Late-Onset Bipolar Disorder: Considerations for Diagnosis and Treatment.

Bipolar I disorder is characterized by the presence of at least one manic episode (DSM-5). Despite a decent percentage of individuals being diagnosed later in life, there currently exist no formal treatment guidelines for late-onset bipolar disorder (LOBD), which remains poorly understood. Typically, manic or manic-like episodes in elderly individuals can be thought of as arising from a secondary, physical cause. However, in the absence of a pre-existing neurological disorder - and when laboratory, imaging, and exam findings do not fully support a neurological picture - the determination of a

Phenotypic cognitive impairment in late-onset delusional disorder.

BACKGROUND: Previous use of heterogeneous diagnostic criteria and insensitive cognitive measures has impeded clarification of the extent and type of cognitive impairment specific to late-onset delusional disorder. We examined whether clinical presentations of late-onset delusional disorder are associated with prodromal or established dementia, and whether it might be a discrete clinical syndrome characterized by its own profile of cognitive impairment. METHOD: Nineteen patients with late-onset delusional disorder from a hospital psychiatric service and 20 patients with dementia of the Alzheim

Late-onset bipolar disorder: a case for careful appraisal.

Bipolar disorder is a chronic mental illness with a profound personal and public health costs attached to it. The majority of patients suffering from bipolar disorder have an onset prior to the fifth decade of their lives. However, a significant number of patients have onset of illness after age 50, commonly referred to as late-onset bipolar disorder. Age of onset can have a significant impact on the nature and course of bipolar illness. We present an interesting case of late-onset bipolar disorder while evaluating secondary mania. The patient was a 76-year-old African American woman who prese

Late-onset mitochondrial disorder with electromyographic evidence of myotonia.

We describe a patient with chronic progressive external ophthalmoplegia (CPEO) due to a deletion of mitochondrial DNA (mtDNA) who had electromyographic evidence of myotonic discharges. Myotonia has not previously been described in association with mitochondrial disease and this report extends the known phenotypic expression of these disorders.

Early- versus late-onset bipolar II disorder.

OBJECTIVE: To compare the clinical features and the outcome between patients with early- and late-onset bipolar II disorder. DESIGN: Case series. SETTING: Outpatient private practice. PATIENTS: One hundred and seventy-nine consecutive outpatients with bipolar II disorder presenting for treatment of a major depressive episode. OUTCOME MEASURES: Duration of illness, severity of depression, recurrences, psychosis, chronicity, atypical features and comorbidity. RESULTS: Patients with early-onset (before 20, 25 or 30 years of age) bipolar II disorder had a significantly longer duration of illn

Late onset eating disorders in Spain: clinical characteristics and therapeutic implications.

OBJECTIVE: The literature on later age of onset (LAO) in women with eating disorders is scarce. We compared the severity of eating disorders, eating disorder subtype, and personality profiles in a clinical sample of consecutively assessed women with eating disorders with later age of onset (LAO, > = 25 years) to women with typical age of onset (TAO, <25 years). METHOD: All eating disorder patients met the Diagnostic and Statistical Manual of Mental Disorders Fourth Edition (DSM-IV) criteria and were admitted to the Eating Disorder Unit of the University Hospital of Bellvitge in Barcelona, Spa

Pituitary-adrenal axis hormones in early-onset versus late-onset panic disorder.

OBJECTIVE: It has been hypothesised that early-onset panic disorder (PD) may constitute a biologically distinct subtype of PD, but the few relevant data are inconclusive. We systematically explored for potential psychopathological and hormonal differences between early-onset (age at onset ≤ 27 years) versus late-onset PD, in consecutively-referred, medication-free, acutely-ill PD outpatients, moreover without comorbid mental disorders except agoraphobia (N = 54; age = 32.3 ± 7.5 years; early-onset = 27; females = 38). METHODS: Hormones assessed (plasma levels) included adrenocorticotropic hor

Monozygotic twins with early-onset schizophrenia and late-onset bipolar disorder: a case report.

INTRODUCTION: Schizophrenia and bipolar disorder are generally considered to be distinct illnesses. One piece of evidence supporting their distinctness is the rarity of schizophrenia and bipolar disorder occurring in monozygotic co‒twins. CASE PRESENTATION: We describe a well-characterized pair of African American, female, monozygotic twins assessed at 53 years of age.Case 1: Twin A developed psychotic symptoms at age 23. She was hospitalized and diagnosed with schizophrenia. Twin A was subsequently hospitalized several more times and was consistently diagnosed as suffering from schizophrenia

Electroconvulsive Therapy for Treatment-Resistant Late-Onset Bipolar Disorder.

This paper presents the case of a woman with a psychiatric history of major depressive disorder and generalized anxiety disorder, who experienced her first manic episode at age 50, likely triggered by occupational stress. She was diagnosed with bipolar I disorder and underwent multiple trials of antipsychotic and mood-stabilizing medications, including cariprazine, olanzapine, lumateperone, lithium, and valproate. Despite several pharmacological interventions, the patient continued to exhibit mood instability, frequent panic attacks, and functional decline. Following a course of 12 electroconv

Multisystem disorder in late-onset chronic progressive external ophthalmoplegia.

INTRODUCTION: Chronic progressive external ophthalmoplegia (CPEO) is a mitochondrial syndrome on a disease spectrum with Kearns-Sayre syndrome (KSS). Clinical presentation is variable and our experience suggested that phenotypic differences exist in CPEO with onset after age 20. METHODS: This descriptive study is a retrospective chart review of 40 patients with late-onset CPEO. Clinical features, laboratory and neurophysiology results were reviewed. RESULTS: Multisystem dysfunction was very common in this series. Gastrointestinal dysfunction was more common than expected (60%) as was migrain

Late onset white matter disease in peroxisome biogenesis disorder.

OBJECTIVE: To report late onset cerebral white matter disease as a distinctive phenotype in peroxisome biogenesis disorder (PBD). BACKGROUND: There is phenotypic and genetic overlap among the PBD known as Zellweger syndrome (ZS), infantile Refsum disease (IRD), and neonatal adrenoleukodystrophy (NALD). Distinctive external features are variable among these three disorders, and neurologic deficit has its onset at birth or in infancy. In a structured follow-up cohort of 25 patients with PBD, not including ZS, three patients had an unusual pattern of cerebral white matter disease with onset past

Neuropsychiatric manifestations in late-onset urea cycle disorder patients.

Inherited urea cycle disorders represent one of the most common groups of inborn errors of metabolism. Late-onset urea cycle disorders caused by partial enzyme deficiencies may present with unexpected clinical phenotypes. We report 9 patients followed up in our hospital presenting late-onset urea cycle disorders who initially manifested neuropsychiatric/neurodevelopmental symptoms (the most prevalent neuropsychiatric/neurodevelopmental diagnoses were mental retardation, attention-deficit hyperactivity disorder [ADHD], language disorder, and delirium). Generally, these clinical pictures did not

Does extracranial arterial pathology play a role in late-onset psychiatric disorders?

INTRODUCTION: Carotid interventions are commonly performed to reduce the risk of disabling stroke and stroke-related death. Carotid interventions may also have an effect on cognitive functions. Mental disorders in the elderly have been related to structural brain pathology. This review of literature was performed to test the hypothesis that atherosclerotic lesions in the carotid and vertebral arteries may have an impact on the development and treatment of late-onset psychiatric disorders. METHODS: A literature search identified articles focusing on the key issues using the Pubmed and Cochrane

Size bias of fragile X premutation alleles in late-onset movement disorders.

BACKGROUND: Fragile X-associated tremor/ataxia syndrome (FXTAS), caused by premutation expansions (55-200 CGG repeats) of the FMR1 gene, shares clinical features with other movement disorders, particularly in the domains of gait ataxia, intention tremor and parkinsonism. However, the prevalence of FXTAS within other diagnostic categories is not well defined. METHODS: A meta-analysis was conducted of all published (n = 14) genetic screens for expanded FMR1 alleles to assess the prevalence and CGG-repeat size bias of FMR1 premutation alleles in those populations. RESULTS: In men with late-onse

Psychosis in late life. Spotting new-onset disorders in your elderly patients.

Psychotic disorders usually develop by middle age, but initial onset can be later in life. Delusions, hallucinations, and other psychotic symptoms may accompany medical conditions, mood disorders, and Alzheimer's disease, thereby confusing the diagnosis. In this review, Dr Harris describes some of the more common psychotic disorders in elderly patients and briefly discusses the newest options in drug therapy.

[Mania in an elderly subject: I. -- manic-depressive disorders of late onset].

Late mania may inaugurate a late-onset bipolar illness. Atypical features and organic brain abnormalities are frequently associated. Late-onset bipolar illness is apparently characterized by a strong instability of mood and a super or subsensitivity to neuroleptics and antidepressant drugs. However, there is a strong efficacy of lithium salts and lithium may be used as a curative and preventive treatment or as a therapeutic test, especially in atypical cases. The occurrence of mania in elderly people involves a specifical etiologic, therapeutic and diagnostic approach.

Spectrum of Movement Disorders of Late-Onset Niemann-Pick Disease Type C.

Niemann-Pick disease type C (NPC), is a rare lysosomal storage disorder, which has a variable presentation based on the age of onset. We describe five adult/adolescent-onset NPC cases presenting with a range of movement disorders along with vertical supranuclear gaze palsy as part of the clinical presentation. A diagnostic delay of 4-17 years from the symptom onset was found in this case series. A high index of clinical suspicion in adult/adolescent patients presenting with vertical supranuclear gaze palsy along with various movement disorder phenomenology can help in the early diagnosis of NP
