Vascular injuries during lumbar laminectomy.

Vascular injury during lumbar laminectomy is an uncommon event. It is most often secondary to a biting instrument, such as a pituitary rongeur. Its diagnosis must be made rapidly in order to save the patient from exsanguination. This is well within the province of the general surgeon, who should be prepared to treat it at any time.

The assessment of appropriate indications for laminectomy.

We have developed criteria to determine the appropriate indications for lumbar laminectomy, using the standard procedure developed at the RAND corporation and the University of California at Los Angeles (RAND-UCLA). A panel of five surgeons and four physicians individually assessed 1000 hypothetical cases of sciatica, back pain only, symptoms of spinal stenosis, spondylolisthesis, miscellaneous indications or the need for repeat laminectomy. For the first round each member of the panel used a scale ranging from 1 (extremely inappropriate) to 9 (extremely appropriate). After discussion and cond

Clinical comparison between simple laminectomy and laminectomy plus posterior instrumentation in surgical treatment of cervical myelopathy.

BACKGROUND: Posterior stabilization in patients treated with laminectomy for spondylotic cervical myelopathy is still a debate. Despite both being reported in literature by several authors, some controversies still exist. The aim of this study is to compare clinical and radiological outcomes in patients treated with laminectomy or laminectomy with posterior stabilization. MATERIAL AND METHODS: We retrospectively evaluated 42 patients affected by cervical myelopathy (mean age 70.43 ± 5.03 years), 19 treated with laminectomy (group A) and 23 with laminectomy and posterior instrumentation (group

Is bilateral laminectomy and fusion superior to total laminectomy and fusion in two-level lumbar spinal stenosis?

BACKGROUND: This study aims to compare the clinical outcomes of bilateral laminectomy fusion (BLF) with total laminectomy and fusion (TLF) surgeries for patients with two-level lumbar spinal stenosis (LSS). METHODS: A total of 124 eligible patients with lumbar spinal stenosis treated either with BLF or TLF between January 2023 and October 2025 at our hospital were enrolled in this study. Patients were divided into two groups according to their operation methods (BLF group and TLF group). All data were collected from their medical records. Data on clinically related factors, such as the Visual

Differences in spinal cord biomechanics after laminectomy, laminoplasty, and laminectomy with fusion for degenerative cervical myelopathy.

OBJECTIVE: Spinal cord stress/strain during neck motion contributes to spinal cord dysfunction in degenerative cervical myelopathy (DCM), yet the effect of surgery on spinal cord biomechanics is unknown. It is expected that motion-preserving and fusion surgeries for DCM will have distinct effects on spinal cord biomechanics. The aim of this study was to compare changes in spinal cord biomechanics after laminectomy with fusion, laminectomy, and laminoplasty using a patient-specific finite element model (FEM) for DCM. METHODS: A patient-specific FEM of the cervical spine and spinal cord was cre

Minimally invasive robotic-assisted lumbar laminectomy.

AIMS: To report the development of the technique for minimally invasive lumbar decompression using robotic-assisted navigation. METHODS: Robotic planning software was used to map out bone removal for a laminar decompression after registration of CT scan images of one cadaveric specimen. A specialized acorn-shaped bone removal robotic drill was used to complete a robotic lumbar laminectomy. Post-procedure advanced imaging was obtained to compare actual bony decompression to the surgical plan. After confirming accuracy of the technique, a minimally invasive robotic-assisted laminectomy was perf

Cervical Spinal Cord Herniation After Laminectomy.

Following a recent cervical laminectomy, a 67-year-old female patient developed neck pain and torticollis after a coughing episode. On physical examination, manifestations of neck pain, stiffness, and 4/5 quadriparesis were noted. Magnetic resonance imaging findings indicated the presence of herniation of the cervical medulla, accompanied by cerebrospinal fluid leakage. Consequently, surgical intervention was performed to reposition the herniated cord within its original subarachnoid space, thereby averting potential recurrence. The patient experienced complete resolution of symptoms within a

Acute Sciatic Neuritis following Lumbar Laminectomy.

It is commonly accepted that the common cause of acute/chronic pain in the distribution of the lumbosacral nerve roots is the herniation of a lumbar intervertebral disc, unless proven otherwise. The surgical treatment of lumbar disc herniation is successful in radicular pain and prevents or limits neurological damage in the majority of patients. Recurrence of sciatica after a successful disc surgery can be due to many possible etiologies. In the clinical setting we believe that the term sciatica might be associated with inflammation. We report a case of acute sciatic neuritis presented with si

Vascular injury during a lumbar laminectomy.

A lumbar laminectomy is a common and routine operation. Damage to abdominal vascular structures during surgery is a relatively infrequent complication; however, when it does occur, it is sudden and life-threatening. We herein report on 2 cases of abdominal vascular injury which occurred during lumbar microdiscectomies. The first case was a 34-year-old man. A bloody surgical field was noted 45 min into the operation along with an increase in heart rate and a decrease in blood pressure. After fluid resuscitation and an ephedrine injection, his vital signs stabilized. The patient was then sent to

Laminectomy for thoracic spinal canal stenosis.

Stenosis of the thoracic spinal canal is a relatively rare disorder with numerous causes. Clinical manifestations include signs and or symptoms consistent with focal thoracic radiculopathy and/or myelopathy. Several surgical approaches for the decompression of the stenotic thoracic canal have been described. Laminectomy is typically reserved for only those cases in which dorsal compression of the neural elements is demonstrated; it is contraindicated when the epidural compression is primarily ventral in location.

[Postoperative, appositional ossifications after lumbar laminectomy].

QUESTION: Post-laminectomy segmental hypermobility as well as appositional ossification were suggested by many authors to contribute to the unsatisfactory long-term results of laminectomy. The aim of this study was to find out whether segmental instability, among other factors, influences the degree of appositional ossification following laminectomy. METHODS: 55 out of 72 patients operated upon by laminectomy or hemilaminectomy for degenerative lumbar spinal stenosis were examined by radiography after an average follow-up period of 5.2 years. Appositional ossification at the site of surgery w

Decompressive lumbar laminectomy for spinal stenosis.

A total of 258 consecutive decompressive lumbar laminectomies performed on 244 individuals presenting with spinal stenosis were analyzed retrospectively. Spinal fusion was avoided in all but two patients. Outcome in terms of pain relief and return to normal activity was evaluated in two stages, one derived from patient charts and having a relatively short-term follow-up time (mean 8.4 months) and a second derived from patient responses to a questionnaire (which also scored for satisfaction with the results of surgery), which had a longer follow-up time (mean 4.7 years). More than 20 clinical a

[Osteoplastic laminectomy using hydroxyapatite ceramic implants].

Cervical osteoplastic laminectomy using ceramic or apatite beads as implants was performed in 31 patients with symptomatic cervical bony canal stenosis. Patients with degenerative changes such as osteochondrosis or disc herniation are included in this study but those with spinal cord tumor, arachnoid cyst or ossification of the posterior longitudinal ligament are not. Initially foraminotomies were performed at the C5-8 levels in an attempt at preventing worsening of radicular signs caused by posterior displacement of the cord and resultant tethering effect on the nerve root. Major vascular injury during lumbar laminectomy.

Major vascular injury during lumbar laminectomy is an extremely rare complication of one of the commonest surgical procedures performed in orthopaedic and neurosurgical units. Its occurrence may be associated with high morbidity and mortality, particularly if it is not diagnosed in the early stages. Early diagnosis and repair is associated with an improved prognosis in the long term. We illustrate these points with a description of two cases, and discuss the anatomical factors predisposing to these injuries and the importance of angiography in the management and early diagnosis of suspected ca

Anterior spinal tuberculosis: paraplegia following laminectomy.

Of 16 patients with anterior spinal tuberculosis treated in this unit during the period 1979-1984, five presented with neurological complications following laminectomy at another hospital. After the decompressive laminectomy all five patients developed an increasing kyphosis, and four of the five patients had deteriorated neurologically. They were transferred to our unit where the four with neurological deterioration underwent surgical stabilisation. This paper illustrates the dangers of posterior spinal surgery in the presence of anterior disease.
