Injection laryngoplasty under ultrasonographic control.

BACKGROUND: Cervical ultrasound allows real-time visualization of the laryngeal structures. We describe its application for injection laryngoplasty in the voice clinic. METHODS: Hyaluronic acid infiltration of the vocal fold under ultrasonographic control. CONCLUSIONS: With this technique, the position of the needle in the depth of the vocal fold and its spatial relations with nearby structures can be assessed. This allows for an accurate infiltration of the material in contrast to other techniques and avoids discomfort related to the gag reflex with nasofibrolaryngoscopy. No specific compli

The use of polydimethylsiloxane for injection laryngoplasty.

BACKGROUND: Injuries of the recurrent laryngeal nerve with consecutive vocal cord paralysis is a typical complication in chest, esophageal, thyroideal, and neck surgery. Glottic insufficiency secondary to such a lesion can be treated by endolaryngeal vocal cord augmentation (injection laryngoplasty). Many different substances have been used, often showing complications or disadvantages. This study reports on the use of injectable polydimethylsiloxane (PDMS), with special regard to the long-term results. METHODS: In this prospective study, 21 patients with unilateral vocal cord paralysis under

Augmentation laryngoplasty: Results and decision plan.

OBJECTIVES: Augmentation laryngoplasty represents a therapeutic choice for patients who suffer from glottic insufficiency of different etiologies. There have been multiple injectables that have been proven effective. The present study examines the short-term effects of vocal fold augmentation on phonation, swallowing, and breathing in individuals with glottic insufficiency. In addition, a decision plan is also proposed. METHODS: Data from medical records and operative notes were used to conduct a retrospective cohort study on patients with glottic insufficiency who underwent vocal fold augmen

Device Failure During Injection Medialization Laryngoplasty.

OBJECTIVES: Injection medialization laryngoplasty is a commonly performed procedure for the management of glottic insufficiency. Among complications of this procedure is device failure, for which the literature is scarce. Our goal was to determine the prevalence of needle failure during injection laryngoplasty among members of the American Bronchoesophagological Association (ABEA). METHODS: A questionnaire was designed and subsequently sent to members of ABEA via electronic mail. Responses were analyzed using descriptive statistics. RESULTS: Twenty-four members (6.7%) completed the survey. E

Intraoperative computed tomography imaging for laryngoplasty.

OBJECTIVES: Intraoperative cone beam computed tomography (CBCT) imaging has the potential to facilitate the surgical procedure. The current preliminary retrospective chart review investigated the benefits of intraoperative CBCT during laryngoplasty. METHOD: This study examined 26 cases that underwent intraoperative CBCT imaging during laryngoplasty, with one patient who counted twice due to first and revision surgery. The visual quality of structures of interest (glottal shape, thyroid cartilage, arytenoid cartilage, and implants) was determined using intraoperative CBCT during laryngoplasty.

Characterisation of palatal dysfunction after laryngoplasty.

REASONS FOR PERFORMING STUDY: Dorsal displacement of the soft palate (DDSP) in the horse has been previously described as intermittent, typically occurring at fast exercise; or persistent, seen at rest. Dorsal displacement of the soft palate has recently been reported following laryngoplasty (LP) and can be associated with continued poor performance and respiratory noise. OBJECTIVES: The current study aimed to characterise the DDSP diagnosed post LP. STUDY DESIGN: Cross-sectional study. METHODS: Owners/trainers of horses undergoing LP at one institution over 6 years were contacted to determ

Needle Fracture During Injection Medialization Laryngoplasty.

OBJECTIVES: Awake injection medialization laryngoplasty is one of the most common therapeutic procedures done by laryngologists in the office or at the bedside. Complications of injection needle fracture are rarely reported. METHODS: This is a case report of a 59-year-old male inpatient who developed left vocal fold immobility with significant glottic insufficiency after pneumonectomy for a large left-sided lung cancer. During bedside injection medialization using thyrohyoid approach, the 25 G needle fractured at the hub and was embedded partly in pre-epiglottic space and partly extending ove

Managing voice impairment after injection laryngoplasty.

To date, there is a paucity of literature on the management of suboptimal voice outcomes after injection laryngoplasty. We present three cases of worsened voice quality and impaired mucosal waveform propagation on videostroboscopy after calcium hydroxylapatite (CaHA) injection. The first was found to have superficial deposits of CaHA in Reinke's space. The second case appeared to have overaugmentation of the vocal folds. The third case had atrophic vocal folds, and despite having a deep CaHA injection (within the thyroarytenoid muscles), the injected vocal folds had diminished mucosal waveform

Bioplastique injection laryngoplasty: voice performance outcome.

AIM: To compare voice performance following Bioplastique injection with that following Isshiki thyroplasty. MEASURES: A 12 item, self-reported voice performance questionnaire was completed and observer-rated perceptual voice analysis scores were also measured, before and after Bioplastique injection, for 14 patients. Results were compared with our previously reported outcomes for 28 thyroplasty patients. RESULTS: Only 14 of 30 patients had complete datasets. For these patients, the mean pre-operative voice performance questionnaire score improved significantly, from 40.3 to 18.9 (p=0.002, Wi

Polydimethylsiloxane particles for permanent injection laryngoplasty.

OBJECTIVES: Polydimethylsiloxane (PDMS) particles are a nonresorbable material that allows for permanent vocal fold augmentation. This study investigated morbidity and voice quality in patients treated for unilateral vocal fold paralysis by injection of PDMS particles. METHODS: Fourteen patients who had neurogenic unilateral vocal fold paralysis of different causes were included in this prospective study. Each patient underwent videostroboscopic assessment before and after operation. Friedrich's dysphonia index (DI), a score system combining subjective and objective parameters, was used to de

Ultrasonographic findings post laryngoplasty in the horse.

Laryngoplasty is commonly used to treat laryngeal hemiplegia in Thoroughbred racehorses. Evaluation of the success of the laryngoplasty is traditionally determined using endoscopy. Laryngeal ultrasonography and normal ultrasonographic appearance have been reported in the standing horse, but post-laryngoplasty and ventriculectomy ultrasonographic evaluation has limited literature coverage. A prospective case series of 10 Thoroughbred racehorses with left laryngeal hemiplegia was examined ultrasonographically and endoscopically prior to 3-10 days, 30-50 days, and 6-12 months after laryngoplasty

A Novel Approach to the Challenging Injection Laryngoplasty.

INTRODUCTION: Injection laryngoplasty (IL) is typically performed either awake or under general anesthesia with paralysis. There is a subgroup of patients, however, for whom neither of these is a good option. For such patients, we report a hybrid technique that allows for a percutaneous injection without paralysis but with sedation to increase patient tolerance. Supraglottic airway laryngotracheal intervention (SALTI) brings the benefits of injection laryngoplasty to a group of patients previously underserved. METHODS: Injection laryngoplasty cases using SALTI technique from July 1, 2013, to
