Leiomyomatosis peritonealis disseminata is a rare disease. Approximately 50 cases have been reported in the world literature so far. Origination from the Müllerian epithelium, which is distributed throughout the subperitoneal mesenchyme seems possible in histogenesis. By specific hormonal stimulation and individual predisposition proliferation takes place along the lines of myofibroblastic differentiation. The case of a 41 year-old woman is presented. The patient's history revealed hormonal therapy for dysmenorrhoea. An incidental finding during diagnostic laparotomy for suspicious endometriot…

Peritoneal leiomyomatosis was discovered in a 33 year old woman. The tumor was sited at the uterine isthmus and another had developed in the broad ligament. No recurrence was noted at follow up after one year after limited surgical resection of these two tumors. A review of the literature revealed 45 cases of disseminated peritoneal leiomyomatosis. The tumor nodules varied considerably in size, number and topography. Their structure was composed of collagen fibres and smooth muscle cells. Their development might be the result of a metaplastic process favored by high levels of estrogen. No spec…

Eight observations of diffuse pulmonary leiomyomatosis in women were studied. This disease in a number of cases was combined with leiomyoma of the uterus. The clinical picture depends to a certain extent on which structural part of the pulmonary tissue is the predominant source of tumor growth. Disorders in the lymphatic system causes chylous pneumothorax. Hyperplasia of smooth muscle fibers occurs multicentrically: in interalveolar septae, along the small bronchi and bronchioli, in the walls of small blood and lymph vessels, intrathoracic lymph nodes. Leiomyomatosis of the lungs is of neoplas…

The ninth documented case of leiomyomatosis peritonealis disseminata (LPD) combined with pregnancy is presented. Light and electron microscopic studies revealed that the smaller tumors were composed partly of decidual cells and partly of fibroblasts, and the larger tumors contained cells resembling smooth muscle cells. Plasma steroid levels were determined during pregnancy. Estrogen levels were within normal range, but progesterone levels were lower than normal, resulting in relative hyperestrogenism. In the case presented, pregnancy, especially with relative hyperestrogenism, may have been a …

Disseminated peritoneal leiomyomatosis is an unusual condition characterized by the development of numerous nodules in the peritoneal cavity which histologically have the appearance of smooth muscle tumors. The ninth proven case of this ill-understood disorder is presented here, with comments on some of the other published instances. Most reported cases were diagnosed in pregnant women, and there is good reason to believe that endocrine factors are of importance in the development of this condition. Attention is drawn to experimental studies in which an apparently similar condition has been pr…

BACKGROUND: Intravenous leiomyomatosis (IVL), pulmonary benign metastatic leiomyomatosis (PBML), and leiomyomatosis peritonealis disseminata (LPD) are leiomyomas with special growth patterns and high postoperative recurrence rates. We report the safety and efficacy of a pilot study of sirolimus in the treatment of recurrent IVL, PBML, and recurrent LPD.
METHODS: This was a pilot study to evaluate the safety and efficacy of sirolimus in the treatment of leiomyomatosis (ClinicalTrials.gov identifier NCT03500367) conducted in China. Patients received oral sirolimus 2 mg once a day for a maximum …

BACKGROUND: Intravenous leiomyomatosis (IVL) is a rare and specific type of smooth muscle tumor that is histologically benign but has a malignant biological behavior. It is commonly associated with a history of uterine leiomyomas.
CASE PRESENTATION: A 36-year-old woman, G1P1, presented to the hospital with left lower abdominal pain for 2 months and she has accepted hysteroscopic myomectomy about 1 year ago. Ultrasound venography, echocardiography and computed tomography venography (CTV) of inferior vena cava were performed, which revealed IVL located in left intramural myometrium walls growin…

The clinical and pathological features of four cases of the rare entity diffuse leiomyomatosis of the uterus are described. The patients, who ranged in age from 28 to 35 years, presented with a six month to ten year history of abnormal uterine bleeding, dysmenorrhoea and an enlarged uterus. Hysterectomy in each case revealed a symmetrically enlarged uterus containing numerous small ill-defined leiomyomatous nodules. On microscopic examination the nodules were composed of compact fascicles and interweaving bundles of plump uniform benign smooth muscle cells. The nodules blended with each other …

Leiomyomas of an extrauterine nature are rare and often present a more challenging diagnosis process for the clinician. Our patient presented with lower abdominal pain associated with menorrhagia over a period of 6 months. Laparoscopic surgery revealed a right-sided broad ligament myoma consisting of multiple soft and diffused fibroids, of more than 400 in total. The myomas were enucleated and completely removed by laparoscopic surgery with minimal blood loss. Histopathological examination and analysis revealed the multiple nodules to be benign leiomyomatosis. The finding of isolated broad lig…

Laparoscopic imaging of intravenous leiomyomatosis is rare. We report a 35-year-old woman with uterine leiomyomatosis, laparoscopic hysterectomy exposed parametrial vessels with whitish plugs extending intravascularly toward the pelvic sidewall. Complete surgical resection and long-term follow-up are important.

BACKGROUND: Intravenous leiomyomatosis is a rare uterine tumor defined as an intraluminal growth of benign smooth muscle cells in either venous or lymphatic vessels outside the confines of, or even in absence of leiomyomas. We report a case of intravenous leiomyomatosis of the uterus managed at our institution.
CASE REPORT: The patient, a 47-year-old woman; gravida 2 para 2, presented without complaints for a routine gynecological examination. An irregular-shaped enlarged uterus with a nodular pelvic mass extending into the broad ligament was found Laparotomy revealed a large retroperitoneal,…

Intravenous leiomyomatosis (IVL) is a rare smooth muscle tumor. Although IVL is histologically benign, it might be aggressive in its behavior and can grow into pelvic veins and the inferior vena cava (IVC) extending into the heart chambers and pulmonary vasculature. Occasionally, it was found to have lung metastasis. We describe four cases of IVL in the IVC with a history of hysterectomy for uterine leiomyoma, one extending into the left renal vein and three growing into the right heart chamber. We report the computed tomography (CT) findings in the four cases and briefly discuss the CT featur…

A female patient aged 28 fell ill with manifestations of sensory aphasia, headache and signs of cerebral compression. Except for a bilateral papilledema there were no further pathological clinical and paraclinical findings. Later on, amaurosis, deafness, anosmia and generalized muscular hypotonia developed. The nuclear magnetic resonance image revealed a major accumulation of contrast medium in the leptomeninx. Biopsy demonstrated a mesenchymal neoplasm in the leptomeninx. After a strong rise in intracranial pressure, the patient died from a bulbar brain syndrome. Microscopy revealed a diffuse

Peritoneal leiomyomatosis (PL) occurs in women of reproductive age. Of 20 women with PL in this study, 10 were pregnant or immediately postpartum, and 7 were taking oral contraceptive steroids. Only three of the 20 women were not pregnant or taking exogenous hormones. All PL nodules were composed mainly of spindle cells, but decidual cells were admixed with the spindle cells in some of the nodules in six of the 10 pregnant and postpartum women. Electron microscopy was applied to the nodules from six women representing the three different hormonal backgrounds. All contained smooth muscle cells.…

Intravenous leiomyoma is a rare condition that occurs when there is a vascular invasion of a pre-existing uterine leiomyoma. The tumor can metastasize to structures such as the heart and lungs. We discuss a case of metastasis to the heart resulting in severe tricuspid regurgitation. Surgical intervention is the primary modality; usually a staged approach involving cardiac surgery along with abdominal and/or pelvic surgery. We want to highlight the importance of fully investigating right-sided cardiac masses. While there are common etiologies for these masses, one must maintain a high degree of
