Effectiveness of a community-based intervention to prevent childhood TB in Lesotho.

BACKGROUND: Child contact management (CCM) is a recognized strategy to prevent TB; however, implementation is suboptimal. PREVENT was a cluster-randomized trial that evaluated the effectiveness and acceptability of a community-based intervention (CBI) to improve CCM in Lesotho.METHODS: Ten health facilities (HFs) were randomized to CBI or standard-of-care (SOC). CBI included nurse training/mentorship, health education by village health workers (VHW), adherence support, and multidisciplinary team meetings. Information on TB cases registered from February 2016 to June 2018 and their child contac

A descriptive study of parasites detected in ticks of domestic animals in Lesotho.

Ticks are medically important and significant vectors of diseases affecting livestock, humans, and companion animals than any other arthropod vectors. In the absence of information on the relationship of tick species and piroplasms parasites in Lesotho, the current study was aimed at detecting piroplasms parasites of economic importance from ticks of domestic animals. A total of 322 pooled tick DNA samples were subjected to PCR screening for the presence of piroplasms. The overall infection rate of piroplasms was 7% with Babesia bigemina at 3.4% (11/322), B. bovis 0.3% (1/322), B. ovis 2.8% (9

Characterization and population genetics of Haemonchus contortus in Merino sheep in Lesotho.

Haemonchus contortus is the most pathogenic and economically restrictive gastrointestinal nematode in the small ruminant industry globally. Morbidity, poor cross-bodily state, and mortality of sheep in Lesotho suggest the presence of H. contortus. The present study investigated the morphological, molecular, and population genetics of H. contortus third-stage larvae infecting sheep in four ecological zones (EZ) of Lesotho. Coprocultures were prepared for larval morphological identification and PCR determination. Larvae were identified morphologically as 100% H. contortus. The Second Internal Tr

Epidemiology of Rabies in Lesotho: The Importance of Routine Surveillance and Virus Characterization.

Rabies is widespread throughout Africa and Asia, despite the fact that the control and elimination of this disease has been proven to be feasible. Lesotho, a small landlocked country surrounded by South Africa, has been known to be endemic for rabies since the 1980s but the epidemiology of the disease remains poorly understood due to limited sample submission, constrained diagnostic capabilities, and a lack of molecular epidemiological data. Considering the existing challenges experienced in Lesotho, we aimed to evaluate the direct, rapid immunohistochemical test (DRIT) as an alternative to th

From modeling to morals: imagining the future of HIV PREP in Lesotho.

Amidst growing global endorsements of new biomedical HIV prevention strategies, ARV-based pre-exposure prophylaxis (ARV PrEP) has garnered considerable attention as a potentially promising prevention strategy. Though it may offer more effective protection for certain at-risk groups than conventional prevention strategies (such as sexual partner reduction, condom use, and prevention of mother-to-child transmission), PrEP is more costly. PrEP requires more ongoing contact between individuals and providers, and a level of surveillance from the health system that is not necessary with other preven

An analysis of home-based oral rehydration therapy in the Kingdom of Lesotho.

Mothers in developing countries are being successfully taught to give an oral rehydration solution (ORS) at home. The quantity of oral rehydration fluid that mothers administer to their child remains a critical question. Inadequate quantities render oral rehydration therapy (ORT) ineffective. The primary focus of our investigation was direct measurement of the quantity of fluid unsupervised mothers gave their children at home. This study validates a methodology that provides precise information on ORS administration in the home, information essential for evaluating the effectiveness of home-ba

Temporal patterns of anthrax outbreaks among livestock in Lesotho, 2005-2016.

BACKGROUND: Although anthrax is endemic in Lesotho, limited information is available on the patterns of the disease among livestock animals. This study investigated temporal patterns of anthrax outbreaks and cases among livestock animals in Lesotho. METHODS: Secondary data of anthrax outbreaks reported to the Department of Livestock Services between January 2005 and December 2016 was used for this study. Proportions of anthrax outbreaks and cases, and their corresponding 95% confidence interval were calculated and compared across year, season, month and region using the Chi-square or Fisher's

Oral health knowledge, attitudes and behaviour among nursing staff in Lesotho.

The decreasing prevalence of oral diseases in industrialised countries is attributed to a paradigm shift of oral health services from being mainly curative to preventative care. In under-resourced countries, the use of non-oral health care workers in the promotion of oral health, can contribute substantially to improving oral health and the adoption of a multidisciplinary team approach in oral health is highly recommended. Owing to the inadequate number of oral health personnel in Lesotho, the use of health care workers other than oral health care workers has been investigated. Nurses are one

A multilevel analysis of the factors associated with minimum acceptable diets among children aged 6-23 months in Lesotho: a study of the Lesotho Multiple Cluster Indicator 2018.

BACKGROUND: The World Health Organisation's Infant and Young Children Feeding Guidelines (IYCF) have been adopted as an internationally acceptable complementary feeding guideline, known as the Minimum Acceptable Diet (MAD). The MAD is the combination of the Minimum Meal Frequency (MMF) and Minimum Dietary Diversity (MDD). MAD is not met in many countries in the world. This study, therefore, aims to investigate the prevalence and multilevel factors associated with a minimum acceptable diet among the children aged 6-23 months in Lesotho. METHODS: The study used the Lesotho Multiple Cluster Indi

Effect of girls' education on cancer awareness and screening in a natural experiment in Lesotho.

Breast and cervical cancers are important causes of disability and premature death among women in Sub-Saharan Africa. Previous research has linked girls' education to cancer service access. Here, we examine the causal effect of girls' educational attainment on cancer screening practices by means of a natural experiment in Lesotho. In particular, we exploit variation in educational attainment among women that was introduced by an educational policy (a school-entry age cut-off). Data on awareness towards breast cancer, knowledge of Pap smear, breast self-exam, breast clinical exam, and having re

Emergence of Acquired Dolutegravir Resistance in Treatment-experienced People With HIV in Lesotho.

BACKGROUND: Since 2019, the World Health Organization has recommended dolutegravir-based antiretroviral therapy (ART) as the preferred regimen for human immunodeficiency virus management. Large-scale programmatic transitioning to dolutegravir-based ART was subsequently implemented across Africa, often in the absence of recent viral load testing and without access to genotypic resistance testing (GRT) in case of viremia. METHODS: This study assessed emerging dolutegravir resistance in the routine care of the Viral Load Cohort North-East Lesotho. We included pediatric and adult participants who

Cancrum oris in HIV infected children in Lesotho: report of two cases.

Cancrum Oris (Noma) has been known to mankind since time immemorial. Its public health importance is lately being realised especially in Africa, where majority of the cases occur. It affects those whose immune systems are compromised and thus, it is no wonder that it is being reported in HIV infected individuals. This development could have serious implications regarding the impact of the HIV pandemic globally and in particular the African region where poverty is rife.

A diphtheria epidemic in Lesotho, 1989. Did vaccination increase the population's susceptibility?

We report a diphtheria outbreak which occurred in the 1st 6 months of 1989 in the Quthing district of Lesotho in Southern Africa. Sixty-eight clinical cases were identified in patients with ages ranging from 14 months to 51 years. The 10-15-year age group represented 38% of the cases and the 15-20-year age group another 26%. Age-adjusted incidence rates were higher in patients aged between 15 and 35 than in those aged 0-15 years (p < 0.001). The overall case fatality rate was 23%. Most deaths occurred in the age range 10-14 years, but the case fatality rate tended to be higher in the younger a

The experiences of doctors in Lesotho during the COVID-19 pandemic: A qualitative study.

The COVID-19 pandemic dramatically increased pressure on healthcare staff who were trying to manage and adjust to the novel situation. For many healthcare workers, this came at a cost to their wellbeing. However, as research is predominantly focused on the Western world, less is known about the experiences of healthcare staff in places such as Africa. Using qualitative interviews, this study investigated the experiences of seven doctors in Lesotho regarding their wellbeing during the pandemic. Through interpretative phenomenological analysis, five themes were identified: under pressure, unprep
