Explantation of intraocular lenses.

Cataract surgery has evolved recently along with the development of intraocular lenses. With this evolution of intraocular lenses from anterior chamber and iris-fixated lenses to more modern foldable posterior intraocular lenses has come an evolution in the complications that necessitate removal of the intraocular lens. Early generations of anterior chamber and iris-fixated intraocular lenses often had severe complications associated with them, such as pseudophakic bullous keratopathy, uveitis-glaucoma-hyphema syndrome, and chronic cystoid macular edema. With modern foldable intraocular lenses

Keratoplasty and intraocular lenses.

This paper describes the surgical results in 163 patients in whom intraocular lenses (IOL) had been implanted, exchanged, or removed during penetrating keratoplasty (PKP). They were divided into the following groups: PKP, cataract extraction, and IOL implant; PKP in aphakia and IOL implant; PKP and IOL exchange; PKP and IOL removal. In all groups clear transplants were obtained in over 92% of the cases, with best optical and visual results obtained in cases of Fuchs' dystrophy with cataract extraction and IOL implant (98% clear grafts and vision better than 20/40 in most cases). There were no

Intraocular lenses in children.

The intraocular lens offers an experimental alternative to be considered for the visual rehabilitation of children with traumatic or unilateral infantile cataracts. The disadvantages of aphakic spectacles or contact lenses and amblyopia have committed many of these eyes to the status of visual cripples. In this series of 41 patients with a maximum of 24 months follow-up, it has been concluded that an IOL may be successfully implanted. I wish to reemphasize that vitreous involvement does not preclude IOL implantation providing all the vitreous is removed from the anterior chamber by the conclus

[Pseudophakic supplementary intraocular lenses].

Non-corrected residual refractive errors after cataract extraction and implantation of an artificial intraocular lens (IOL) lead to limited visual acuity or suboptimal usage of enhanced optical properties of IOLs, such as bifocal implants, which are dependent on postoperative emmetropia. In addition to the established corneal procedures for correction of ametropia (excimer laser surgery or limbal relaxing incisions) sulcus supported pseudophakic supplementary IOLs can also be implanted. These IOLs have been designed to deal with the problem associated with sulcus implantation, such as pigment

Trifocal intraocular lenses versus bifocal intraocular lenses after cataract extraction among participants with presbyopia.

BACKGROUND: Presbyopia occurs when the lens of the eyes loses its elasticity leading to loss of accommodation. The lens may also progress to develop cataract, affecting visual acuity and contrast sensitivity. One option of care for individuals with presbyopia and cataract is the use of multifocal or extended depth of focus intraocular lens (IOL) after cataract surgery. Although trifocal and bifocal IOLs are designed to restore three and two focal points respectively, trifocal lens may be preferable because it restores near, intermediate, and far vision, and may also provide a greater range of

Comparison of visual performance of multifocal intraocular lenses with same material monofocal intraocular lenses.

PURPOSE: To compare the visual performance of multifocal intraocular lenses (IOLs) and monofocal IOLs made of the same material. METHODS: The subjects included patients implanted with either Tecnis® monofocal IOLs (ZA9003 or ZCB00) or Tecnis® multifocal IOLs (ZMA00 or ZMB00) bilaterally. We conducted a retrospective study comparing the two types of IOLs. The multifocal group included 46 patients who were implanted with Tecnis® multifocal IOLs bilaterally. The monofocal group was an age- and sex-matched control group, and included 85 patients who were implanted with Tecnis® monofocal IOLs bila

[Presbyopia Correction with Intraocular Lenses].

Surgical correction of presbyopia can be performed by refractive lens exchange, replacing the lens with a monofocal, a multifocal or an extended-depth-of-focus (EDOF) intraocular lens (IOL). Option 1, a monofocal IOL, must be combined with monovision, otherwise presbyopia cannot be corrected. Monovision in this context means one eye for distance and one eye targeted for - 1.5 D. With this concept glasses are still required for near (30 - 40 cm) and for driving, but intermediate distances and everyday life is usually possible without glasses. Option 2, a multifocal IOL, results in spectacle ind

Multifocal intraocular lenses: An overview.

Multifocal intraocular lenses are increasingly used in the management of pseudophakic presbyopia. After multifocal intraocular lens implantation, most patients do not need spectacles or contact lenses and are pleased with the result. Complications, however, may affect the patient's quality of life and level of satisfaction. Common problems with multifocal lenses are blurred vision and photic phenomena associated with residual ametropia, posterior capsule opacification, large pupil size, wavefront anomalies, dry eye, and lens decentration. The main reasons for these are failure to neuroadapt, l

[Determination of toric intraocular lenses].

BACKGROUND: In the last decades, toric posterior chamber lenses (TPCLs) for cataract surgery and phakic toric lenses (PTLs) for refractive surgery have become more and more popular for correcting high or excessive corneal astigmatism. The purpose of this article is to present a vergence-based calculation scheme for TPCLs and PTLs. METHODS: In Gaussian optics (in the paraxial space), spherocylindrical optical surfaces can be described in a mathematically equivalent formulation as vergences. There are dual notations: The standard notation is used for transforming vergences through a homogeneous

Single optic accommodative intraocular lenses.

Currently, two single optic accommodative intraocular lenses are available for implantation. Both designs rely on the interaction of the ciliary muscle with the zonule and capsule to induce axial movement of the optic. This article reviews the historical development, laboratory data, and clinical findings relating to these interesting devices.

Vision with bifocal intraocular lenses.

The laboratory and clinical results of tests of different bifocal intraocular lenses (BIOLs) are compared. Included are two-zone refractive BIOLs with a central near zone and an annular distance zone (group 1); three-zone refractive BIOLs with a central distance zone, an annular near zone, and a peripheral distance zone (group 2); diffractive BIOLs (group 3); multizone refractive BIOLs (group 4); and other BIOLs (group 5). All BIOLs cause a significant loss of image contrast and acuity at low contrast. The incidence of visual side effects (e.g., halos, glare) varies but is significantly higher

Optical quality of intraocular lenses.

The ANSI Z80.7 standard for intraocular lens quality is being updated. This paper identifies areas that require revision and suggests that a clear distinction between the lens design and the quality control test methods be made. The standard monofocal lens design consists of two spherical surfaces, and lens power can be specified by the thick lens equation, using body temperature refractive index values at a wavelength of 550 nm. Interferometry might be used as a primary manufacturing test method for poly(methyl methacrylate) lenses; tolerance values for lens surface errors are suggested. Othe

Congenital cataract and intraocular lenses.

We implanted nine intraocular lenses for the aphakic correction of congenital monocular cataracts in eight children (eight eyes). During follow-up periods ranging from 18 to 50 months in six children, there were no major complications connected to the surgery. Parental cooperation with treatment for amblyopia was satisfactory in all cases. The degree of strabismus, the fixation pattern, and the optokinetic nystagmus responses improved postoperatively in all six. Three children old enough to cooperate during visual testing had visual acuities better than 20/200 and two of these had visual acuit

Premium intraocular lenses in children.

Multifocal and toric intraocular lenses (IOLs) or the so-called premium IOLs are currently widely used in adult patients as a one-step refractive solution following cataract surgery. However, the decision to implant a premium IOL in a pediatric patient involves multiple factors affecting the child's visual development and is associated with several dilemmas and surgical challenges. The purpose of this review is to summarize these factors and analyse the influence of each of them on the visual outcomes following premium IOL implantation. A review of literature was conducted using the relevant k

[Calculation of phakic intraocular lenses].

BACKGROUND: A high corneal astigmatism is a major limitation for the attainable visual acuity with spectacle or contact lens correction. In addition, people with higher ametropia may feel discomfort while wearing contact lenses or spectacles which leads to the desire for a permanent correction. Phakic intraocular lenses provide correction possibilities of higher spherocylindrical ametropies beyond those of corneal refractive surgery. METHODS: For the calculation of toric phakic lenses the matrix formalism for calculation of toric pseudophakic implants is applied. The methods are presented and
