Development of a collaborative learning health system data mart in pediatric liver transplantation.

Pediatric liver transplant is a lifesaving intervention for children with disparate pathology ranging from single-gene defects to global liver dysfunction and complications from progressive cirrhosis and portal hypertension. The Starzl Network for Excellence in Pediatric Transplantation (SNEPT), a novel learning health system dedicated to pediatric liver transplant, has previously identified practice variability, waitlist mortality, perioperative complications, and inadequate quality initiative implementation as critical barriers that deserve prioritization in the field. This project was a nov

Embedded Research in the Learning Healthcare System: Ongoing Challenges and Recommendations for Researchers, Clinicians, and Health System Leaders.

Embedded research is an innovative means to improve performance in the learning healthcare system (LHS). However, few descriptions of successful embedded research programs have been published. In this perspective, we describe the Care Improvement Research Team, a mature partnership between researchers and clinicians at Kaiser Permanente Southern California. The program supports a core team of researchers and staff with dedicated resources to partner with health system leaders and practicing clinicians, using diverse methods to identify and rectify gaps in clinical practice. For example, recent

A method for testing health system resilience: Development, application and lessons learned.

BACKGROUND: Recent adverse events, such as the COVID-19 pandemic, economic crises, conflict, migration and extreme weather have demonstrated how a wide range of shocks can challenge health systems. While emergency preparedness and planning is a well established component of health system governance in many countries, no methodology has existed to systematically test the resilience of a health system to a broad range of shocks. OBJECTIVE: To share lessons learned from conducing resilience testing exercises. METHODS: The resilience testing methodology is a mixed-method country-led exercise des

Stroke Learning Health Systems: A Topical Narrative Review With Case Examples.

To our knowledge, the adoption of Learning Health System (LHS) concepts or approaches for improving stroke care, patient outcomes, and value have not previously been summarized. This topical review provides a summary of the published evidence about LHSs applied to stroke, and case examples applied to different aspects of stroke care from high and low-to-middle income countries. Our attempt to systematically identify the relevant literature and obtain real-world examples demonstrated the dissemination gaps, the lack of learning and action for many of the related LHS concepts across the continuu

Anthrax attacks and practice patterns: a learning opportunity for health care systems.

Sudden and unexpected events directly influencing clinical practice patterns are uncommon. After the first report of bioterrorism-related anthrax, the authors studied retrospectively 13 months of anthrax-related antibiotic prescription rates for Veterans Affairs outpatients in one urban area where no cases of anthrax were reported. During the 26 days after the first anthrax report, the rate of acute respiratory illnesses treated with fluoroquinolones was 62.8 per 10,000 outpatient visits, an increase of 41 percent over the rate of 44.4 observed approximately one year earlier (p = 0.058). Acute

Advancing the application of systems thinking in health: sustainability evaluation as learning and sense-making in a complex urban health system in Northern Bangladesh.

BACKGROUND: Starting in 1999, Concern Worldwide Inc. (Concern) worked with two Bangladeshi municipal health departments to support delivery of maternal and child health preventive services. A mid-term evaluation identified sustainability challenges. Concern relied on systems thinking implicitly to re-prioritize sustainability, but stakeholders also required a method, an explicit set of processes, to guide their decisions and choices during and after the project. METHODS: Concern chose the Sustainability Framework method to generate creative thinking from stakeholders, create a common vision,

How has Guinea learnt from the response to outbreaks? A learning health system analysis.

INTRODUCTION: Learning is a key attribute of a resilient health system and, therefore, is central to health system strengthening. The main objective of this study was to analyse how Guinea's health system has learnt from the response to outbreaks between 2014 and 2021. METHODS: We used a retrospective longitudinal single embedded case study design, applying the framework conceptualised by Sheikh and Abimbola for analysing learning health systems. Data were collected employing a mixed methods systematic review carried out in March 2022 and an online survey conducted in April 2022. RESULTS: Th

Towards a justice-based reimagination of digital health research and innovation: actionable considerations for learning health systems.

The need to embed decolonising methodologies in health-care practice has been recognised globally. The term decolonising health, which has become a topic of increasing public and academic discourse since 2021, refers to the dismantling of colonial systems of dominance and oppression ingrained within health systems and structures. Despite the growing recognition of the need for decolonisation in broader health research and practice, the application of a decolonial lens to the rapidly evolving field of digital health remains largely underexplored. An active commitment to decolonisation should be

Learning health systems: Driving real-world impact in mental health and substance use disorder research.

The Veterans Health Administration (VHA), under the U.S. Department of Veterans Affairs (VA), is one of the largest single providers of health care in the U.S. VA supports an embedded research program that addresses VA clinical priorities in close partnership with operations leaders, which is a hallmark of a Learning Health System (LHS). Using the LHS framework, we describe current VA research initiatives in mental health and substance use disorders that rigorously evaluate national programs and policies designed to reduce the risk of suicide and opioid use disorder (data to knowledge); test i

Interpretable machine learning identifies health system levers for survival outcomes of patients with prostate cancer.

BACKGROUND: Advances in the care of patients with prostate cancer across the disease spectrum have resulted in substantial survival gains over recent decades. However, global disparities in prostate cancer survival, stage at diagnosis, and access to timely diagnosis and treatment remain among the greatest across all malignancies. We aimed to quantify country-level health system correlates of the prostate cancer mortality-to-incidence ratio (MIR). METHODS: We applied interpretable machine learning using CatBoost with SHAP (SHapley Additive exPlanations) to all 185 countries with prostate cance

An AI-based intelligent diagnosis system for adolescent mental health based on multitask deep learning.

BACKGROUND AND OBJECTIVES: Adolescent depression and anxiety are becoming increasingly prevalent in China, with rates reaching 20%-30%, driven largely by intense academic pressure and the cultural tendency toward somatization. Traditional screening tools, such as the Patient Health Questionnaire-9 (PHQ-9) and Generalized Anxiety Disorder-7 (GAD-7), often suffer from subjective bias, recall errors, and underreporting due to social stigma. This study developed an AI-based intelligent diagnosis system (IDS) using multitask deep learning to non-intrusively predict comorbid depression and anxiety s

Harnessing Potential: A Systematic Investigation of How and Why Learning Happens in a Health System.

BACKGROUND: The ability to learn is central to innovation and performance across industries, and it is the hallmark feature of the rapidly growing movement toward Learning Health Systems. While learning drivers-such as experimentation, risk-taking, participative decision-making, dialogue, and interaction with the external environment-are well defined, they have received little attention in healthcare contexts. AIM: This study explored the nature of organizational learning within Trillium Health Partners (THP), one of Canada's largest community-based teaching hospital systems. METHODS: Formal

Enabling a Learning Public Health System: Enhanced Surveillance of HIV and Other Sexually Transmitted Infections.

Sexually transmitted infections (STIs) continue to pose a substantial public health challenge in the United States (US). Surveillance, a cornerstone of disease control and prevention, can be strengthened to promote more timely, efficient, and equitable practices by incorporating health information exchange (HIE) and other large-scale health data sources into reporting. New York City patient-level electronic health record data between January 1, 2018 and June 30, 2023 were obtained from Healthix, the largest US public HIE. Healthix data were linked to neighborhood-level information from the Ame
