Stridor in the neonate: laryngomalacia.

Stridor in the neonate is usually congenital in etiology, the most common cause being laryngomalacia. Emergency airway management is always the first priority. The history is highly suggestive of the diagnosis in the majority of cases, but endoscopy is required for confirmation. The possibility of synchronous lesions must not be overlooked. Laryngomalacia presents with stridor usually within the first few weeks of life. The stridor is inspiratory in nature and varies with position and activity level. Anatomic, histologic, and neurologic factors have been proposed as the cause of laryngomalacia…

Epiglottoplasty for the treatment of laryngomalacia.

Laryngomalacia is the most common congenital laryngeal anomaly. Patients present with different degrees of stridor and feeding problems that usually resolve by 18 months of age. A small number present with severe feeding problems, failure to thrive, stridor with cyanosis, and apnea, which may result in cardiopulmonary disease. These infants require surgical intervention, usually a hyomandibulopexy or tracheotomy. We present a new procedure, epiglottoplasty, that is performed endoscopically and involves excision of redundant mucosa over the lateral edges of the epiglottis, aryepiglottic folds, …

Laryngomalacia: Our Clinical Experience.

OBJECTIVE: The aim of this study is to analyse the clinical symptoms, follow-up and treatment properties of the laryngomalacia patients that we encountered between 2009 and 2014. METHODS: Records of 81 laryngomalacia patients who were followed up in our clinic between 2009 and 2014 were retrospectively analysed. Patients' gender, age, time of onset of the symptoms, chief complaints, other co-existing congenital laryngeal anomalies and treatment and follow-up properties were evaluated. RESULTS: Of the 81 patients, 48 were male and 33 were female, and the mean age was 4.9 months. The average p…

[Laryngomalacia--endoscopic microsurgical treatment].

Laryngomalacia is caused by inspiratory collapse of the supraglottic structures in babies. This predominant cause of stridor in the first years of life has a graded occurrence from an insignificant sound phenomenon to insufficient alveolar ventilation with possible serious complications and development of dystrophy. The laryngomalacia must be sub-classified by direct laryngoscopy with a flexible endoscope under spontaneous respiration. Microsurgical techniques for performing supraglottoplasty have recently been introduced. The authors describe symptoms and results in four cases of laryngomalac…

What parents are reading about laryngomalacia: Quality and readability of internet resources on laryngomalacia.

OBJECTIVE: The goal of this study is to measure the quality and readability of websites related to laryngomalacia, and to compare the quality and readability scores for the sites accessed through the most popular search engines. INTRODUCTION: Laryngomalacia is a common diagnosis in children but is often difficult for parents to comprehend. As information available on the internet is unregulated, the quality and readability of this information may vary. METHODS: An advanced search on Google, Yahoo, and Bing was conducted using the terms "laryngomalacia" OR "soft larynx" OR "floppy voice box."…

Synchronous airway lesions in laryngomalacia.

OBJECTIVE: Laryngomalacia is the most common cause of congenital stridor. Laryngomalacia may be associated with other structural and functional airway lesions. While previous studies suggested a 10-45% rate of synchronous airway lesions (SALs), the exact rate and it's clinical significance is unknown. The purpose of this study was to determine the prevalence of SALs below the glottic level in congenital laryngomalacia, and to investigate possible relations with other clinical findings. METHODS: A cohort of 228 infants with congenital stridor who underwent fiberoptic flexible bronchoscopy (FFB…

Endoscopic correction of severe laryngomalacia.

Tracheostomy for management of severe laryngomalacia is associated with significant morbidity and mortality. Two cases are reported wherein the laryngeal abnormality was corrected by ary-epiglottic fold incision and CO2 laser supraglottoplasty. Stridor and respiratory obstruction were relieved and a long term tracheostomy avoided. Endoscopic correction of laryngomalacia offers significant benefits over conventional treatment with tracheostomy in terms of decreased morbidity and improved quality of life.

Neuropathological features of pediatric laryngomalacia.

OBJECTIVE: Laryngomalacia is the most common pediatric laryngeal anomaly. The pathophysiology of laryngomalacia is not well defined; the leading hypothesis suggests weak laryngeal tone and neuromuscular discoordination. Only a few studies explored the histopathology of the laryngeal submucosal nerves, with reported nerve hypertrophy. Our study aims to describe the histopathology of submucosal nerves in specimens obtained from children with severe laryngomalacia compared to pediatric cadaveric controls. STUDY DESIGN: Prospective study. SETTINGS: Tertiary care children's hospital. METHODS: Hi…

[Laryngomalacia treated with CO2 laser].

Laryngomalacia is the most common laryngeal anomaly which causes inspiratory stridor in newborns. The disease is usually self-limiting and resolves before the age of two years. We present a case of severe laryngomalacia with feeding disorder and airway obstruction which needed surgical management--supraglottoplasty. The shortened aryepiglottic folds were incised using CO(2) laser and jet ventilation. The patient was observed at the hospital for one week after surgery and discharged. Four weeks after treatment, the patient was free of airway obstruction and feeding problems.

Endoscopic laser for severe laryngomalacia.

Stridor in the pediatric age group needs detailed evaluation. Laryngomalacia, the commonest cause of stridor is mostly benign, but in about 10% patients can be an important cause of morbidity and mortality. Laser surgical correction in patients with severe laryngomalacia gives good results. We evaluated 32 patients of stridor. All were screened with fibreoptic laryngoscopy and whenever indicated, direct endoscopy was carried out. 13 (40%) of the patients had laryngomalacia. Of these, 8 had severe laryngomalacia and underwent treatment with diode laser. All of them showed definite post procedur…

Microdebrider-assisted supraglottoplasty for laryngomalacia.

OBJECTIVES: We describe our series in the surgical treatment of laryngomalacia using a microdebrider. METHODS: We performed a retrospective review of patients who underwent microdebrider-assisted supraglottoplasty for laryngomalacia between October 2004 and February 2008. Patients with neurologic conditions and secondary airway lesions were excluded. The main outcome measures included complications, pain, resolution of stridor, presence of aspiration, and need for revision surgery. RESULTS: Twenty-eight patients underwent microdebrider-assisted supraglottoplasty. The mean age at diagnosis wa…

Waardenburg syndrome associated with laryngomalacia.

Waardenburg syndrome (WS) is a rare autosomal dominant condition characterised by sensorineural hearing loss, in conjunction with pigmentary abnormalities and defects of the neural crest-derived tissues. Depending on the additional phenotypic characteristics, WS is classified into four types, viz. WS1, WS2, WS3 and WS4. We report a 45-day-old male infant with WS1, who presented with inspiratory stridor associated with difficulty in respiration. Direct flexible laryngoscopic examination during evaluation confirmed laryngomalacia as the cause of the symptoms. The baby was managed conservatively …

[Laryngomalacia. Definition, diagnosis and therapy].

Laryngomalacia, a rarely mentioned clinical entity in the German literature, is reviewed in this article. The clinical picture, diagnostics and pathogenesis are presented. The prognosis is favorable. Usually the inspiratory stridor disappears after the second year of life in most of the children. However, 7-22% of those involved develop life threatening symptoms which require surgical intervention. We performed laser supraglottoplasty, the method of choice, on five children with severe laryngomalacia. Intraoperative pre and post surgical findings and the surgical procedure are documented. The …

Laryngomalacia: a proposed classification form.

Laryngomalacia is a common cause of stridor in children. The disorder has a heterogenous presentation, from the mildest form, which resolves with maturation, to the most severe form, requiring tracheotomy. While there is a vast literature on the subject, there is neither stratification nor correlation of clinical presentation, endoscopic appearance, treatment and outcome. In order to statistically evaluate the choice of treatment based upon presentation, patients must first be classified by relevant predictors of disease severity. A form is proposed to classify the clinical presentation of lar…

Outcome in congenital stridor (laryngomalacia).

Twenty-one children who had been diagnosed as having laryngomalacia by direct laryngoscopy in infancy were reviewed 7 to 12 years later. The natural history of the disease is documented. A wide variation in the time of onset and duration of the stridor was found and there was a high incidence of feeding difficulties. A previously reported association with mental retardation or cerebral palsy is not confirmed. 4 out of the 21 children had early speech problems.

Supraglottoplasty outcomes in laryngomalacia in children.

PURPOSE OF REVIEW: This review evaluates current evidence on outcomes following supraglottoplasty in children with laryngomalacia. Emphasis is placed on surgical indications, perioperative care, and short-term and long-term functional outcomes. RECENT FINDINGS: Contemporary studies confirm that supraglottoplasty significantly improves respiratory obstruction, feeding, and sleep-related symptoms for moderate-to-severe laryngomalacia. Patient selection remains critical, with prematurity, neurologic comorbidities, and swallowing dysfunction associated with higher revision and complication rates.…

A new classification scheme for laryngomalacia.

OBJECTIVES: To propose a novel classification method for assessing the severity of laryngomalacia (LM). MATERIALS AND METHODS: During a three-year period, all paediatric patients diagnosed with laryngomalacia were included. A disease-specific questionnaire consisting of eleven questions with four possible responses was administered to gather patient information, and a scoring system was utilized to classify the severity of their condition based on medical history and examination findings. Patients were assigned to two groups: group 1 included those having LM with no or mild co morbidities, gr…
