perinatal physiology and welfare in water buffalo: calving, mother-calf bonding, lactation, and postpartum disorders.

The peripartum period is a critical biological stage characterized by coordinated endocrine, physiological, and behavioral adaptations that determine successful parturition, neonatal survival, and productive efficiency in water buffaloes. This review aims to discuss the changes that buffalo cows show at calving onset, as well as the implications of the mother-calf bond, lactation, and postpartum pathologies, highlighting their relevance to animal welfare and neonate viability. As in other ruminants, parturition is initiated by the activation of the fetal hypothalamic-pituitary-adrenal axis, tr

Disorders of bone and mineral metabolism in pregnancy and lactation: A case based clinical review.

Bone and mineral metabolism in the human body undergoes significant adaptations during pregnancy and lactation to meet the physiological demands of both the mother and fetus. The growing fetus requires approximately 30 g of calcium, with 80% of this transferred from the mother during the third trimester. These adaptations involve complex hormonal changes, such as increased parathyroid hormone-related peptide (PTHrP) and 1,25-dihydroxyvitamin D, ensuring the mother maintains calcium balance despite fetal demands. However, these changes can also exacerbate pre-existing metabolic bone disorders,

The genetic relationship of body weight and early-lactation health disorders in two experimental herds.

The objectives of this study were to estimate genetic parameters for body weight (BW) and BW change (BWC) and genetic correlations of BW and BWC with diseases and genomic predicted transmitting abilities (PTA) of productive and conformation traits of Holsteins during the first 120 DIM. Daily BW data were from the Afiweigh cow body weighing system (SAE Afikim, Kibbutz Afikim, Israel), which records BW as a cow exits the milking parlor. Disease categories included metabolic diseases, ketosis, infectious diseases, mastitis, reproductive diseases, and other diseases. Edited data included 68,914 an

Is the use of botulinum toxin injection for treating neurological disorders safe during pregnancy and lactation?

Botulinum neurotoxin (BoNT) is a mainstay in treating several neurological disorders including in women of reproductive age. This review, based on 43 studies encompassing 658 human cases and supporting animal data, evaluates its safety during pregnancy and lactation. Available evidence is limited and predominantly observational, with most reports describing first-trimester exposure at doses below 200 units. As a result, data on later trimesters and higher doses remain scarce. Notably, the available literature does not show an increased prevalence of fetal losses and malformations, compared to

Specificity of lactate infusions in social phobia versus panic disorders.

Lactate infusions produced panic attacks in one of 15 patients with social phobia, four of nine with agoraphobia, and 10 of 20 with panic disorder in a blind study. The proportion of patients with social phobia who panicked in response to lactate was significantly lower than that of patients with agoraphobia or panic disorder. These findings lend validity to the DSM-III classification of anxiety states. They also suggest that the pathophysiology of social phobia differs from that of disorders characterized by spontaneous panic attacks.

Vasopressin response to lactate infusion in normals and patients with panic disorder.

Infusions of sodium lactate evoke panic symptoms in patients with panic disorder but not in normal subjects. Although plasma vasopressin (AVP) is known to rise in response to other forms of stress, its response during this maneuver has not previously been reported. We measured plasma AVP in double-blind infusions of sodium lactate in 5 normal subjects, 6 patients with panic disorder, and, again in 4 patients after chronic alprazolam. In all groups administered lactate, AVP rose significantly above baseline values (p less than 0.05) though no change was seen in a control group of patients durin

Behavior and productivity of cows milked in automated systems before diagnosis of health disorders in early lactation.

Associations of electronically recorded data were examined before diagnosis of health disorders in early-lactation cows in herds with automated milking systems (AMS). Rumination time, activity, and milk yield data were collected for 8 mo for 605 early-lactation cows in 9 commercial AMS herds. Using multivariable generalized linear regression models controlling for parity and days in milk, data were examined relative to the day of diagnosis for health disorders occurring in absence of, or at least 14 d before, another disorder: mastitis (n = 13), new cases of lameness (n = 45), subclinical keto

Phenotypic relationships of common health disorders in dairy cows to lactation persistency estimated from daily milk weights.

The objective of this study was to investigate the phenotypic relationship between common health disorders in dairy cows and lactation persistency, uncorrelated with 305-d yield. The relationships with peak yield and days in milk (DIM) at peak were also studied. Daily milk weights and treatment incidence records of 991 Holstein lactations from experimental dairy herds at Virginia Tech and Pennsylvania State University were used. Persistency was calculated as a function of daily yield deviations from standard lactation curves, developed separately for first (FL) and later lactations (LL), and d

Arterial blood gas changes in panic disorder and lactate-induced panic.

Lactate infusions were conducted in 12 male panic patients and 8 male normal controls with arterial catheters in place to reassess previously reported acid-base changes based on venous blood samples. The analysis of arterial pH, carbon dioxide pressure, and bicarbonate concentration confirmed most venous findings. At baseline, before the infusion, venous blood shows evidence of mixed chronic and acute respiratory alkalosis in patients while arterial blood gasses are most consistent with developing acute respiratory alkalosis. During the infusion both bloods are consistent with mixed metabolic

Pharmacological Treatment of Attention Deficit Hyperactivity Disorder During Pregnancy and Lactation.

INTRODUCTION: Attention deficit/hyperactivity disorder (ADHD) is a neurobehavioral problem found in 2-5% of adults. Stimulants and drugs that affect the dopaminergic, noradrenergic and/or serotonergic systems are effective treatment and are increasingly prescribed to women at child bearing age. It is consequently important that reliable information on the safety of these drugs in pregnancy is available so that appropriate therapeutic choices can be made. RESULTS: The data on stimulants (methylphenidate and amphetamines) are generally showing that there is no increase in the rate of major cong

Alterations in total lactate dehydrogenase and its isoenzyme-5 in hepatic disorders.

Values for total lactate dehydrogenase (LD, EC 1.1.1.27) and LD isoenzyme-5 were determined in serum of 106 patients with benign hepatic disorders, 54 of whom had acute liver disorders, either acute hepatitis (39 patients) or acute circulatory disturbances (15 patients). Fifty-two had chronic hepatic disorders, either cirrhosis (25 patients) or chronic right heart failure (27 patients). Overall, values for LD were above normal for 86 percent of the 106 patients with benign hepatic disorders. In 83 percent of 30 patients with non-fulminant viral hepatitis, LD values were below 350 U per L, whil

Response to sodium lactate in panic disorder: relationship to presenting clinical variables.

Sodium lactate infusion provokes more physiological and psychological symptoms of panic in patients with panic attacks than in normal controls. The relationship between response to sodium lactate infusion and presenting clinical characteristics was examined in 50 patients with panic disorder or agoraphobia with panic attacks. Lactate-induced panic was significantly related only to a patient-reported family history of panic. Rating of physical symptoms during lactate infusion, but not overall response to lactate, was significantly correlated with Symptom Checklist-90 somatization scores. In gen
