Advanced Lemierre syndrome requiring surgery.

A 38-year-old homeless man was admitted with a 2-week history of a sore throat, increasing shortness of breath, and high fever. Clinical examination showed enlarged and tender submandibular and anterior cervical lymph nodes and a pronounced enlargement of the left peritonsillar region (Figure 1a). CT scan of the throat and the chest showed left peritonsillar abscess formation, occlusion of the left internal jugular vein with inflammatory wall thickening and perijugular soft tissue infiltration, pulmonary abscesses, and bilateral pleural effusions (Figures 1b-e, arrowed). Anaerobe blood culture

Cuffed-catheter-related Lemierre syndrome.

Sepsis is a common problem in patients who have cuffed catheters for hemodialysis. Line sepsis, however, has not often been associated with septic thrombophlebitis as seen in Lemierre syndrome. Thrombophlebitis of a central vein associated with catheter placement is extremely rare and when encountered is known to affect the femoral or the subclavian vein. Thrombophlebitis of the internal jugular vein after hemodialysis catheter placement has not been reported. We present the case of a woman with line sepsis who developed Lemierre syndrome, a serious complication of which the medical community

Lemierre syndrome and acute mastoiditis.

Lemierre syndrome seldom follows an episode of pharyngotonsillitis. Characteristically, it is comprised of septic thrombosis of the internal jugular vein and bacteremia, leading to lung emboli and metastatic abscess formation. We describe Lemierre syndrome that complicates an acute mastoiditis, with considerations regarding its pathogenesis and management. Despite its sporadic occurrence, awareness of Lemierre syndrome is important, since early recognition reduces both the morbidity and mortality associated with it.

Lemierre's and Lemierre's-like syndromes in association with infectious mononucleosis.

OBJECTIVE: This study aimed to review cases of Lemierre's and Lemierre's-like syndromes in paediatric patients, to examine a possible association with Epstein-Barr virus as a predisposing factor, and to assess the impact of this virus on the severity of illness. METHODS: We performed a retrospective analysis of data from the in-patient database at Winthrop University Hospital, from January 2001 to October 2007. We reviewed clinical and laboratory findings as well as the outcome of infection in patients aged 21 years or less with a diagnosis of Lemierre's syndrome. An additional case of Lemier

Is this a variant of Lemierre's syndrome?

Lemierre's syndrome is an oropharyngeal infection leading to secondary septic thrombophlebitis of the internal jugular vein (IJV). It is classically anaerobic in origin and Fusobacterium necrophorum is the commonest pathogen. We report an unusual variant of this condition in a 68-year-man with uncontrolled diabetes mellitus who presented with extensive cellulitis of the left neck. No primary oropharyngeal sepsis was detected. Neck radiographs revealed soft tissue gas and computed tomography confirmed neck space infection with gas formation and complete thrombosis of the left IJV. Klebsiella pn

Lemierre's syndrome (necrobacillosis).

Lemierre's syndrome or postanginal septicaemia (necrobacillosis) is caused by an acute oropharyngeal infection with secondary septic thrombophlebitis of the internal jugular vein and frequent metastatic infections. Fusobacterium necrophorum is the most common pathogen isolated from the patients. The interval between the oropharyngeal infection and the onset of the septicaemia is usually short. The most common sites of septic embolisms are the lungs and joints, and other locations can be affected. A high degree of clinical suspicion is needed to diagnose the syndrome. Computed tomography of the

[Lemierre-like syndrome, an emerging diagnosis].

Lemierre's syndrome is characterized by septic thrombophlebitis of the jugular vein, bacteremia, and septic embolisms, resulting from an oropharyngeal infection frequently caused by Fusobacterium. In recent decades, there have been increasing reports of patients with a variant called Lemierre-like syndrome. In these cases, the pathogen involved is Staphylococcus aureus, particularly methicillin-resistant strains. We report the case of a 64-year-old man with no comorbidities who presented with a chin abscess due to facial folliculitis. He progressed with bacteremia, external jugular vein thromb

Lemierre Syndrome With Streptococcus constellatus Bacteremia.

Lemierre syndrome is characterized by thrombophlebitis of the internal jugular vein (IJV) secondary to bacterial pharyngitis or tonsillitis. Though antibiotic use has made this a rarer syndrome, it can nevertheless manifest in patients presenting with pharyngitis. Herein, we describe a 20-year-old male patient with no relevant medical history presenting with signs concerning for pneumonia and was ultimately diagnosed with Lemierre syndrome with Streptococcus constellatus bacteremia. Complications included IJV thrombus with presumed septic emboli to the lungs. The patient was discharged on ampi

Complex Lemierre syndrome with multisystemic abscesses.

We present here the challenging case of severe Lemierre syndrome in a healthy woman in her late twenties, whose clinical presentation was characterised by lung abscesses and disseminated systemic abscesses in the brain, the abdomen and the soft-tissues, as a likely consequence of a patent foramen ovale. Blood cultures were positive for Fusobacterium necrophorum and a right lingual vein thrombosis was detected at a late stage when the patient developed a septic shock. Initial antimicrobial therapy with metronidazole and ceftriaxone was modified to meropenem due to progressive worsening. The pat

Lemierre Syndrome Complicated by Emphysematous Osteomyelitis.

Lemierre syndrome is characterized by severe pharyngitis, internal jugular vein thrombosis, and septic emboli. We present a case of emphysematous osteomyelitis secondary to Lemierre syndrome in a 27-year-old previously healthy man. Despite the high mortality associated with these conditions, full symptom resolution can be achieved with early diagnosis and aggressive management.

[Lemierre syndrome and influenza A (H1N1)].

Lemierre syndrome is an anaerobic bacteremia associated with a septic thrombophlebitis of the internal jugular vein. Septic emboli can be found in many organs. It often occurs after pharyngitis. Today, Lemierre syndrome is quite rare, but without rapid treatment, it may become life-threatening. A 4-year-old child presented with a febrile headache and torticollis. He was influenza A (H1N1)-positive. He also had beta-hemolytic streptococcal pharyngitis. A secondary CT scan was taken because of clinical worsening (non reducible torticollis). Parapharyngeal abscess and septic thrombophlebitis in t

Lemierre syndrome complicated by bronchopleural fistula.

We present a 19-year-old woman, a case of Lemierre syndrome, who presented with fever, sore throat, and left shoulder pain. Imaging revealed a thrombus in the right internal jugular vein, multiple nodular shadows below both pleura with some cavitations, right lung necrotizing pneumonia, pyothorax, abscess in the infraspinatus muscle, and multiloculated fluid collections in the left hip joint. After inserting a chest tube and administering urokinase for the pyothorax, a bronchopleural fistula was suspected. The fistula was identified based on clinical symptoms and computed tomography scan findi

Lemierre syndrome: Current evidence and rationale of the Bacteria-Associated Thrombosis, Thrombophlebitis and LEmierre syndrome (BATTLE) registry.

Lemierre syndrome is a rare but potentially fatal condition characterized by septic thrombophlebitis of the head and neck district, preferentially affecting adolescents and young adults and manifesting as a complication of a local bacterial infection - typically, a pharyngotonsillitis or an abscess. It is historically associated with the Gram-negative anaerobic rod Fusobacterium necrophorum and with thrombophlebitis of the internal jugular vein. However, its definition has never been firmly established, and its spectrum within the continuum of bacteria-associated thrombophlebitis may be larger

Lemierre's and Lemierre's-like syndromes in children: survival and thromboembolic outcomes.

OBJECTIVE: Lemierre's syndrome, or jugular vein thrombosis (JVT) associated with anaerobic infection of the head and neck and frequently complicated by septic pulmonary embolism (PE), has historically been described as a disease of young adults. In recent years, an increasing number of case reports of childhood Lemierre's syndrome have been published, focusing mostly on the clinical and laboratory findings at disease presentation and the outcomes of infection. Given the potentially life-threatening thromboembolic complications of this disorder, we reviewed our single-institutional experience w

Lemierre's syndrome--the syndrome quite forgotten.

A 17 year old male presented with seizures, headache, and fever and left chronic suppurative otitis media. A 35 year old male presented with headache, giddiness, vomiting, pain in eyes, diplopia and right chronic suppurative otitis media. Brain imaging in both revealed thrombosis of lateral and sigmoid sinus and also of internal jugular vein on the left and right side respectively. A diagnosis of Lemierre's syndrome was made in both. They were treated with antibiotics and anticoagulants, and they responded to treatment. We want to report this case as we feel, that with the advent of the antibi

Lemierre Syndrome After Influenza A Virus Infection.

Lemierre syndrome is a rare condition characterized by septic thrombophlebitis of the internal jugular vein with metastatic septic emboli, most commonly affecting the lungs. We report a case of Lemierre syndrome temporally associated with influenza A infection in a previously healthy 21-year-old male. The patient initially presented to the emergency department with severe headache and tested positive for influenza A. Over the following seven days, his symptoms progressed to neck pain with decreased range of motion, hemoptysis, cough, and persistent myalgias. Despite four additional emergency d
